Clinical evaluation of serum amlodipine level in patients with angina pectoris.

Serum amlodipine levels were determined in 18 patients with vasospastic angina. Patients were divided into two groups: Group A (n = 9) received amlodipine 5 mg by single daily administration, and Group B (n = 9) received 10 mg given by single daily administration for the first 3 days, then 5 mg from the 4th day on. The serum amlodipine concentration in Group A took 7 days to reach a steady state of around 8 ng/ml. The level in Group B was 8.9 ng/ml at 3 days. From these results, the optimal dosage of amlodipine in the treatment of angina pectoris is 10 mg for the initial 3 days followed by 5 mg thereafter.

Analysis of heart-infiltrating T-cell clonotypes in experimental autoimmune myocarditis in rats.

Experimental autoimmune myocarditis (EAM) resembles the lethal giant cell myocarditis seen in humans, and the recurrent forms lead to dilated cardiomyopathy (DCM). EAM in rats induced by a subcutaneous injection of cardiac myosin has been shown to be a T cell-mediated autoimmune disease. Alpha beta T cells have proved to be important by the observation that antibodies to alpha beta T-cell receptor (TCR) prevent disease progression. Alpha beta cells recognize antigenic peptides bound to major histocompatibility (MHC) molecules by alpha beta TCR, and complementarity determining region 3 (CDR3) is considered the most important region for antigen recognition. To elucidate the nature of this T cell-mediated myocarditis, we analyzed TCR V beta chains of heart-infiltrating T cells. In the early state of EAM, none of 22 TCR V beta chain transcripts seemed to be dominant by reverse transcription-polymerase chain reaction analysis of total RNA and flow cytometric analysis. On the other hand, single-strand conformation polymorphism analysis of TCR V beta 8.2, V beta 8.5, V beta 10, and V beta 16 cDNA amplified by polymerase chain reaction encompassing the CDR3 revealed oligoclonal expansion in heart-infiltrating T cells isolated from animals at various disease stages. cDNA encoding V beta CDR3 from heart-infiltrating and pericardial effusion T cells in rats with EAM revealed more restricted sequences than did cells from rats with normal spleens. Clones from distinct lesions of the same animal were identical, and clones from heart-infiltrating and pericardial effusion T cells from the same animal showed overlap. Thus, CDR3 of the TCR beta chain may be important in rat EAM, and heart infiltrating T cells are considered to recognize the specific antigen.

Morphometric study on the fetal thyroid gland in the nude mouse (BALB/cAnNCrj-nu/nu).

Fetal thyroid glands of athymic nude mice (BALB/cAnNCrj-nu/nu) were examined morphometrically on day 18 of gestation. Compared to euthymic litter mate controls (BALB/ cAnNCrj-nu/+), both the cell height and diameter of thyroid follicles were significantly smaller; fewer well-developed follicles were found in the peripheral region of the thyroid; the body weight and total volume of the thyroid gland were also smaller in nude mice. These results suggest underdevelopment of the fetal thyroid gland of the athymic nude mouse.

Changes in significance of axillary lymph node dissection for patients with breast cancer.

Axillary lymph node dissection has been a routine part of breast cancer treatment for more than 100 years. As so few patients have been shown to have positive nodes, more consideration should be given to eliminating axillary node dissection for duct carcinoma in situ (DCIS) and T1a lesions. And for patients with T1/2N0M0 cancer of the breast, lumpectomy alone without axillary dissection followed by radiation therapy of the intact breast and regional lymph nodes should be a reasonable treatment without any arm morbidity. Between September 1989 and September 1994, we have treated 40 breast cancer patients with this method and no local recurrence nor distant metastasis has been encountered so far. Therefore, it is concluded that axillary dissection should be performed routinely only for N1b lesions and larger.

Immune reaction induced by X-rays and pions and its stimulation by schizophyllan (SPG).

Female C57BL/6 mice aged 6-8 weeks with transplanted Lewis lung cancer cells were used to investigate the anti-tumour effects and immune reactions in tumour tissue induced by X-ray and pion irradiation and their modification by schizophyllan (SPG). The effect of SPG on the rate of lung metastasis and the survival time of the mice was also studied using the same tumour system. These studies showed that in this tumour system the "practical' relative biological effectiveness (RBE) of pions was 1.33 in the dose ranges used (3 Gy x 4 = P3; 6 Gy x 4 = P6). SPG increased the suppression of tumour growth associated with moderate doses of radiation: X-rays (4 Gy x 4 = X4) or P3. SPG also decreased the number of lung metastases and prolonged the life span of the mice, these effects being independent of radiation. The addition of SPG to radiation increased both the macrophage infiltration and T-lymphocyte infiltration in the local tumour and the lung nodules. There did not appear to be any major differential effect of SPG on the pion-treated mice compared with those treated with X-rays.

