[Early diagnosis of acoustic neuroma by the vestibular tests. Indication for computerized tomography and cisternomeatography (author's transl)].

In a series of 390 cases with suspicion of acoustic neurinomas 78 such tumors could be diagnosed, including 12 early stage neurinomas. This relatively high detection quote of small neurinomas is due to a special diagnostical programme: Every patient with unilateral and sensorineural hearing loss, independent of vertigo anamnesis or of the result of x-rays must be further examined by a vestibular tests. All 78 patients with acoustic neuroma had pathological vestibular findings. The positional test turned out to be the most sensitive examination in the early diagnosis of acoustic neuromas and yields a still higher incidence than the thermic test: 95% of the patients with a neuroma showed pathological findings in the positional test. Every patient suffering from an unidentified unilateral and sensorineural hearing loss combined with a pathological result in the positional tests must be further checked by a cisternomeatography or computerized tomography using airinsufflation. Every fifth of these patients showed typical signs of an acoustic neuroma in the neuroradiological tests. 68 neuromas are operated today and verified histologically, 10 patients are still waiting for surgical treatment.

Neurotological diagnosis of vestibular disorders in children.

The most marked findings in a group of 66 children were a spontaneous nystagmus presenting in 29% of the cases, and an abnormal caloric reaction in 40%. For qualitative and quantitative assessments of the positional test and caloric stimulation, at our University ENT department at Erlangen we use the positiogram and frequency calorigram described here, which are particularly suitable for follow-up observations and for the purpose of providing an expert medical opinion.

[Vestibular results before and after neurectomy of the vestibular nerve in cases of vertigo resistant to medical treatment (author's transl)].

25 patients suffering from vertigo due to peripheral-vestibular disorders resistant to medical treatment underwent neurectomy of the vestibular nerve. Präoperatively 24 showed a pathological result in the neurotological examination. In 83% of the cases an abnormal caloric reaction could be seen in the frequency-calorigram, which graphically represents the caloric test. The postoperative objective vestibular results always correlated with the subjective feeling of vertigo. We could see a decrease and finally a disappearance of the horizontal unidirectional spontaneous nystagmus. It is of value to examine the gaze nystagmus in none directions of gaze. In the positiogram, a quantitative scheme of the positional test, one could recongize a decrease of the nystagmus intensity in the follow-up examination. The same was found by testing the vestibulo-spinal reactions. Although the neurectomy of the vestibular nerve involves a serious operation it is nevertheless the first choice of treatment. 96% of the cases are free from vertigo. Postoperative there is a very low complication quota.

[Evaluation of the fistula test and the positional test in the diagnosis of a fistula of the labyrinth (author's transl)].

111 patients with a chronic otitis media and symptoms of dizziness were investigated. Out of 75 patients, who were operated, a fistula of the labyrinth was suspected in 25 patients due to a positive fistula test or a positive (fistula) positional test. A fistula was found in 15 patients. In those 15 cases both the fistula and the positional (fistula) test were positive in nine cases. In the remaining six cases three had a positive fistula test and three a positive (fistula) positional test. The other 50 patients all with negative fistula reactions one fistula was whereas found by the operation. This would indicate, that a positive fistula test and a positive (fistula) positional test are of value in the preoperative evaluation of patients with chronic otitis media and symptoms of vertigo.

[Evaluation of the fistula test and the positional test in the diagnosis of a fistula of the labyrinth (author's transl)].

In 111 patients with a chronic otitis media and symptoms of dizziness the positional reactions were examined. Among 75 patients, who underwent operations later on, a fistula of the labyrinth was preoperatively expected in 25 patients according to a positive fistula pressure sign or a pathological fistula positional reaction. A positive fistula positional sign shows a contralateral beating transitory nystagmus in the head hanging position, while after rapid sit up the nystagmus is starting to beat into the ipsilateral direction. A labyrinthine fistula was operatively confirmed in 15 patients out of 25 suspicious cases. In these 15 cases both the fistula and the positional test were positive in 9 cases. 3 had a positive fistula pressure test and 3 a positive fistula positional sign. In 50 patients with negative fistula reactions only one fistula was observed during operation. These data underline the importance of both pressure and positional reactions in the prediction of labyrinthine fistulas.

[For the documentation of the results of the positional test a new schematic pointer diagram is recommended, the positiogram (author's transl)].

It contains six fields representing the six main positions of the test. In each field the intensity and the duration of the positional nystagmus are given by the coordinates of the maximal frequency as abscissa and of the duration as ordinate. The direction of the nystagmus is arrowed by the direction of the resulting pointer. The survey on all the six fields informs about the pertaining nystagmus direction, its intensity and positional type. The positiogram enhances a quick evaluation of the positional test and facilitates follow-up studies.

[The frequency-calorigram an analogous representation of the caloric nystagmus reaction].

A new diagram is proposed for the analogous representation of the vestibular reactions to carloric stimulation (calorigram). It provides a centripetal scale for both ears with exchangeable parameters. The response is given by pointers representing both the intensity and direction of the nystagmus. The arrangement facilitates the visual recognition of typical vestibular disorders, and allows the synoptical comparison of follow up-tests. Its usefulness was proved by the application in 1100 caloric examinations, in which the maximal postcaloric frequency of the nystagmus was evaluated from the ENG and recorded in the frequency calorigram.
