Electrophysiologic study and ergonovine provocation of coronary spasm in unexplained syncope.

A total of 38 patients with syncope in whom a cause was not assigned or suggested by the initial history, physical examination and electrocardiography (ECG) were studied. Twenty-four patients underwent cardiac examination with Holter ECG, electrophysiologic testing with programmed ventricular stimulation (EPS) and/or coronary arteriography including ergonovine provocation (ergonovine CAG). The study with Holter ECG, EPS and ergonovine CAG yielded a presumptive diagnosis in 36 patients (36/38 = 95%), 11 with vasopastic angina, 7 with ventricular tachycardia, 4 with ischemic heart disease, 9 with sick sinus syndrome (SSS), 1 with drug induced SSS, 3 with A-V block, 1 with supraventricular tachycardia, 1 with hypertrophic cardiomyopathy, 1 with aortic valve stenosis and 1 with carotid sinus syndrome (included are 1 patient with ventricular tachycardia+ischemic heart disease, 1 with SSS+vasopastic angina and 1 with ventricular tachycardia+vasospastic angina). Therapy based on these findings provided complete symptomatic relief in all patients during a mean follow up of 25 +/- 10 months (range 5-45 months). In conclusion, EPS and ergonovine CAG are useful in the diagnosis and therapy of unexplained syncope.

Vascular architecture of 3-methyl choranthrene-induced rat fibrosarcoma demonstrated by scanning electron microscopy of resin casts.

A detailed knowledge of the vascular system of tumors is requisite to a clear understanding of tumor physiology and pathology. Accordingly, in this study, examination was made of the vascular architecture of rat fibrosarcomas induced by 3-methyl choranthrene by scanning electron microscopy of resin casts to observe the three-dimensional vascular pattern of the sarcoma. The results were compared with those of corresponding serial sections of India ink-injected tissue. The major arteries in and around tumors appeared devoid of intra-arterial cushions which regulate blood flow. The arterial lining of resin casts was smooth just at the site where it entered the tumor. This may possibly be indication of degeneration and loss of the smooth muscle layer in the vascular wall. Casts of capillaries were very thin from 4 to 6 microns in diameter and very few sprouts could be seen. Capillaries, frequently present in superficial regions of the tumor, formed a rich irregular fine polygonal network. In fibrous tissue in deeper regions of the tumor, this network was coarse. The absence of intra-arterial cushions and degeneration of muscle cells may thus possibly be the reason for the characteristic haemodynamics of tumors.

[A case of infective endocarditis in which a new vegetation appeared on a different site during chemotherapy].

42-year-old man, who had been febrile for about a month, was admitted to our hospital. Laboratory testing showed leukocytosis and high titer of CRP. Streptococcus sanguis II was detected in his blood culture. According to the echocardiogram, he had a vegetation on the anterior mitral leaflet, so he was diagnosed as having infective endocarditis. Antibiotic susceptibility test using the disc method showed (3+) response to penicillin G. After intravenous administration of 20 million units of penicillin G for 3 weeks, a new vegetation appeared on the posterior mitral leaflet although the one on the anterior mitral leaflet had disappeared. Imipenem/cilastatin was administered until the acute phase reactants became negative. But the vegetation did not disappear, so he had vegetectomy. This is the first case report of infective endocarditis in which a new vegetation appeared on a different site despite the disappearance of the first lesion during chemotherapy.

Fibronectin expression in cancer tissues from patients undergoing radiation therapy.

