Characteristics of two lines of guinea pigs (BHS and BHR) differing in bronchial sensitivity to acetylcholine and histamine exposure.

In a previous study, we reported that as model animals to be used in the study of bronchial asthma in humans, two lines of guinea pigs were developed by ourselves: bronchial hypersensitive line (BHS) and bronchial hyposensitive line (BHR) as a control. Studies on biological characteristics in guinea pigs of two lines were undertaken, and the following results were obtained. 1) Airway resistance of guinea pigs of the two lines to intravenously administered acetylcholine, histamine and leukotriene D4 was found to be different between BHS and BHR. Airway resistance of BHS to the chemicals was increased compared with those of BHR. 2) The number of muscarinic acetylcholine receptors in lung membrane preparation and its affinity increased significantly in BHS compared with those of BHR. In beta-adrenergic and histamine H1 receptors, there was observed no difference between BHS and BHR. 3) No difference in IgE antibody production to ovalbumin was observed between BHS and BHR. 4) When total leukocytes and differential leukocyte count (percentage, %) in peripheral blood of BHS and BHR were examined, relative percentage of lymphocytes and eosinophils was significantly higher in BHS than in BHR, while percentage of neutrophils was significantly lower in the former than in the latter.

[CABG in 2 patients with severe left ventricular dysfunction].

Coronary artery bypass grafting was performed in two patients with left ventricular dysfunction (EF 16%). In both cases they had chest pain at rest and ischemic myocardium were detected by the SPECT. We considered them candidate for the operation regardless of LV dysfunction. They were eventful in the postoperative course. One case was discharged and is free from chest pain now, but another unfortunately died for multiple organ failure.

[Spontaneous aortic cusp rupture].

A 54-year-old man with sudden onset of aortic regurgitation due to aortic cusp rupture is presented. Urgent operation was undertaken and mitral valve replacement was performed with good post-operative course. Intraoperative examination revealed the detachment of right coronary cusp, prolapsing into left ventricle. There was no findings suggestive of bacterial endocarditis. Histological examination of excised valve revealed non-specific chronic inflammatory changes with focal area of myxomatous changes.

Direct projections of non-pyramidal neurons of Ammon's horn to the amygdala and the entorhinal cortex.

When WGA-HRP (wheat germ agglutinin-horseradish peroxidase conjugate) was injected into the amygdala (lateral and basolateral amygdaloid nuclei) or entorhinal cortex of the cat, a number of nonpyramidal neurons in Ammon's horn were retrogradely labeled. The results indicate that some non-pyramidal neurons in Ammon's horn send projection fibers to the amygdala and entorhinal cortex.

Coronary artery calcification in Kawasaki disease.

To evaluate the angiographic features of coronary lesions in Kawasaki disease with coronary artery calcification, cinefluoroscopy and cineangiography were retrospectively reviewed in 116 patients who had undergone coronary angiography between 1982 and 1989. Angiographic abnormalities of coronary arteries were demonstrated in 55 of the 116 patients. In 5 (9.1%) of the 55 patients, 9 with calcification were identified by cinefluoroscopy and chest x-ray. Eight of the 9 calcified lesions showed a circular or ring-shape configuration. Coronary angiography revealed a total occlusion of the right coronary artery with collateral circulation from the distal left coronary artery in 2 patients and a severe stenosis of the right coronary artery in 2 patients, in whom anticoagulant therapy had not been continued during the follow-up periods. The remaining patient in whom anticoagulant therapy had been continued had bilateral aneurysms but no significant stenosis. These results indicate that a ring-shape calcification on chest x-ray in a patient with a history of Kawasaki disease may suggest an involvement by coronary artery stenosis even when anticoagulant drugs had been given. Therefore, coronary angiography should be performed to evaluate the stenotic lesions if this type of calcification is found by routine radiographic examination.

The role of leukocyte depletion by in vivo use of leukocyte filter in lung preservation after warm ischemia.

