Choroidal melanocytic tumor observed for 41 years before enucleation.

A pigmented choroidal lesion was observed 41 years prior to enucleation. The patient, with a clinical diagnosis of malignant melanoma in an eye with good visual function, was followed up for the last 25 years by one of the authors. Sudden growth of the lesion and loss of vision led to enucleation and allowed histologic confirmation of the diagnosis of malignant melanoma. A pigmented iris lesion in the same eye was also present for 25 years and showed no notable growth under observation. The patient died of metastatic disease approximately one year after enucleation.

Isolation of Neisseria meningitidis in urogenital/rectal infections.

In 9 cases (6 men, 3 women) during 1975 we have isolated N. meningitidis from routine samples from urogenital/rectal locations to be examined for gonococci by culture. One of the men harboured meningococci in the rectum, the other 5 in the urethra. Two of the women had meningococci in the cervix and one in the urethra. Of the meningococci isolated, one belonged to serogroup A, 6 to group B and 2 to group C. Six of the patients, 4 men and 2 women, who attended the Department of Venereal Diseases, are described in greater detail. Of these, only one had symptoms. The meningococci disappeared spontaneously in 5 cases, whilst a man with unsymptomatic infection still harboured meningococci in the rectum after one month. Two of the 4 men were homosexual.

Ventilating tubes in the middle ear.

One hundred children from 1 to 14 years of age had serous otitis media, the manifestations of which were bilateral in all but four patients. Thus, 196 ears were treated with aeration of the middle ear by means of a short transmyringeal tube. The mean duration of the treatment was 3.4 years, and the mean observation time was 7.3 years. Of the ears treated, 79.1% regained normal hearing, though scarring of the tympanic membrane and smaller adhesions were observed. The result was not satisfactory in 20.9% of the ears treated, the hearing losses varying from 25 to 60 dB. In 7.7% (15 ears) chronic otitis media developed; in 5.6% (11 of the 15 ears) cholesteatoma was confirmed surgically. Adhesive processes were present in 11.2%.
