[A necrobiotic nodule indistinguishable from lung adenocarcinoma].

A 43-year-old woman was referred for examination because of an abnormal shadow on a chest X-ray film. She had a 12-year history of seropositive rheumatoid arthritis. Chest X-ray films and CT scans showed a pleurabased solitary nodule without a cavity. Cytological examination of a transbronchial biopsy specimen did not lead to a diagnosis, so thoracoscopic enucleation was performed. Histologically, the nodule consisted of lymphocytes and fibroblasts surrounding a central necrotic area, which indicated that it was a rheumatoid nodule. This solitary necrobiotic nodule was radiographically indistinguishable from lung adenocarcinoma, so histologic confirmation was necessary.

Acute, subacute and chronic oral toxicity studies of the new serotonin (5-HT)3-receptor antagonist ramosetron in beagle dogs.

The oral toxicity of ramosetron ((R)-5-[(1-methyl-3-indolyl) carbonyl]-4,5,6,7-tetrahydro-1H-benzimidazole hydrochloride, CAS 132907-72-3, YM060), a new compound having serotonin (5-HT)3 receptor antagonist activity was investigated in beagle dogs. To evaluate the acute toxicity, two groups of beagle dogs, each comprised of one male and one female, were given YM060 bulk powder in gelatin capsules at dose of 0, 3 mg/kg or 0, 30 and 60 mg/kg in ascending order in at least 7-day intervals. After the final dose, animals were observed for 2 weeks. No deaths were observed at any dose. At 60 mg/kg, the male exhibited frequent vomiting, salivation and prone position 1-3 h after administration, when the plasma concentration of the unchanged drug reached Cmax or was close to Cmax. The female exhibited no changes except vomiting. No effects on either the male or the female were detected in body weight, food consumption, electrocardiography, hematology, plasma biochemistry or urinalysis. To evaluate the subacute toxicity of YM060, three male and 3 female beagle dogs per group received doses of 0, 1, 3, 10 and 20 mg/kg/d for 13 weeks. YM060 was triturated 10-fold using lactose and filled in gelatin capsules before use. The plasma concentration of unchanged drug increased almost dose-dependently, peaked about 2 h post-dosing and subsequently decreased with time. The plasma concentration-time profile after the final dose at week 13 was not different from that after the initial dose. No treatment-related changes were observed up to 3 mg/kg/d.(ABSTRACT TRUNCATED AT 250 WORDS)

A large contribution of a cyclic AMP-independent pathway to turtle olfactory transduction.

Although multiple pathways are involved in the olfactory transduction mechanism, cAMP-dependent pathway has been considered to contribute mainly to the transduction. We examined the degree of contribution of cAMP-independent pathway to the turtle olfactory response by recording inward currents from isolated cells, nerve impulses from cilia and olfactory bulbar responses. The results obtained by the three recordings were essentially consistent with each other, but detail studies were carried out by recording the bulbar response to obtain quantitative data. Application of an odorant cocktail to the isolated olfactory neuron after injection of 1 mM cAMP from the patch pipette elicited a large inward current. Mean amplitude of inward currents evoked by the cocktail with 1 mM cAMP in the patch pipette was similar to that without cAMP in the pipette. Application of the cocktail after the response to 50 microM forskolin was adapted also induced a large inward current. Application of the odorant cocktail to the olfactory epithelium, after the response to 50 microM forskolin was adapted, brought about an appreciable increase in the impulse frequency. The bulbar response to forskolin alone reached a saturation level around 10 microM. After the response to 50 microM forskolin was adapted, 11 species of odorants were applied to the olfactory epithelium. The magnitudes of responses to the odorants after forskolin were 45-80% of those of the control responses. There was no essential difference in the degree of the suppression by forskolin between cAMP- and IP3-producing odorants classified in the rat, suggesting that certain part of the forskolin-suppressive component was brought about by nonspecific action of forskolin. Application of a membrane permeant cAMP analogue, cpt-cAMP elicited a large response, and 0.1 mM citralva after 3 mM cpt-cAMP elicited 51% of the control response which was close to the response to citralva after 50 microM forskolin. A membrane permeant cGMP analogue, db-cGMP elicited a small response and the response to 0.1 mM citralva was unaffected by db-cGMP. It was concluded that cAMP-independent (probably IP3-independent) pathway greatly contributes to the turtle olfactory transduction.