The oxygen metabolites of leukocytes have been implicated as playing the central role in reperfusion injury of preserved lung for transplantation. Furthermore, it is still unknown how leukotrienes take part in this process. Twelve mongrel dogs were used, divided into two groups. In group A, left thoracotomy and hilar stripping of the left lung was performed under anesthesia. During warm ischemia (two hours) and reperfusion (two hours), a filter was used three times between the femoral artery and vein, and quantitative in vivo blood filtration was performed. In group B, warm ischemia and reperfusion was done in the same fashion without filtration. As for tissue free radical assay, lipid peroxidation products were measured by means of thiobarbituric acid. Tissue leukotriene B4 and C4 levels were measured by radio immunoassay. In addition, peripheral blood count and tissue wet/dry ratio were analyzed. Significant leukocyte depletion of peripheral blood with leukocyte efflux from reperfused lung was observed in group A. Significant increase of tissue malondialdehyde (MDA) titers after reperfusion was observed in group B, whereas no significant fluctuation of tissue MDA titers was noted in group A. Concerning leukotriene B4 and C4, no significant increase was observed in either group. Tissue damage estimated by lung wet/dry ratio was considered to be controlled in group A compared with group B. Thus, the authors conclude that in vivo use of a leukocyte filter is efficient in removal of peripheral blood leukocytes and in preservation of reperfused lung after warm ischemia and that oxygen metabolites of leukocytes are considered to play a role in tissue damage in this process.

Strain differences in guinea pigs' bronchial sensitivity to acetylcholine.

The bronchial sensitivity to acetylcholine (ACh) of guinea pigs of various strains was investigated to clarify strain differences. Inbred Strain 2, Strain 13 and JY-1 and non-inbred Hartley strain (two colonies) were used in this experiment. (1) Guinea pigs were exposed to 0.08% ACh aerosol and the time needed to produce falling down (TNPFD) was determined. Mean +/- standard error of TNPFD (n = 14 per group) of animals was 182 +/- 28 sec, 148 +/- 22 sec, 210 +/- 30 sec, 342 +/- 24 sec and 406 +/- 36 sec in Strain 2, Strain 13, JY-1, Hartley (Japan SLC) and Hartley (Hitachi), respectively. There was a significant difference in TNPFD between inbred strains and non-inbred strains (P less than 0.05 or P less than 0.01), indicating that inbred strains had higher sensitivity. (2) Guinea pigs were exposed to 20-5000 micrograms/ml ACh for 2 min. The mean dose threshold as determined by transcutaneous oxygen pressure was 524 micrograms/ml, 424 micrograms/ml, 614 micrograms/ml, 1317 micrograms/ml and 1651 micrograms/ml (n = 14 per group) in Strain 2, Strain 13, JY-1, Hartley (Japan SLC) and Hartley (Hitachi), respectively. Inbred strains showed lower dose thresholds than non-inbred strains. (3) Isolated trachea-lungs of 5 guinea pigs were perfused with 10(-9)-10(-5) g/ml ACh to determine strain differences. Dose response curves of animals of inbred strains shifted to the left (lower concentrations), unlike those of non-inbred strains, suggesting that inbred strains had higher sensitivity to ACh than non-inbred strains.(ABSTRACT TRUNCATED AT 250 WORDS)

Genetic profile of alloxan-induced diabetes-susceptible mice (ALS) and-resistant mice (ALR).

Two inbred strains from a foundation stock derived from Crj: CD-1 (ICR) mice were established after more than 20 generations of full-sib mating, and by simultaneous selective breeding for developing and not developing diabetes after alloxan administration (45 mg/kg in males, 47 mg/kg in females). To elucidate the genetic background of the two inbred strains, i. e., alloxan-induced diabetes-susceptible (ALS) strain and alloxan-induced diabetes-resistant (ALR) strain, their biochemical genetic markers and immunogenetic markers were examined. 1) For both strains, investigation of biochemical genetic markers at 19 loci and immunogenetic markers at 11 loci revealed no variation in any gene within the same strain, showing to be homogeneous, thus indicating establishment of the inbred strains. 2) The two strains differed from each other at 2 loci of biochemical genetic markers and at 5 loci of immunogenetic markers. 3) The ALS and ALR strains can be regarded as new inbred strains derived from ICR mice. 4) The results show that the marker genes of the two strains are different at 7 loci, but it remains unclear whether or not these genes are involved in the difference between the two strains in susceptibility to alloxan.