[Evaluation of thin section CT scanning in the prone position of metastatic axillary lymph nodes for breast cancer].

A retrospective study was performed to determine whether thin section CT scanning in the prone position of the breast and the axilla yielded useful information regarding the status of axillary lymph nodes in patients with breast cancer. Thirty-six patients with breast carcinomas were scanned preoperatively from the supraclavicular regions to the breast in the prone position with 5 mm sections. Axillary lymph nodes measuring > or = 5 mm on the short axis were considered abnormal. Correlation with axillary dissection was obtained in all patients, giving a positive predictive value for axillary metastases of 83.3%, with 88.2% sensitivity, 84.2% specificity, and 88.8% negative predictive value. We concluded that thin section CT scan in the prone position was an accurate predictor of axillary lymph node involvement. We made a phantom with lymph node swelling to evaluate whether CT scanning with 5 mm sections was necessary for detecting 5 mm swollen lymph nodes. We scanned the phantom with 5 mm and 10 mm sections. Twelve radiologists counted the swollen lymph nodes on 5 mm section images and 10 mm section images of the phantom. The average number of miscounts was 1.1 (miscount rate 6.8%) on 5 mm section and 2.8 (15%) on 10 mm sections. We concluded that 5 mm section CT scanning is superior for detecting 5 mm lymph nodes.

A 10-year experience of intraoperative radiotherapy for gastric carcinoma and a new surgical method of creating a wider irradiation field for cases of total gastrectomy patients.

PURPOSE: To improve the prognosis of gastric cancer, radical surgical resection with extensive lymph node dissection plus intraoperative radiation therapy (IORT) was tried in our clinic. In addition, a new operative procedure was created to obtain wider irradiation field for total gastrectomy patients. METHODS AND MATERIALS: A total of 183 gastric cancer patients who underwent radical gastrectomy with or without IORT from August 1983 to July 1992 were retrospectively evaluated. The patients were divided into two groups: group 1 consisted of 58 patients who underwent radical operation plus IORT. A single dose of 28 to 30 Gy was delivered around the celiac axis with an electron beam of 12 MeV. Group 2, our historical control group, which showed no difference in age, sex and stage, consisted of 120 patients who underwent only radical surgery. In addition, a new method of total gastrectomy with IORT after mobilization of the tail and body of the pancreas was devised to get wider irradiation field for advanced gastric cancer. RESULTS: Of the Stage II gastric cancer patients, all the 11 patients of group 1 are alive, whereas in group 2, the 4-year and the 8-year survival rates were 60% and 48%, respectively. In Stage III patients, the 8-year survival rate of group 1 was 55% vs. 35% in group 2. As for Stage IV patients, the 5-year survival rate of group 1 was 12% and that of group 2 was 13%. CONCLUSION: Using this combined treatment modality of radical surgical operation+IORT, improved survival rates were obtained for Stage II and III gastric cancer patients. However, the method was ineffective for more advanced, Stage IV, patients. The wider irradiation field method used for total gastrectomy patients was safe and no complications were encountered.

Effects of 15-deoxyspergualin on experimental autoimmune giant cell myocarditis of the rat.

BACKGROUND: The benefits of immunosuppressive therapy for human myocarditis are controversial. The effects of a new immunosuppressant agent, 15-deoxyspergualin (DSG), on rats with experimental autoimmune myocarditis (EAM), an animal model of human giant cell myocarditis, were examined. METHODS AND RESULTS: Lewis rats were immunized with cardiac myosin in Freund's complete adjuvant on day 0. In the first experiment, the effective doses of DSG required to prevent EAM were investigated. Rats were placed into one of five groups: the control group (A) was administered saline from days 1 to 10; group B, 0.3 mg/kg per day of DSG; group C, 1.0 mg/kg per day of DSG; group D, 3.0 mg/kg per day of DSG, and group E, 10.0 mg/kg per day of DSG. Rats were killed on day 28. The heart weight/body weight ratios of the rats of groups D and E were significantly lower than that of the control group. Macroscopic and microscopic scores for myocarditis decreased in groups D and E. In the next experiment, the effects of delayed administration of DSG in preventing autoimmune myocarditis were studied. Two groups of rats received 3.0 and 10.0 mg/kg per day of DSG from days 6 to 15, respectively. Two other groups of rats received the same doses of DSG from days 11 to 20. No preventive effect of delayed DSG treatment was observed. The effects of long-term, delayed initiation therapy then were evaluated. Rats were administered 10.0 mg/kg per day of DSG from days 6 to 25. The heart weight/body weight ratio and macroscopic and microscopic scores of the rats so treated significantly decreased compared with the controls. CONCLUSIONS: It was demonstrated that DSG can prevent the development of cardiac myosin-induced autoimmune myocarditis.

