[Axillary accessory breast tissue].

A 34-year-old women had a hen's egg-sized supernumerary mammary gland with no nipple or areola in the left anterior axillary line for the last 8 years. The main complaint about the axillary "lump" was that it was aesthetically displeasing, and it was excised. The importance of an axillary breast lies in the possibility that pathologic changes could develop in its substance and that clinically it could be confused with a lipoma. Early diagnosis and correct excision are usually recommended.

[Facial granuloma].

INTRODUCTION: The aim of this case report is to show the results of therapeutical procedures in the facial granuloma. CASE: A 48 year old male patient had multiple facial granulomas persisting for at least 8 years. They were unresponsive to dapsone. The surgical removal of two lesions and the liquid nitrogen cryotherapy of the remaining lesions were efficient without recurrence in a follow-up time of 6 months. COMMENTS: Lesions of facial granuloma do not spontaneously regress; they are easily recognized and differentiated from other conditions occurring on the face. New therapeutical approaches are welcome, even if they are empirical, in so far as they are efficient and reproducible.

[Basal cell carcinoma of the leg with metastases to the surrounding skin and inguinal lymph nodes].

A case of metastasizing basal cell carcinoma in a 57-year-old male is described. The primary tumor localised on the left lower leg metastasized to near skin and inguinal lymph nodes. The primary and metastatic skin tumors were excised and the defects covered by free skin grafts. Besides, the inguinal lymph nodes were block dissected and a part 10 cm long of the left calf bone was resected.

[Acne fulminans].

The authors describe the case of a 12 year-old boy suffering from acne fulminans. Typical clinical findings were observed. The disease responded to treatment with prednisolon and antibiotics.

Surgery of venous leg ulcers.

Numerical data are presented to prove the endemic nature and importance of the varicose and postthrombotic syndrome. The fact that even in the most fortunate cases conservative therapy can bring about only temporary improvement, has directed attention to surgical methods. The surgical methods of treatment of varicos (VS) and postthrombotic (PS) syndrome are reviewed. The surgical procedure considered adequate is described. It consists of the following steps. After phlebography, in the case of satisfactorily functioning deep veins, the superficial venous system is extirpated according to Trendelenburg, Madelung and Babcock, the insufficient perforants are exposed and ligated, the ulcer is excised deeply and broadly into the intact tissue. The effect is covered at a later date with a semi-thick skin graft. The importance of supplemental conservative methods (preparation and postoperative treatment) is stressed. This complex therapy was applied in 112 severe cases of venous failure; in 96% the result was excellent, in 3% adequate and in 1% poor. The guiding principles for the successful solution of chronic venous failure are a correct diagnosis and a correctly performed operation. Only proper care can guarantee good results.

Circular crural ulcer; its surgical therapy.

Experience with the surgical therapy of two serious cases of crural ulcer is described. The ulcers were widely and deeply excised together with the fascia and covered three weeks later by a split free skin graft. As a technical innovation, a mesh graft was applied in one case and in the other the ulcer was removed in two steps to avoid secondary shrinking. The functional result was in both cases unobjectionable even two years after the operation. In similar, apparently hopeless, cases the method will help to avoid amputation and to achieve complete freedom from symptoms for years.
