Plasmacytoma of the colon: case report with lambda light chain, demonstrated by immunoperoxidase studies.

A case of solitary extramedullary plasmacytoma in the transverse colon of a 20-year-old woman is reported. This case is unusual in that the plasmacytomas are only rarely observed in the gastrointestinal tract and are especially uncommon in the colon and in young women. The lesion was composed of pleomorphic plasma cells and contained areas of extracellular amyloid deposition. Its neoplastic nature was verified by the use of immunoperoxidase studies which are positive only for lambda light chain. The subject of gastrointestinal plasmacytomas and the use of immunoperoxidase studies on plasmacytic lesions are discussed.

The pathology of Yersinia enterocolitica ileocolitis.

The terminal ileum and proximal colon were resected in two children presenting with fever and right lower quadrant abdominal pain. The pathological findings were characteristic; elongated ulcerations with underlying lymphoid hyperplasia involved the distal ileum and smaller punctate aphthoid ulcers involved the distal ileum and colon. Transmural inflammation was seen in both cases and resulted in thickened, erythematous distal ileum mistaken at surgery for Crohn's disease. Yersinia enterocolitica was recovered from operative cultures of both the specimens. One of the two patients had a 1:1280 serological titer for Yersinia enterocolitica 1 week after surgery. The pathologic findings of Yersinia enterocolitica ileocolitis are distinctive and easily differentiated from Crohn's disease and other pathologic processes in this region. Operative diagnosis can be difficult but if the possibility of Yersinia infection is considered, the correct diagnosis can usually be made without unnecessary surgical resection.

Cytological diagnosis of pulmonary cryptococcosis.

In six cases of pulmonary cryptococcosis cytological studies helped to establish the correct diagnosis. With the introduction of new diagnostic techniques, such as fine-needle aspiration and fiberoptic bronchoscopy, preoperative and antemortem diagnoses of pulmonary cryptococcosis will be made more frequently. It is important to be familiar with the cytological appearance of the organism and its importance in sputum or aspirated material.

Lymphoid hypophysitis in a patient with hypoglycemic episodes.

Destructive inflammatory processes of the anterior pituitary gland are unusual and are rarely recognized either during life or at autopsy. A woman with recurrent episodes of hypoglycemia was found on autopsy to have a destructive lymphoid infiltrate in her adenohypophysis that appeared to result in only partial hypopituitarism. This lesion is thought to be an autoimmune disorder and has been reproduced experimentally. Four other cases have been reported previously in the world literature and are reviewed in this paper.
