[Lipid blood spectrum in patients with amiodarone associated hypothyroidism. Effect of replacement therapy with L-thyroxine].

Blood lipid levels were measured in 23 patients with amiodarone associated hypothyroidism (most of them had ischemic heart disease). Abnormalities of lipid spectrum were found in 12 of these patients. All 12 patients were subjected to replacement therapy with l-thyroxine. Compensation of thyroid status was associated with average 12.6 and 12.3% lowering of total and low density lipoprotein cholesterol, respectively. However target levels of low density lipoprotein cholesterol were achieved only in 1 patient. There were no significant changes of high density lipoprotein cholesterol and triglycerides.

[Co-renitek treatment of patients with moderate and severe forms of hypertensive disease].

AIM: To evaluate efficacy and tolerance of a compound drug co-renitec combining an ACE inhibitor enalapril maleate and diuretic hydrochlorothiazide co-renitec taken for 16 weeks in essential hypertension (EH). MATERIAL AND METHODS: 28 patients with EH (16 males and 12 females aged 47-74 years) of mean duration 13.1 +/- 1.6 years. Blood pressure (BP) was monitored for 24 hours with the device SL 90207 (SpaceLabs Medical, USA). Microalbuminuria (MAU) was estimated with the use of immunoturbodimetric test. RESULTS: By 24-hour monitoring, co-renitec reduced day BP by 14.9/8.9 +/- 3/2 mm Hg, nocturnal BP lowered by 18.6/11.4 +/- 3/2 mmHg, pulse pressure also fell. Coefficient T/P was 53.5% for systolic BP (SBP) and 59.6% for diastolic BP (DBP). The target SBP was reached in 77% patients, target DBP--in 69%. Co-renitec significantly decreased MAU, albumines excretion normalized in 46% patients. CONCLUSION: Co-renitec lowers both day and nocturnal blood pressure, improves 24-h rhythm of BP, has a positive effect on the kidneys. This allows its recommendation as a first-line drug in patients with moderate and severe EH.

Intensification of free radical oxidation of low-density lipoproteins in the plasma of patients with ischemic heart disease receiving beta-hydroxy-beta-methylglutaryl-coenzyme A reductase inhibitor cerivastatin and inhibition of low-density lipoprotein peroxidation with antioxidant probucol.

Inhibitors of the key enzyme of cholesterol biosynthesis beta-hydroxy-beta-methylglutaryl-coenzyme A reductase (statins) decrease cholesterol content in atherogenic low-density lipoproteins in patients with coronary heart disease and hypercholesterolemia, but inhibited biosynthesis of ubiquinone Q10 protecting low-density lipoproteins from free radical oxidation. Cerivastatin in a daily dose of 0.4 mg markedly increased the content of lipid peroxides in low-density lipoproteins. However, complex therapy with cerivastatin and antioxidant probucol (250 mg/day) was accompanied by a sharp decrease in the content of lipid peroxides in low-density lipoproteins in patients with coronary heart disease in vivo. These data indicate that antioxidant agents should be used in combination with inhibitors of beta-hydroxy-beta-methylglutaryl-coenzyme A reductase (hypolipidemic preparations) for the therapy of patients with coronary heart disease.

[Serum lipoprotein (A) in patients with myocardial infarction and aortoarteritis].

To elucidate the physiological role of lipoprotein (a), a new independent atherosclerosis risk factor, the levels of lipoprotein (a) and 5 proteins of an acute phase were measured in acute (myocardial infarction, n = 21) and chronic (non-specific aortoarteritis, n = 15) tissue lesions. Blood was taken for analysis from patients with myocardial infarction when they were admitted to hospital for anginal attacks and then 4 times during a month and from patients with aortoarteritis at the stage of exacerbation 1 month before and after therapy. The findings indicate that in acute and chronic tissue lesions, there is a change in blood lipoprotein (a) levels, which is largely similar to that in the levels of acute phase proteins. One cause of such changes may be an increase in the synthesis of apolipoprotein (a) that is identical to that of other glycoproteins--acute phase proteins--in the reaction of the acute phase and then in the regeneration of damaged tissues.

[A kinetic method of determining creatinine using the Jaffé reaction].

The authors suggest a kinetic method for measuring the blood serum and urine creatinine by Jaffe's test. The method is highly specific, requires little biologic material, and rules out the deproteinization procedure. The specificity of the measurement is effected by selecting the period of time when it is creatinine that reacts with alkaline picrate. A thorough selection of the reaction mixture components and of the order of their addition helps minimize the interference of the substances reacting with alkaline picrate during the same period. The method is adapted to biochemical analyzers.

[A comparison of methodologic approaches to the study of amylase in the blood].

