[Inflammatory (infectious) complications in arteriovenous fistulae for hemodialysis].

Angioaccess procedures for hemodialysis over a 12-year period were retrospectively reviewed to ascertain the frequency of infectious complications. A total of 571 angioaccess procedures were performed including 88 e-polytetrafluoroethylene (ePTFE) grafts. Early infectious complications occurred only in 5 autologne fistulae. Late infectious complications developed in 2.3% autogenous fistulae and in 7.95% ePTFE grafts. Infected arteriovenous fistulae can be the cause of further complications (thrombosis, bleeding, aneurysm, sepsis) and call for surgical repair.

[Use of sulperazone in acute surgical conditions].

In the period between February 2, 1988 and October 30, 1998, the investigators administered Sulperazon to 20 patients at the Surgery Department 1, University Hospital Olomouc, in whom they expected good clinical effect (i.e., high concentration in bill, effect on respiratory tract pathogens). Sulperazon was administered in 12 cases as a drug of first choice, in 8 cases as a drug of second choice. In all cases, relief from clinical symptoms of infection was observed within 5 days of treatment and in 83% of cases eradication of previously cultivated pathogen was achieved. Sulperazon was well tolerated and no side effects or impoivment of laboratory test were observed. In 4 cases, prolonged administration was complicated by candida superinfection. Sulperazon proved to be effective in surgery in treatment of infectious complications of bile duct and pancreas diseases and in thoracic cavity pathology.

[Aneurysm as a complication of arteriovenous anastomoses for hemodialysis].

Angioaccess procedures for hemodialysis over a 12-year period were retrospectively reviewed to ascertain the frequency of aneurysm complications. A total of 571 angioaccess procedures were performed including 88 polytetrafluoroethylene grafts. 1.24% autogenous fistulae and 5.7% ePTFE grafts had uncomplicated aneurysm. Complicated (thrombosis, bleeding, infection) aneurysm occurred in 5.2% autogenous fistulae and in 5.7% PTFE grafts. The authors recommend a surgical repair of all angioaccess-associated aneurysms in the time of uncomplicated occurrence. If the aneurysm is complicated, the risk of the function lose of the av shunt by surgical therapy increase.

[Indications for enteral nutrition in surgery].

Survey of basic indications of enteral nutrition in Surgery. 1. Application in preoperative preparation. 2. Postoperative preparation in malnutritive and critically-ill patients, at early stage. 3. EN applied also for accelerated motility of the stomach and bowels after major operations (in retroperitoneal operations). 4. Application in traumatology: polytraumas, burns, subsequent sepsis, multiorgan failure; also in specific therapy, e.g. traumatic fistulae of GIT. 5. Use in acute and chronic pancreatitis. 6. Use of enteral probe in special states (e.g. in treatment of fistulae in upper and lower parts of GIT (dehiscence of anastomosis, bowels ruptures). 7. Application in the syndrome of short intestine. 8. Use in reconvalescence after severe operations (sipping) 9. Application in terminal cachectic states.

[Hyperfunctional arteriovenous anastomosis and complications in hemodialysis access].

A less frequent late complication of arteriovenous fistulas for haemodialysis is the development of a hyperfunctional fistula with ischaemia of the peripheral part of the extremity. These complications affect autologous fistulas as well as those created by the graft. Ischemia is much more common when the brachial artery is used as the flow vessel. For prevention the size of the arterial anastomosis of the shunt is of basic importance. The authors evaluate therapeutic possibilities of this complication.

[Significance of monitoring cytokine levels in the prognosis of acute pancreatitis].

The importance of cytokines as mediators and immunomodulators is increasing in all disciplines. Acute pancreatitis is a disease where the trigger mechanisms of the inflammation and thus also the influence of cytokines are very closely associated and are therefore apparent. The authors investigated on that model the levels of IL-2, IL-6, IL-8, IL-2r and neopterin in relation to the stage of the disease and time. They provided evidence in a group of 33 patients that the cytokine level indicates very accurately the stage of the disease and thus also its prognosis. For the assessment proper of the diagnosis the cytokine level is less important as the rise of the level is quite unspecific.

[Monitoring of selected biochemical markers of tissue ischemia, particularly of the digestive tract, in experiments and clinical practice].

The increase of organ transposition ill tile reconstruction of digestive tract, particularly transposition of the stomach and colon, is essential ill gullet surgery. The transposition is always accompanied by certain bigger or less degree of ischemia. In this study the authors followed the biochemical markers of tissue ischaemia, capable to lay down both pre- and postoperative vitality of the tucked organ an the anastomosis. Disclosing of the initial and later stadium of intestine ischemia is tile aim. Monitoring of the lactate in the serum is very useful. Its using was proved in the rich experimental group and it was certified in clinical practice.

[Establishment of arteriovenous anastomoses with interposition of ePTFE (Goretex) prostheses].

The authors remind of the possibility to establish an arteriovenous anastomosis by interposition of an ePTFE prosthesis into the subclavian to brachial artery with an outflow into the subclavian or internal jugular vein. The method is suitable for case when the autologous circulation is exhausted and calls for perfect peroperative evaluation of the vascular system of the patient with subsequent careful monitoring of the function of the anastomosis.

[Erythropoietin and erythropoiesis after kidney transplantation].

