[Serum levels and gene expressions of interleukin-1 beta (IL-1 beta), interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-alpha) in human renal cell carcinomas].

We examined serum levels of interleukin-1 beta (IL-1 beta), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha) in 32 patients with renal cell carcinoma. Serum IL-1 beta was elevated in only one patient (3.1%), IL-6 in 17 (53.1%) and TNF-alpha in 6 (18.8%). Positive rate of serum IL-6 increased with progression of the stage (stage I and II: stage III and IV = 14.3%: 88.9%, p < 0.0001) and was higher for rapid growth type (slow and intermediate growth type: rapid growth type = 36.4%: 90.0%, p < 0.005). However, increased serum levels of IL-1 beta and TNF-alpha were not correlated with stage, grade, or growth type. Gene expressions of IL-1 beta, IL-6, IL-6 receptor and TNF-alpha in the tissues resected from 15 patients with renal cell carcinoma and 3 cell lines established from renal cancers were evaluated by Northern blot analysis. Gene expression of IL-1 beta and TNF-alpha were not detected in any of the tissues or cell cultures, but highly enhanced expressions of IL-6 were revealed in two tumor specimens (13.3%, 1 rapid and 1 slow growth type) and 2 of 3 cell lines. Expressions of IL-6 receptor in tissues and cells were weak. These results suggest that IL-6 plays an important role in the progression of renal cell carcinoma, and that measurement of IL-6 in patients with renal cell carcinoma may be useful clinically.

[Endoscopic findings of nipple fixation in Kock pouch].

Fixing the nipple to the pouch wall is indispensable to reduce the incidence of late complications of the Kock pouch. We performed endoscopy of the Kock pouch in 28 patients to evaluate the nipple valve fixation. We carried out submucosal fixation of nipple in 17 patients, fixation using staples in 2 patients and full layer fixation in last 9 patients. On endoscopy, both efferent and afferent nipples were poorly fixed or completely detached in 8 of the 17 patients who underwent submucosal fixation. On the other hand, there were no patients in whom the nipple was poorly fixed or completely detached among the other 11 patients. We experienced nipple malfunction in only one of the last 9 patients. Based on endoscopic findings and clinical results, we concluded that full layer fixation was reliable and useful to reduce the incidence of nipple valve complications.

[The effect of glycosaminoglycans on calcium oxalate crystal formation].

The effect of glycosaminoglycans on calcium oxalate crystal formation in the supersaturated solution was studied by examining the size and shape of calcium oxalate crystals generated under an optical microscope. It was found that heparan sulfate and heparin were more effective growth inhibitors than chondroitin sulfate and hyaluronic acid at concentrations within their respective urinary range. With increasing calcium and/or glycosaminoglycans concentration in the solution, the degree of growth inhibition caused by glycosaminoglycans was enhanced. Calcium oxalate crystal shapes generated with various glycosaminoglycans varied with glycosaminoglycan species. One of the causes of those differences in the shape and degree of growth inhibition might be the structural differences between them, that is, the number of sulfate residue and O- or N-form they contain. Calcium oxalate crystal shapes in the presence of heparin or heparan sulfate at higher concentrations were similar to those of calcium oxalate monohydrate crystals in the urinary sediments of hyperoxaluric patients. These facts might suggest the possibility that heparin and/or heparan sulfate were present in the crystal forming region.

[Acute renal failure after renal trauma in a case of a functional solitary kidney].

Herein we report a case of acute renal failure after renal trauma in a patient with contralateral hypoplastic kidney. The patient was treated conservatively including temporary hemodialysis successfully. In the case of renal trauma, the status and function of the contralateral kidney must be evaluated first, and in the case of a solitary kidney, treatment should be centered on the preservation of its function.

[Percutaneous nephrolithotomy by electrohydraulic shock wave].

Twelve patients underwent percutaneous nephrolithotomy in our hospital. Six of these patients had stone disintegration by electrohydraulic shock wave. The procedure was safe and effective for achieving rapid stone disintegration. Translocation of the stone fragments and central metal core of the probe left in the ureter were clinical problems. However, they passed spontaneously. Usefulness and problems of electrohydraulic lithotripsy were discussed.