The blood serum alpha-amylase activity has been measured in normal male subjects by three methods, i. e. amyloclastic (W. Caraway's), with the use of the chromogenic insoluble substrate, and saccharogenic enzymic technique. The enzymic method has proved to be the most accurate and reproducible, whereas the amyloclastic one the least so. The enzyme activity values determined by the three methods are in poorly manifest though reliable correlation. The findings suggest that the data of the methods based on measurements of various parameters of the enzyme are not fully compatible.

[Separation of alpha-amylase isoenzymes using cellulose acetate electrophoresis].

alpha-Amylase isozymes were separated by electrophoresis in cellulose acetate to detect the isoforms of the chromogenic substrate manufactured by Lachema, Czechoslovakia. In a group of 20 normal subjects aged 25 to 45, alpha-amylase pancreatic isozyme activity prevailed. Patients with acute myocardial infarction developed, during 36 hours after the onset of the anginal attack, a reduction in the ratio of pancreatic amylase/salivary amylase activities in both increased and normal total alpha-amylase activities. The suggested modified technique of electrophoretic separation of alpha-amylase isozymes may become an effective method for differential diagnosis. Use of this method will help locate the source of hyperamylasemia in various diseases and will thus specify the diagnosis, ruling out the useless therapy for pancreatitis.

[Effectiveness of isolation ultracentrifugation in the combined treatment of heart failure refractory to therapy developing in the presence of severe arterial hypertension with stable and malignant courses].

The feasibility and practicability of isolated ultrafiltration, or extracorporeal evacuation of excessive body fluid, was assessed in patients with severe stable and malignant arterial hypertension, complicated by refractory heart failure. This procedure, added to combined treatment, is a means to overcome resistance to medication, achieve a stable compensation of circulatory insufficiency symptoms, bring down arterial blood pressure and use smaller doses of hypotensive and diuretic drugs. Isolated ultrafiltration proves less effective in patients with cachectic circulatory insufficiency.

[Protective effect of extracellular K+ in the myocardium in disorders of energy metabolism].

In an experimental study on isolated isovolumetric guinea-pig hearts, a 2.2-fold reduction of the coronary duct combined with metabolic block caused by dinitrophenol (0.05 mM) resulted in an eleven-fold drop in the attained pressure, and a shorter electric systole, a smaller P wave, and an ST displacement on the ECG. Deep suppression of mechanical and electric activity was combined with reduced glycogen content, and greater K+ and Na+ withdrawal from the heart. Quantitatively, the drop in contractility correlated with K+ withdrawal and ST displacement. A sudden increase in coronary perfusion rate was accompanied by a rapid increase in the rate of K+ elimination from the heart which was proportionate to the rate of hyperfusion, with a simultaneous transitory rise in attained pressure, a great increment in diastolic pressure and the emergence of arrhythmia and fibrillation, the severity of the changes correlating with the degree of K+ loss. A moderate increase in K+ concentration of the perfusate prevented the development of fibrillation and drastically reduced the degree of contracture and glycogen drop during hyperfusion. The obtained results suggest that moderate accumulation of extracellular K+ during the early phase of energy generation disorder can be of a protective nature, as it contributes to a sharp reduction in contractility and energy spending to improve cell survivability in critical conditions.

[Inhibitors of trypsin-like proteolytic enzymes as means of preventing the development of secondary necrosis in burn wounds].

The effect of proteolytic inhibitor contrical on the experimental burn wound healing was studied in rats using biochemical, histological and histochemical methods. In control untreated animals with flame burn of 20% of body surface wound healing was associated with development of secondary necrosis, marked inflammatory reaction, augmented activity of proteases and peptidases. The use of contrical prevented secondary necrosis; this effect was apparently related to reduction of tissue proteolytic enzymes activity.

[Characteristics of the lactate dehydrogenase isoenzyme spectrum of the skin in the presence of thermal lesions].

Experiments were conducted on rats; a study was made of the activity of lactic dehydrogenase (LDH) and its isoenzymes in the zone of affection and in the adjacent areas of the skin at various periods after the burn infliction; on the 1st--8th day there occurred a reduction of the sum total LDH activity in the scab zone and the underlying tissue by 70--80%, and in the margin and the intact skin--by 50%. These changes were accompanied by shifts in the isoenzymatic LDH spectrum in the affected tissue; the activity of fraction 5 displayed a sharp rise on the 1st day and that of fractions 2 and 3--a reduction, on the contrary, by the 8th day there was seen some diminution of fraction 5 activity and an elevation of fractions 2 and 3 activity. The following picture is observed on the 14th--22nd day after the burn; the sum total LDH activity remains low, the isoenzymatic LDH spectrum in the margin and the scab is largely normalized, whereas in the underlying tissue there persist changes in the LDH enzyme ratio (a reduction of fractions 1--3 activity, and a rise of fraction 5 activity).
