High-resolution fluorescence in situ hybridization of RBM- and TSPY-related cosmids on released Y chromatin in humans and pygmy chimpanzees.

Applying two-colour fluorescence in situ hybridization (FISH) we simultaneously hybridized RBM- and TSPY-related cosmids to Y chromosomes in prophase and to released Y chromatin in interphase nuclei of man and pygmy chimpanzee. Whereas, even on prophasic Y chromosomes, no resolution of the overlapping RBM and TSPY signal clusters could be achieved, the RBM and TSPY signals are completely separated from each other in our maximum released Y chromatin stretches in interphase nuclei. These results unequivocally lend support to the view that the RBM and TSPY families have an interspersed organization on the Y chromosomes of man and higher apes. Thus, the distribution of RBM and TSPY signals might well go back to a common organization of these genes next to each other on an ancient Y chromosome.

[Comparison of miniplate and lag screw osteosynthesis in treatment of collum fractures].

The question whether- and if by which means-operative reconstruction of condylar process fractures is appropriate is still unresolved. This study was initiated to compare osteosynthesis of low dislocated and/or luxated condylar fractures by means of miniplates or lag screws according to Eckelt. 22 patients participated, 16 being treated with miniplate osteosynthesis, 6 with lag screws. Investigation concentrated on the parameters TMJ-function (side shift in dynamic and static occlusion, maximum interincisal distance, impairment of pro- and laterotrusion), radiologic post-operative fragment position, intra- and postoperative complications, and evaluation by the patients. Miniplate osteosynthesis showed better results leading in 80% to an unimpaired TMJ function compared to 50% of the lag screw group. Miniplate osteosynthesis was performed faster. No intra- or postoperative problems occurred in contrary to the lag screw, where one fracture of the screw intra- and one post-operatively were noted. Even if the numbers are too small for statistical calculations, trends are shown favouring miniplate osteosynthesis for operative reconstruction of condylar fractures.

[Severity of obstructive sleep apnea syndrome--correlation with cephalometric parameters].

In 132 patients with polysomnographic records suffering from OSAS a cephalometric analysis according to Hasund supplemented by special OSAS-parameters was performed. The statistical analysis showed a correlation between severity of OSAS and width of posterior airway space, length of the soft palate, hyoid position and posterior growth development of the mandible. A principal component analysis showed four groups: 1) prognathic patients, 2) retrognathic patients with narrow PAS, 3) patients with long soft palate and low standing hyoid, and 4) orthognatic obese subjects. Lateral cephalometry is an important contribution to OSAS-diagnostics and should be performed on all OSAS patients in the course of interdisciplinary cooperation.

Conscious midazolam sedation in third molar surgery--aspects of post-operative patient evaluation.

This study was conducted on 426 patients undergoing third molar surgery to evaluate their opinion on surgery and the follow-up period concerning postoperative behaviour, pain, and complaints. Two groups were formed as patients had to choose between local anaesthesia only or additional conscious sedation by means of intravenous midazolam (0.1 mg/kg). Women and younger patients preferred conscious sedation. Surgery was described as significantly less distressing by the sedated group. No difference in the evaluation of the follow-up period between both groups existed. Patients of the midazolam group took more analgesics, tended to stay longer in bed and reported on protracted cooling. Non-sedated persons older than 30 years complained about a slower decrease in postoperative pain. According to these findings, sensitive, cautious patients tend to prefer conscious sedation which is reflected in their behaviour. No relationship between the evaluation of surgery itself and the follow-up period could be found.
