[An evaluation of the plasma dynamics of the 16 alpha-hydroxy steroids in fetuses and neonates (author's transl)].

This study was done to evaluate the plasma dynamics of 16 alpha-hydroxy dehydroepiandrosterone (16 alpha OH-DHA),-pregnenolone (16 alpha OH-Preg) and -progesterone (16 alpha OH-Prog) in both the unconjugated and conjugated forms in fetuses and neonates. A comparison between second and third trimester umbilical plasma levels revealed that conjugated 16 alpha OH-DHA was already elevated in the second trimester, and both 16 alpha OH-Preg and 16 alpha OH-Prog were increased. At normal deliveries, all of the steroid levels in umbilical plasma, especially conjugates, were consistently higher than in maternal plasma. There were definite correlation between umbilical arterial and venous unconjugated and conjugated 16 alpha OH-Prog and conjugated 16 alpha OH-DHA, while no correlation of 16 alpha OH-Preg. Further, umbilical arterial 16 alpha OH-Preg showed a distinct correlation when compared to umbilical venous 16 alpha OH-Prog, suggesting that 16 alpha OH-DHA and 16 alpha OH-Preg are fetal in origin, while 16 alpha OH-Prog is synthesized in the placenta from 16 alpha OH-Preg. All of the steroid levels in the neonatal peripheral plasma up to and including the fifth post partum day were consistently higher than umbilical plasma levels. In low birth weight neonates, the umbilical plasma and peripheral neonatal plasma 16 alpha OH-Preg were consistently higher than levels found in normal birth weight neonates, while 16 alpha OH-Prog and 16 alpha OH-DHA did not show a statistical difference between low and normal birth weight neonates. The significance of these steroid dynamics and their physiological role are discussed.

Management of severe hypertension with nifedipine in combination with clonidine or propranolol.

The hypotensive effect of nifedipine was studied in 9 patients with severe hypertension. After oral administration of 20 mg nifedipine mean arterial pressure (MAP) fell, reaching lowest values about 1 h p.a. The antihypertensive effect lasted 7 h. Treatment with nifedipine alone (80 mg/day) lowered MAP significantly on the first day, but it tended to rise again therealter. Addition of clonidine (450 micrograms/day) or propranolol (120 mg/day) enhanced the hypotensive effect. Furosemide (120 mg/day) was ineffective when added to the above described regimen. Without nifedipine the combine drugs failed to keep MAP down. The decrease of MAP observed when giving this combination of drugs was associated with a significant fall in total peripheral resistance and plasma renin activity, but did not alter cardiac output and blood volume. We concluded that nifedipine together with clonidine or propranolol is very effective in the management of severe hypertension.

Estimation of urinary kininogenase activity using bovine serum low molecular weight kininogen.

Estimation of urinary kininogenase activity by radioimmunoassay of generated kinin was studied. Bovine serum low molecular weight kininogen was proved not to cross-react with kallidin antibody and also bradykinin antibody. This kininogen was used as substrate measuring urinary kininogenase activity. Separation of released kinin from the kininogen was not required in the present method. Urinary kallikrein activity was found to be significantly decreased in essential hypertension, in chronic glomerulonephritis and in patients who had received renal transplantation. On the contrary, an increase in urinary kallikrein was found in primary aldosteronism and in Bartter's syndrome. The present method was very useful for measuring kininogenase activity.

Plasma levels of 16 alpha-hydroxypregnenolone, 16 alpha-hydroxyprogesterone and 16 alpha-hydroxydehydroepiandrosterone in the fetus and neonates.

Simultaneous determination of unconjugated 16 alpha-hydroxypregnenolone (16 alphaOH-Preg), 16 alpha-hydroxyprogesterone (16 alphaOH-Prog) and 16 alpha-hydroxydehydroepiandrosterone (16 alphaOH-DHEA) in fetal and neonatal plasma was performed utilizing a newly developed radioimmunoassay. In all neonates, the three 16 alpha-hydroxysteroid levels were consistently higher in umbilical cord plasma than in the maternal peripheral circulation. 16 alpha-OH-Preg in the umbilical arterial plasma increased from 11.2 +/- 3.1 at 24 weeks to 29.7 +/- 12.0 ng/ml at term, 16 alphaOH-Prog from 15.5 +/- 3.2 to 34.3 +/- 11.0 ng/ml and 16 alphaOH-DHEA from 5.1 +/- 1.2 to 5.9 +/- 1.0 ng/ml. In the anencephalic neonates, only 16 alphaOH-Preg showed an increase pattern under ACTH priming. 16 alpha-OH-Preg levels for normal full term neonates remain relatively constant at the first 24 hr and show a slight decrease at 3 days post partum. In small full term neonates, 16 alphaOH-Preg levels in umbilical arterial plasma are considerably higher than in normal neonates and remain at roughly equivalent levels for the first 5 days post partum. 16 alphaOH-Prog and 16 alphaOH-DHEA levels in umbilical arterial plasma in normal and small full term neonates are almost equal and both groups show a rapid decrease during the first 24 hr. Comparison with findings of the three 16 alpha-hydroxysteroids in fetal and neonatal plasma is discussed.

Change in plasma renin activity by cold storage of plasma in normal subjects and patients with essential hypertension and primary aldosteronism.

The time courses of change in renin activity after cold storage of human plasma at -5 degrees C and pH 7.4 were examined in 5 normal subjects, 6 patients with essential hypertension and one female patient with primary aldosteronism before and after extirpation of the adrenal tumor. In the 5 normal subjects and 6 essential hypertensives, the gradual increase in plasma renin activity was observed until 10 days of cold storage. The same result was obtained in the case of primary aldosteronism. However, there was no increase in renin activity despite of cold storage for 10 days in plasma which was sampled from this patient 45 days after operation. These data indicate that a period of 4 days for cryoactivation of human plasma renin as has been reported by Sealey et al. is not sufficient to accomplish activation of renin by cold storage.

Factors influencing the release of renin in patients under chronic dialysis treatment.

Plasma renin activities (resting PRA, post-dialysis delta PRA) were studied in 61 patients under chronic dialysis treatment. Removed sodium and removed water were estimated at each dialysis. Delta PRA/removed-sodium and delta PRA/removed-water were calculated as indices in response to the removal of sodium and water during the dialysis. 1)Resting PRA (pre-dialysis PRA) was positively correlated to delta PRA/removed-sodium, delta PRA/removed-water, serum osmolality, and diastolic blood pressure, but negatively to serum sodium concentration, age, and pulse pressure/diastolic blood pressure. Statistically significant factors controlling the resting PRA were delta PRA/removed-sodium, delta PRA//removed-water, and serum sodium concentration. Resting PRA was slightly correlated to diastolic blood pressure and age. 2)Post-dialysis PRA was significantly correlated to the resting PRA, delta PRA/removed-sodium, delta PRA/removed-water, serum sodium concentration, and age, but not to the blood pressure indices.