An ultrastructural study on experimental autoimmune myocarditis with special reference to effector cells.

The previously described animal model of autoimmune myocarditis induced by immunization of Lewis rats with cardiac myosin was investigated pathologically, ultrastructurally and immunohistologically. Cardiac dendritic cells and infiltrating macrophages were dominantly encamped at the front of the myocarditis. The macrophages accompanied by neutrophils infested cardiocytes, forming rosette figures as a sign of active cardiocytolysis. The dendritic cells reached into intact muscle at a short distance from the inflammation. T cells, the most important candidate for effector cells in this animal model, were generally found at a distance from active muscle necrosis.

Intrahepatic portal-venous changes in dogs with dirofilariasis: scanning electron microscopy of resin casts of vasculature.

Intra hepatic portal-venous changes in dogs with dirofilariasis, in which characteristic cavernous transformation has been histologically observed, were studied by scanning electron microscopy (SEM) of resin casts of vasculature. Changes in the central and sublobular veins were the most significant. The venous channels were extremely dilated, thin-walled and showed cavernous structure. SEM observation showed multi-saccular formation with beads-like, helical and extremely dilated configuration. The conducting venules, i.e. terminal tributaries of the portal vein, showed a rapid taper just before the point of merging into sinusoids. Neither vascular proliferation nor collateral circulation around the portal channels could be detected in the present resin cast study.

Vascular pattern of a metastatic liver tumor in a dog:--a scanning electron microscopic study of resin casts.

Metastasis of a malignant mixed tumor occurring in the abdominal wall was found in the liver of a 15-year-old dog male. The vascular system of the metastatic lesion was examined by scanning electron microscopy using the resin cast technique. The existence of arterio-portal shunts was confirmed. The shunts arose from branches of the hepatic artery in retrograde or vertical direction and merged into the capillary plexus replacing the branches of the portal vein, forming the so-called thread and streaks vessel in the field of hepatic angiography. Vessels with the thread and streaks appearance histologically represented tumor emboli growing in the lumen of a large branch of the portal vein. There were capillaries running inside along the portal vein and forming a longitudinal dense network in the vein. Branches of the hepatic artery feeding the metastatic lesion were well developed and formed a coarse arterial network in their course. Tributaries of the hepatic vein from the lesion were also well developed.

Exercise-induced angina with intermittent ST-segment elevation.

A 59-year-old man was admitted for evaluation of mild exertional angina in the morning. During graded treadmill exercise stress testing, the patient had reproducibly intermittent chest pains associated with ST-segment elevations in leads II, III and aVF. A baseline coronary angiogram showed a coronary narrowing of 90% in the middle segment of the right coronary artery. The coronary narrowing was immediately resolved by an intracoronary injection of nitroglycerin. This was a very rare case of variant angina with intermittent ST-segment elevations and chest pains which were reproducibly demonstrated during exercise and the recovery phase of treadmill testing.

[Development of apoptosis induced by whole-body irradiation in murine liver].

Apoptosis is known to be induced by radiation. However, the correlation between radiation-induced apoptosis and radiation injury in tumors in vivo has been unclear. In this paper, we report the study of apoptosis induced by whole-body irradiation using an immunohistochemical technique to detect DNA fragmentation in murine liver. A dose of 7 Gy was employed as LD50/30. DNA fragmentation was observed 30 min after radiation, and it peaked between 1 and 6 hours. DNA fragmentation could be detected 48 hours after radiation in capillary endothelium. This study was able to reveal the development of radiation-induced apoptosis in the liver by detecting DNA fragmentation in situ.

Localization of porcine cardiac myosin epitopes that induce experimental autoimmune myocarditis.

Porcine cardiac myosin induced severe myocarditis in Lewis rats, similar to that seen after immunization with human or rat cardiac myosin. We investigated the localization of pathogenic epitopes by using porcine cardiac myosin. Subfragment-1 (S-1) and the rod were obtained by alpha-chymotryptic digestion. The rod was further fragmented by using cyanogen bromide cleavage. Three subfragments of S-1 were prepared by tryptic digestion. All Lewis rats immunized with the rod exhibited severe myocarditis, and the immunization of the cyanogen bromide-cleaved peptide equivalent to human beta-cardiac myosin heavy chain RDCB9 (residues 1070 to 1165) induced moderate myocarditis. Although none of the rats immunized with the whole S-1 exhibited any myocardial lesions, moderate inflammatory cardiac lesions were detectable in rats immunized with the tryptic digests of S-1. Our results indicate that the myocardiogenic epitopes in Lewis rats are located in the RDCB9 (residues 1070 to 1165) of the cardiac myosin rod and that a cryptic minor epitope may reside in S-1.