[A case of CABG in a patient with chronic idiopathic thrombocytopenic purpura].

This is a report of the management of a patient with chronic idiopathic thrombocytopenic purpura (ITP) and ischemic heart disease. High-dose transvenous gamma-globulin therapy (400 mg/kg/day for five days) was started five days prior to operation, yielding an increase in platelet count from 5.8 X 10(4)/mm3 to 12.8 X 10(4)/mm3. Platelet transfusion was used at the end of cardiopulmonary bypass and on the first postoperative day. There were no problems with hemostasis during the operative procedure and the postoperative course was uneventful. We prefer high-dose transvenous gamma-globulin therapy for ITP prior to open heart surgery as a treatment of choice.

An autopsy case of cardiomyopathy with restrictive physiology in a child.

An autopsy case of cardiomyopathy with restrictive physiology associated with subendocardial myocardial necrosis and fibrosis in a seven-year-old child is described. Cardiac catheterization showed high left ventricular end-diastolic pressure with a dip-and-plateau pattern. Macroscopically, marked dilatation with fibroelastosis in both atria and mild dilatation in both ventricles were observed. Histologic examination revealed extensive subendocardial necrosis with marked disorganization of myocardial cells and moderate arteriolosclerosis. We speculate that these extensive myocardial lesions, which contributed to the restrictive hemodynamic changes, could have been induced by anoxia or a Ca2+ transport abnormality of unknown cause.

[Medical treatment of type III dissecting aortic aneurysms: influence of the shape of the false lumen assessed by X-ray CT as a factor in the long-term prognosis].

The future needs of surgical repair for type III dissecting aneurysms among medically-treated patients were estimated according to the influence of the shapes of false lumina assessed by contrast-enhanced CT scans and by the level of blood pressure control at the time of discharge on the long-term prognoses. Thirty-six patients, 30 males and six females, whose mean age was 61 years, underwent enhanced CT scans at their discharge. Events, including deaths and dissection-related surgery were analyzed by a nonparametric actuarial method of Kaplan and Meier. The changes in CT findings were also studied in 33 patients who underwent follow-up CT scans within a mean interval of 3.5 years. 1. Systolic blood pressure was maintained below 140 mmHg (good control group) in 15 patients, and above 140 mmHg (poor control group) in 18 controls. There were no significant differences between the two groups for event-free survival rates. 2. The shapes of false lumina assessed by enhanced CT scans at the time of discharge were categorized in three types; 21 patients (group A) without false lumina of the aorta, or with a small crescentic false lumen in the thoracic aorta (type a), six patients (group B) with intimal flaps and two contrast-material-filled lumina in the thoracic aorta (type b), and nine patients (group C) with expanded false lumina or a false lumen whose margin was convex towards a true lumen in the thoracic aorta (type c).(ABSTRACT TRUNCATED AT 250 WORDS)

[Difference in the path between the incisal point and the tracer of simple mandibular movement recording apparatuses during the mandibular movement in the sagittal plane].

In order to study the difference in the path between the incisal point and its tracers, which were LED or a magnet, for two types of mechanism used in ready-made simple mandibular movement recording apparatuses, we calculated their locations during the rotally opening or protrusion of the mandible in the sagittal plane. The movement of the LED tracer was larger than that of the incisal point during the rotally opening movement, and its magnification ratio increased with increased opening. During the protrusion, the difference in their movements was affected by the balance of the inclination of the condyle path and the incisal path, and the movement of the tracer expanded vertically but contracted horizontally as the inclination of the incisal path was increased. The movement of magnet tracer was larger than that of the incisal point during the opening movement, but its magnification ratio decreased as the degree of opening increased. During the protrusion, the horizontal movement of the tracer was smaller than that of the incisal point, and its magnification ratio decreased as the inclination of incisal path became smaller. Its vertical movement was similar to that of the incisal point.