In a group of 15 patients after the first transplantation of a cadaverous kidney the authors monitored for the period of one month the serum erythropoietin concentration (Epo), haemoglobin levels (Hb), the haematocrit (Ht) and serum creatinine (Cr). The objective of the investigation was to evaluate in relation to serum concentrations of Epo and the function of the graft. Erythropoietin was examined by the authors own method before and after transplantation during the first 10 days daily and then on the 12th, 15th, 20th, 25th and 30th day. Normal values are 22-44 mU/ml. The mean Epo value before transplantation was 41.9 +/- 18.8 mU/ml. After transplantation the highest mean value of Epo was recorded on the second day when it reached a value of 102.8 +/- 144 mU/ml. The mean Epo serum concentration at the end of the study was 45.9 +/- 20.9 mU/ml and did not differ significantly from values recorded before transplantation. Between Hb and Ht values and Epo serum concentrations no correlation was revealed. Conversely, statistical analysis revealed a close inverse relationship between the drop of creatininaemia and the rise of Hb and Ht values. From the results of the investigation ensues that the decisive condition for the onset of the erythropoietic response after renal transplantation is adjustment of the milieu interiéur, induced by the restored excretory function of the graft which creates prerequisites for a normal response of the bone marrow to Epo.

[Stenosis and thrombosis of the central venous tract as a cause of manifestations of venous hypertension after establishment of an arteriovenous anastomosis for hemodialysis].

The authors draw attention to stenoses and occlusions of the central venous tract (subclavicular and brachiocephalic vein) which may be the cause of manifestations of venous hypertension on the upper extremity after establishment of an arteriovenous anastomosis for haemodialysis. In patients where this condition is suspected, careful preoperative examination of the venous circulation is essential. Possible treatment is discussed.

[An instrument for hepatic artery compression in palliative treatment of liver metastases using repeated ischemia. A model and experimental study].

Method of interrupted ischaemization of the liver afflicted with metastases from colorectal cancer is not definitively evaluated yet. Using the literary reports the authors proposed (in cooperation with MEDIN) a instrument for compression of the liver artery, economically easy available. The reability of this instrument was tested in model conditions and his safety to the tested artery was proved on the experimental animal.

[Retrosternal esophagocoloplasty as current means of surgery for esophageal carcinoma].

The authors present an account of their experience with retrosternal oesophagocoloplasty as a palliative by pas in inoperable carcinoma of the oesophagus. They differentiate secondary and primary indications. So far they used this operation in 27 patients. The total lethality is 33%, the mean survival period is 8 months. Some patients with the unilateral paresis of the left recurrent nerve have laryngoesophageal dysfunction with the danger of aspiration during swallowing. The shape, position and function of the paretic vocal cord can be favourable influenced by the administration of hydrophile gel into the vocal cord by the endoscopic route.

Some reflections about portosystemic shunt in portal hypertension.

The authors report on the group of 85 patients with portal hypertension in whom a portosystemic shunt was established for uncontrolled bleeding from varices--in the period from 1968 to 1993. A total lethality was 9.4%, in planned operations 5.9%, in acute interventions 22%. Some aspects of shunt-operation are discussed (prehepatic portal hypertension, acute shunt, shunt preceding liver transplantation). It is concluded that the portosystemic shunt still plays a role in the therapy of complicated portal hypertension, although only as a selective intervention.

Analgesia after intercostal thoracotomy.

At present, several possibilities of postoperative analgesia have been known in the case of thoracotomy. Most frequent is the bolus administration of various analgetic drugs, continuous analgetic infusions, or, now more preferred the epidural application of analgetic drugs. The authors compared two groups of patients subjected to intrathoracic surgery and hospitalized at the Ist Surgical Clinic in Olomouc. The first group (23 patients) was given analgetics by the classical method (bolus administration), to the second group (15 patients) epidural morphine was applied. No significant differences were found between the both groups. Postoperative complications (e.g. atelectasis) did not occur in any group of patients, values of blood gas according to Astrup were within normal limits. The day of the operation was subjectively better tolerated in the second group but there were some insignificant complications related to morphine administration (vomiting, urine retention) even in 60% of cases.

[Late complications for hemodialysis specific arteriovenous fistulas].

Late complications of the arteriovenous fistulas for hemodialysis occur after the AV fistulas development and functioning. The following complications are frequent: stenoses and late thromboses, hemorrhage, inflammatory complications, aneurysms and undesirable hemodynamic consequences of the fistulas. A good functioning of the fistula requires an early diagnosis of the complication and a prompt surgical intervention at the clinic where these operations are made routinely.

Oesophagus carcinoma amino acids control.

Serum amino acids level examination of the oesophagus carcinoma patients admitted to I Surgical Clinic of Teaching Hospital in Olomouc from 1/1/1990 to 31/7/1991 were carried out by the authors. The standard values were found with the group of patients with resectionable tumor or with a tumor in non-advanced stage on the one hand and on the other hand there were some lower values with the patients with a tumor in an advanced stage. In this way two groups of the sick were examined--the first one with benign oesophagus stenosis accompanied by nutritional disorder, the second with resectionable lung carcinoma without nutritional disorder. Serum amino acids level examination is therefore considered to be rather an indicator of the important factor of a possible surgical treatment of a tumor.
