Use of clonal assay in determination of urothelial drug sensitivity in carcinoma in situ of the bladder: clinical correlations in 5 patients.

Five patients with carcinoma in situ of the bladder, including 4 with associated grossly apparent tumors, were followed for 2 years or until invasive disease appeared while receiving intravesical chemotherapy. In vitro drug sensitivity determinations were made using the clonal assay. Of 9 retrospective and prospective in vitro/in vivo drug sensitivity correlations 7 proved correct. However, in vitro growth occurred in only 50 per cent of the urothelial samplings in which adequate cells were present. The assay has some potential use in selecting agents for intravesical chemotherapy but the problem of unpredictable and limited in vitro growth must be overcome.

Congenital ureteral valves: report of 2 patients, including one with a solitary kidney and associated hypertension.

We report 2 cases of congenital ureteral valves treated successfully by excision of the involved segment and ureteroureterostomy. Hypertension in 1 patient and recurrent urinary tract infection in the other prompted initial evaluation. In the hypertensive patient renin collections from the vena cava and the renal vein of the solitary kidney were normal. Nevertheless, relief of obstruction has resolved the hypertension for 3 years. We review the literature regarding ureteral valves and discuss the mechanism by which unilateral hydronephrosis probably produces hypertension.

An in vitro clonal assay for bladder cancer: studies of the biologic potential of the urothelium and determination of in vitro sensitivity to cytotoxic agents.

We have applied an in vitro colony-forming assay system to the study of primary urothelial explants obtained transurethrally by bladder barbotage and tumor biopsy from 91 patients. Urothelial cells obtained by bladder barbotage from patients with bladder cancer and from a "control" group of individuals exhibit differential capacity to clone in agar. Of samples from bladder cancer patients, 88% formed colonies, whereas only 27% of these from a "control" group did so. Of samples from a "suspicious" patient group, 70% formed colonies. In vitro drug sensitivity studies of cells obtained by biopsy of solid tumors demonstrate a spectrum of drug sensitivity to cytotoxic agents.

Massive nontraumatic hematuria: a challenge demanding immediate action.

Massive nontraumatic hematuria threatens the life of the patient and presents a complex management problem for the clinician. A rational approach to the condition demands rapid initial assessment followed by specific diagnosis of the cause of bleeding and appropriate corrective steps.

Normal saline slush preparation for renal surgery.

We describe a simple, rapid method to prepare normal saline slush with commercially available slush mugs that are gas-sterilized in routine fashion and stored in freezer. Slush can be prepared intraoperatively in 10 minutes with this method.

Development of an agar-methyl cellulose clonogenic assay for cells in transitional cell carcinoma of the human bladder.

We report the development of a clonogenic assay for progenitor cells in transitional cell carcinoma of the bladder. Colony growth has been demonstrated from cells obtained both from surgical biopsies and from bladder barbotages. Electron microscopic and karyotypic evidence supports the contention that these progenitors represent a part of the population maintaining the tumor in vivo. Colony growth occurred in 9 of 11 surgical biopsy samples and in 6 of 6 bladder barbotage samples. Plating efficiency ranged up to 0.7%, and colony size was in some instances greater than 1000 cells. The assay appears potentially useful for analysis of the biology of human transitional cell carcinoma.

Unusual presentation of genital tuberculosis.

We report two cases of unusual presentation of genital tuberculosis. One patient had an enlarging perineal mass as the only clinical evidence of genitourinary tract involvement. The second had a testicular mass difficult to distinguish clinically from testicular tumor. There was no evidence of upper tract disease.

Morbidity and mortality of renal exploration for cyst.

Surgical exploration for a renal mass is a procedure associated with a low mortality and morbidity, even in a population with a median age of 58 years, most of whom were listed according to the American Society of Anesthesiologists as class II or greater. As a consequence, the procedure warrants serious consideration as a diagnostic measure. Nonsurgical diagnostic procedures are associated with less immediate risk to the patient but with less immediate risk to the patient but with more limited accuracy. The decision as to which methods of evaluation to use in each particular patient must be individualized. The general and related symptoms, physical and laboratory findings and the quality and results of roentgenography and puncture studies of the patient warrant consideration in recommending a reasonable course. Whatever procedures are recommended, the patient should be adequately informed to recognize that alternative treatment approaches are available.