Temperature increase abolishes ability of turtle olfactory receptors to discriminate similar odorant.

We examined the effects of temperature changes on odor-discriminating ability of turtle olfactory receptors in vivo by applying the cross-adaptation method to the olfactory bulbar responses. The olfactory system discriminated well all eight pairs of odorants examined at 5 and 18 degrees C. The ability of the olfactory receptors to discriminate pairs of odorants having similar structures (e.g., trans-3-hexenol and cis-3-hexenol; d-carvone and l-carvone) was reversibly abolished by increasing the temperature up to 40 degrees C, whereas discrimination of odorants having quite different structures was much less affected. The membrane fluidity of cells isolated from turtle olfactory epithelia and liposomes made of lipids extracted from the epithelia changed in a similar temperature range as for the decrease of the odor-discriminating ability, suggesting that an increase in membrane fluidity is correlated with the abolishment of the odor-discriminating ability. The present results also suggest that in vivo desensitization (adaptation) occurs not at the cellular level but at the receptor level. This mechanism was supported by the data recorded from a single olfactory cilium, showing that a single cell has both receptors for l-carvone and d-carvone and that the response to d-carvone appeared after the response to l-carvone was adapted.

Hemoglobin Bethesda causing polycythemia in a Japanese family.

A family with Hemoglobin Bethesda is reported. A 23-year-old man was hospitalized for the evaluation of polycythemia. Analysis of hemoglobin using high pressure liquid chromatography showed the presence of hemoglobinopathy. Separation of globin into alpha and beta chains revealed approximately 50% of the beta chain to be abnormal. Analysis of the DNA sequence of the beta chain gene identified Hb Bethesda. The family study disclosed that his father and sister also had the same hemoglobinopathy. This case is the first report of Hb Bethesda in Japan.

[Two cases of coronary artery fistula with coronary artery aneurysm].

Two cases of coronary artery fistula with coronary artery aneurysm were reported. Coronary angiography showed dilated right coronary artery with the formation of aneurysm drained to the right atrium and to the right ventricle respectively. The inflow ostium from the coronary artery to the aneurysm were closed from inside adding aneurysmorraphy. Postoperative clinical courses were uneventful and residual shunts were not detected by angiography. We reported these cases with literature.

[Right-sided infective endocarditis with ventricular septal defect].

Two patients underwent surgical treatment for right-sided infective endocarditis with ventricular septal defect. In both cases, blood cultures showed Peptostreptococcus, and the operation was performed at non-active phase after antibiotics therapy. The case 1 was a 7-year-old girl who was observed a vegetation on the chorda of the anterior paillary muscle by echocardiography. The defect was directly closed and the vegetation was excised. The case 2 was 22-year-old female who had been diagnosed of VSD in her infancy. A high fever continued and echocardiography revealed a vegetation attached to the septal tricuspid leaflet. Partial excision of the leaflet and autopericard patch plasty was performed, and the VSD was directly closed. Postoperatively intravenous antibiotic therapy was given for periods of 6 weeks, and clinical course were uneventful in both cases. Local excision of vegetation and leaflet repair by autopericard patch plasty should be performed in cases with localized vegetation and minor valvular regurgitation.

[Case report: suppurative arthritis due to group B Streptococcus in a patient with rheumatoid arthritis].

A 62-year-old woman, who had been diagnosed as having rheumatoid arthritis (RA) with systemic amyloidosis and diabetes mellitus, was admitted to our hospital because of polyarthralgia on October 1, 1987. She had some subcutaneous nodules and rheumatic pleural effusion. Therefore she was treated with 20 milligrams of prednisolone (PSL) daily. On the ninth day after the beginning of steroid therapy, she complained of severe pain and a new swelling in her right knee joint. The knee joint aspirate on arthrocentesis yielded a pure growth of group B Streptococcus (GBS). Blood culture was also positive for GBS. Her suppurative arthritis gradually improved by treatment with penicillin G. However, after the discontinuance of PSL, her pleural effusion deteriorated and she died on January 10, 1988. To our knowledge, there have been no prior reports of group B streptococcal suppurative arthritis complicating RA in Japan.

Effects of microbial products on glucose consumption and morphology of macrophages.

