Dysfunction of the bladder neck: a urodynamic study.

Urodynamic investigations were performed in 9 males, suffering from dysfunction of the bladder neck (detrusor bladder neck dyssynergia). The parameters of micturition were compared with the values in normal males and in patients with prostatic obstruction. By introducing a new factor, the maximum opening time, we found a characteristic pattern in dysfunction of the bladder neck, making selection of patients for endoscopic incision of the bladder neck possible. The maximum and mean flow rates increased significantly in 5 patients treated with incision of the bladder neck. The diagnostic value of voiding cystourethrography and urodynamic investigation is discussed.

The standardization of terminology of lower urinary tract function.

The terminology of lower urinary tract function will be standardized by the ICS. This report contains the recommendations dealing with urinary incontinence, procedures related to the evaluation of urine storage (cystometry, urethral closure pressure profile) and units of measurement.

Residual kidney function after unilateral nephrectomy. Pre- and postoperative estimation by renography and clearance measurements.

The residual kidney function was predicted from preoperative renography and determination of the glomerular filtration rate (GFR) in 57 patients undergoing unilateral nephrectomy for cancer, postrenal obstruction or renovascular hypertension. Postoperative GFR measurements were carried out 6-36 months after the operation. In eleven patients where the kidney removed had no function, no significant difference was found between pre- and postoperative GFR values. In 46 patients where the kidney removed had some function, the preoperative estimate was a little too high in only two patients. In this group, the postoperative GFR on an average amounted to 42% above the preoperatively predicted value. We conclude that the combination of 51Cr-EDTA clearance and renography is a reliable, non-invasive method for determination of the minimum residual kidney function before unilateral nephrectomy is carried out.

Treatment of urinary incontinence by the Scott-Bradley-Timm artificial sphincter. A report of eight cases.

Eight patients were treated with an implantable artificial urinary sphincter. Five patients had neurogenic bladders, two had postprostatectomy incontinence and one severe recurring stress incontinence. The main problem was mechanical failure of the device in four patients. The present results (follow up two to ten months) may be classified as satisfactory in six and unsatisfactory in two patients.

Morphology and function of the bladder and urethra in female urinary incontinence.

The conventional investigations, gynaecological examination and cystoscopy give very little information about the causes of incontinence. History taking makes it possible to divide the symptoms into either stress- or urge incontinence. Cystometry, lateral voiding cysto-urethrography and urodynamic examination including sphincter EMG enable us to divide the pathological conditions into suspension defects and bladder dysfunctions. The suspension defects are bladder base insufficiency, vesical descensus and trigonocele. The operation of choice for these conditions depends on whether the suspension defect is anterior or posterior. The bladder dysfunctions are supranuclear and infranuclear neurogenic dysfunctions. The treatment in these cases will ordinarily be medical.

Torsion of the testis diagnosed by ultrasound.

An ultrasonic method to diagnose acute testicular torsion is presented. With the doppler principle the presence of absence of intratesticular arterial pulsation can be demonstrated. In 5 patients with acute testicular torsion and 10 patients with epididymitis a correct ultrasonic diagnosis was made. In long-standing torsions some arterial pulsation may be present.