[The effect of peripheral blood leukocyte depletion on reperfusion injury of warm ischemic lung].

UNLABELLED: To clarify the role of leukocytes in reperfusion injury of preserved lung for lung transplantation, leukocyte depletion was performed and lung tissue generation of oxygen metabolites and histological evaluation were investigated. METHOD: 12 mongrel dogs were utilized, divided into 2 groups. In group A, hilar-stripped left lung was submitted to 2 hour warm ischemia and 2 hour reperfusion. Blood filtration was performed three times using leukocyte filter. In group B, warm ischemia and reperfusion were performed without filtration. Lipid peroxidation products of lung tissue were measured by thiobarbituric acid assay. Tissue damage was estimated by wet/dry ratio and histological analysis. Results 1). Peripheral blood counts revealed that effective leukocyte depletion was obtained and leukocyte efflux from reperfused lung was observed in group A. 2) Significant increase of lung tissue oxygen metabolite generation after reperfusion was observed in group B, while no significant fluctuate of tissue MDA titers was noted in group A. Tissue damage estimated by histological analysis and wet/dry ratio were considered to be controlled in group A compared with group B. In conclusion, leukocyte depletion by in vivo use of leukocyte filter was effective for reperfusion injury of warm ischemic lung and oxygen metabolites may play a role in these process.

[A successful repair of post-infarction left ventricular free wall rupture].

Left ventricular free wall rupture secondary to acute myocardial infarction is almost invariably fatal. This report is the case presentation of a successful repair of left ventricular free wall rupture. A 55-year-old man, with a diagnosis of acute infero-lateral myocardial infarction, was transferred from another hospital to our CCU having recurrent chest pain on the fourth day after infarction. Shortly after admission, he lost his consciousness and fell into cardiogenic shock. Echocardiography showed a large pericardial fluid. He was immediately transferred to the operating room with the diagnosis of the heart rupture. After opening the pericardium containing 200 cc of blood, cardiac tamponade was relieved. The posterolateral portion of the left ventricle was found to be bluishly discolored, with a 8 mm-long tear of epicardium. Using cardiopulmonary bypass, the tear was closed with Teflon-reinforced sutures. The post-operative course was uneventful.

[Valve prostheses of mitral position in children].

Twenty children less than 15 years old of age who had undergone mitral (left-sided atrioventricular) valve replacement before August 1983 have been followed up until October 1988. Fourteen biological valves (B-group) and seventeen mechanical valves (M-group) were used in them. Preoperative conditions had been similar between B- and M-groups. For the B-group, there were one operative death and two late deaths. For the M-group there were no operative death and three late deaths. An actuarial survival rate at ten years was 53.5 +/- 23.3% for the B-group and 85.7 +/- 9.4% for the M-group (not significant). All biological valves were calcified and failed in 27 to 89 months. Three mechanical valves caused thrombosis. There was no other complication for both groups. An event free rate was 0% at eight years for the B-group, however it was 70.6 +/- 11.0% at ten years for the M-group (p less than 0.025). Our data indicates that the mechanical valve is more suitable than the biological valve for children.

[A case of bioprosthetic mitral valvular dysfunction due to pannus-formation].

A case of bioprosthetic mitral valvular dysfunction accompanied by mitral stenosis due to pannus is reported. A 69-year-old woman, whose mitral valve had been replaced with a Hancock bioprosthetic valve in June 1979, underwent the second mitral valve replacement in September 1988 because of valvular dysfunction. The ring and cusps of this Hancock valve was partially covered with hard and milky white pannus which resulted in mitral stenosis. The tear of cusp was observed at the point of contact with pannus, which resulted in mitral regurgitation. Histologically, pannus was consisted of two-layered structure, fibrin and collagen fiber, which demonstrated this over-growth was based on thrombus. Main causes of bioprosthetic valve dysfunction are calcification and tear of cusps. But, valvular dysfunction due to pannus-formation described in this case has been experienced very rarely.