We studied the effects of microbial products on glucose consumption and morphology of macrophages which were elicited with thioglycollate medium. Macromolecules such as lipopolysaccharide (LPS), tumor promoters, and respiratory inhibitors increased macrophage glucose consumption without inducing evident morphological changes. The assay system was used to screen for active substances in culture broth extracts from actinomycetes. Among them, aureothin increased glucose consumption of macrophages and inhibited respiration of a rat mitochondrial fraction. Concanamycin A induced morphological changes of macrophages into needle-like shapes but not of cloned cells including the macrophage-like cells J774.1. This compound changed fibrosarcoma L929 cells into round shapes without affecting the shape of a nontransformed fibroblast, BALB/3T3 cells. Antimycin and concanamycin A increased tumor-killing activity of macrophages when added during the effector phase. These results suggest that this assay system is simple and sufficiently reproducible and thus usable for screening for modulators of macrophage function among natural products.

Melastin, a novel product of Streptomyces that selectively inhibits leukemia cell growth.

In the course of our screening for new immunomodulators, a novel compound, melastin, was purified from the culture broth of Streptomyces. Melastin was purified through absorption to Diaion HP-20, ethanol precipitation, and anion exchange column as a brown powder. The molecular weight was estimated as 5000 +/- 3000 by gel filtration HPLC. Melastin suppressed lipopolysaccharide (LPS)-induced blastogenesis of B cells more profoundly than concanavalin A (con A)- or phytohemagglutinin (PHA)-induced blastogenesis of T cells. Moreover, it selectively inhibited the growth of several leukemia cells as compared with interleukin-dependent nontransformed leukocytes. No selectivity was observed between nontransformed fibroblasts and their oncogene-transformed variants. Melastin did not selectively inhibit macromolecule synthesis of leukemia cells.

Direct identification of lung cancer cells producing granulocyte colony stimulating factor with monoclonal anti-G-CSF antibodies.

This is a case report of an 82-year-old man with squamous cell lung cancer which produced G-CSF. The leukocyte cell count was elevated to 94,400/mm3. The serum G-CSF concentration was also elevated to 351 pg/ml. This is the first report to demonstrate, by application of immunohistochemical staining techniques directly to tumor tissue obtained at autopsy, squamous carcinoma cells which produce G-CSF. The tumor cells were stained with monoclonal anti-G-CSF antibodies.

Diagnostic and therapeutic dilemmas in rapidly progressive sensorineural hearing loss and sudden deafness. A reappraisal of immune reactivity in inner ear disorders.

Sera from 76 patients with a clinical diagnosis of idiopathic rapidly progressive sensorineural hearing loss (SNHL) (n = 15), sudden deafness (n = 31) and with other etiologies of their hearing loss (n = 30) were analysed by western blot assay. Seventy-three percent of the cases with rapidly progressive SNHL had cross-reacting antibodies (27, 45, 50, 68 kD). The overall response to immunoprogressive therapy was effective in only 50% of cases. Sixty-five percent of the patients with sudden deafness also had cross-reacting antibodies (27, 45, 50, 80 kD). In these cases steroid therapy was more effective in re-establishing the hearing than no treatment, regardless of the western blot outcome. Spontaneous recovery occurred in approx. 50% of cases, but only in those with a positive assay. The antigenic epitopes detected with immunoblotting were not cochlea specific; they were also found in protein extracts of other organs (cranial nerves, kidney, brain).

Successful treatment of primary gastric non-Hodgkin's lymphoma with chemotherapy and radiotherapy.

A case of primary gastric lymphoma is reported. The lymphoma was of the diffusely infiltrating type extending from the supraangular region to the prepyloric region of the stomach. The patient refused gastrectomy, therefore combined treatment with BACOP chemotherapy and radiotherapy was carried out. Complete remission was achieved without any major complications. The patient has remained free from relapse for over 20 months.

The association of serum hepatitis C virus RNA with serum aminotransferase activities.

Serum samples from 71 patients with hepatitis C virus infection at various stages were studied to determine the clinical significance of the detection of hepatitis C virus RNA by the polymerase chain reaction. There was a significant correlation of serum HCV RNA with elevation of the serum aminotransferase levels. These data suggest that the detection of HCV RNA in the serum might be useful for evaluation of the extent of ongoing liver damage.
