Persisting hallucinations following childhood sexual abuse.

Hallucinations can persist for many years after childhood sexual abuse. If we recognise this, we will not mis-diagnose psychosis and we may treat with psychotherapy (talk). The hallucinations are distinct from hallucinations in schizophrenia though patients have frequently been given that diagnosis. They would generally be classified as pseudo-hallucinations. They are generally self-referential. They can involve all sensory modalities. Three case reports illustrate this link. Methods for interviewing and providing ongoing help are discussed. Issues in phenomenology and diagnosis are considered. Post-traumatic stress disorder is the best diagnostic fit, though psychotic depression may explain some cases. Freud's case of Frau P (1896) was an early report of this link.

Evaluating summer camps 1979 - problems of measuring behaviour change in children.

To develop experience in assessing behaviour change in children, sixteen children attending week-long summer camps run by the Department of Psychiatry, Adelaide Children's Hospital in January 1979, were assessed every two months for six months. Target complaints, symptom checklists and measures of social behaviour were chosen as being most likely to be sensitive to behaviour change. Hypotheses about the summer camp experience having an influence on severity of target behaviour problems, symptoms indicating child psychiatric disorder, peer group participation, mastery of aggression and development of conscience and social values were not convincingly supported. Agreement between camp staff and parents rating the children's social behaviour using the same instrument a week apart was poor. Simple ranked adjective scales have advantages over goal attainment scaling for rating target complaints, Implications for treatment evaluation in child psychiatry are discussed.

Tardive dyskinesia and antihistamines.

A case of tardive dyskinesia due to the prolonged administration of antihistamines is reported. Clinical features are discussed, as well as the alleviation of these by haloperidol.

Marital interaction in depression.

The great majority of depressed patients are currently married or formerly married and depression is commonest at ages when marital satisfaction is least. Study of patterns of marital interaction in depression may provide useful leads for understanding the aetiology and therapy of depressive syndromes. Theoretical views of depression as loss, as coerced behaviour and as coercive behaviour suggest useful hypotheses. Both marital interaction and depression are complex variables to define and measure. A literature review suggests reduced general affective involvement, reduced expressions of affection, increased criticism, increased husband domination and a marked struggle for interpersonal control in the marriages of depressed persons. Marital therapy may be the most useful psychotherapeutic approach for depression.

Posted summaries of marital therapy.

Summaries of marital therapy sessions, similar to those used in group psychotherapy by Yalom, were posted to reach couples three or four days after each session. The summaries of 600 to 4,000 words included a narrative account of the session and some editorial comment by the therapist. Couples completed a questionnaire assessing the use of the summaries. Replies to questionnaires and clinical impressions indicate the summaries had good acceptability. Couples felt the summaries increased their focus on marital problems between the sessions and provided helpful clarification.
