[Morphology and various problems of the pathogenesis of alcoholic microangiopathy].

Alcoholic microangiopathy was studied on the biopsy material from kidneys, skin and salivary glands of alcoholic patients by means of histochemical, immunohistochemical and electron-microscopic methods. Its formation is mainly caused by the direct action of ethanol on microcirculatory vessels. Alcohol-induced microangiopathy is morphologically manifested by the accumulation of intermediate filaments in the cytoplasm of endotheliocytes and perivascular infiltrate macrophages. The expression of pathological changes in the cytoskeleton is reduced due to the abstinence from alcohol. Glomerulonephritis in alcoholic patients, that can be considered as a peculiar manifestation of alcoholic microangiopathy, is immunocomplex with predominance in these immune complexes of IgA in combination with IgG and C3 complement fraction. Morphologically it is characterized by the accumulation of intermediate filaments in the cytoplasm of podocytes, mesangiocytes, nephrocytes and some endothelial cells, as well as early and quick formation of fibroplastic transformation and cirrhosis of the kidney.

[Morphogenesis of nephrocyte dystrophy].

Morphogenesis of dystrophy of nephrocytes was studied by analysis of 106 biopsies of kidneys from patients with glomerulonephritis using light (semi-thin sections) and electron microscopy. The development of one or another kind of nephrocyte dystrophy was shown to be due to the deficiency of certain mechanisms of renal reabsorption and secretion associated with the corresponding membrane-enzyme systems of the cell. Therefore there is no ground to believe in transition of one kind of nephrocyte dystrophy into another as has been assumed until recently. The point is in the combination of various types of dystrophy (hyalino-drop, hydropic, fatty) within the object under study (biopsy), tubule, or even one nephrocyte. The analysis of the data obtained permits some general conclusions to be made with regard to the morphogenesis and the essence of dystrophy.

[Changes in the kidney tubules in different forms of glomerulonephritis].

Changes in the renal tubules were studied by light microscopy in 106 puncture biopsies of the kidneys of patients with glomerulonephritis (GN). A certain regularity in the rate of finding of dystrophic and atrophic changes of the tubular epithelium in different morphological types of GN and a correlation of these changes with the level of proteinuria and values of the tubular functions of the kidneys were established. The most severe and extensive changes predominate in the proximal parts of the nephron and are associated with the involvement of the stroma and blood vessels of the kidneys, allowing the differentiation of tubular, tubulo-interstitial, and tubulo-interstitial-vascular components of GN. Changes in the stroma and vessels correlate with the values of glomerular filtration and concentration capacity of the kidneys and aggravate the decreased tubular functions.