Fibronectin expression and distribution were examined in cancer tissues from 19 patients with cancer of the head and neck regions. Samples taken before and after irradiation of approximately 10 Gy, 20 Gy or 30 Gy were analyzed by the avidin-biotin-horseradish peroxidase method using mouse monoclonal antibodies against human fibronectin. The results were correlated with the patient's prognosis after radiation therapy. No remarkable changes in the fibronectin expression or distribution were found between tissue specimens taken before and after each dose of irradiation. The prognosis, however, varied according to the degree of expression and the distribution pattern of fibronectin. Seven patients in which the cancer tissue was encircled by a thick fibronectin network are still alive without recurrence 4.5-6 years after treatment, whereas 6 patients in which fibronectin was only faintly expressed or focally distributed died or developed recurrence soon after treatment. The present findings demonstrate that fibronectin expression and distribution in cancer tissue are intimately related to the patient's prognosis, and that the analysis of these two parameters is applicable as a predictive assay in radiotherapy of cancer of the head and neck regions.

Changes in the Ki-67 labeling rates of head and neck squamous cell carcinomas during preoperative radiation therapy.

Immunostaining with Ki-67 monoclonal antibody was performed on frozen sections of biopsy specimens obtained before and during preoperative radiation therapy from 21 patients with head and neck squamous cell carcinoma. The Ki-67 labeling rates before radiation therapy and at radiation doses of 10 and 20 Gy ranged from 21 to 71% (mean: 35.0%), from 7 to 49% (mean: 25.8%) and from 1 to 44% (mean: 14.8%), respectively. One of the 2 patients whose tumors showing Ki-67 labeling rates of greater than 48% (mean +1 SD) before radiation therapy suffered local relapse shortly after the treatment. Moreover, tumors with rapidly decreased Ki-67 labeling rates (lower than 3%) at radiation doses of 20 Gy were related to poor clinical outcome: 4 out of 6 patients whose tumors showed Ki-67 labeling rates below 3% (mean -1 SD) at 20 Gy of irradiation had local relapses or showed distant metastases. These findings indicate that immunostaining with Ki-67 monoclonal antibody of biopsy specimens of head and neck squamous cell carcinoma, before and during radiation therapy, is very useful in assessing the clinical outcome of the patients.

[A case report of bone metastasis diagnosed by MRI and effectively treated with UFT after breast conserving therapy for breast cancer].

A 66-year-old woman with left breast cancer (medullary carcinoma; T1cN1M0; Stage II A) was treated with breast conserving therapy combined with lumpectomy, radiotherapy, chemotherapy and endocrine therapy beginning in March, 1990. She complained of back pain and was diagnosed as having bone metastasis to the lower thoracic spine by bone scintigram and MRI examination in September, 1990. Oral administration of UFT (300 mg/day) was started, and 5 months later, back pain disappeared; nineteen months later, no definite findings of bone metastasis were observed on bone scintigram and MRI. It was concluded that long-term oral administration of UFT is an effective remedy for bone metastasis after breast conserving therapy for breast cancer.

[Immunohistochemical study of fibronectin expression in cancer tissue before and after irradiation].

We examined the expression and distribution of fibronectin in cancer tissues from 19 patients with cancer of the head and neck regions. Samples taken before and after irradiation with approximately 10 Gy and 20 Gy or 30 Gy were immunohistochemically stained by the avidin-biotin-horseradish peroxidase method using a monoclonal antibody against human fibronectin. No remarkable changes in the expression or distribution pattern of fibronectin were found between tissue specimens taken before and after each dose of irradiation. The prognosis, however, differed according to the pattern of fibronectin distribution. Eight patients in whom the cancer tissue was thickly encircled by fibronectin are alive without local recurrence or distant metastasis. Meanwhile, six patients in whom fibronectin was expressed faintly or focally (or scattered) in the cancer tissue died or developed recurrence soon after treatment. These findings indicate that the immunohistochemical analysis of fibronectin in cancer tissue exposed to irradiation may be applicable as a predictive assay in radiotherapy of cancer.

Coping style of schizophrenic patients in the recovery from acute psychotic state: a preliminary study.

This study was undertaken to clarify a clinically effective coping style of 13 schizophrenic patients during recovery from an acute psychotic state. Sixteen recovered depressives served as the control group. A comparison of the coping behavior profile between the two groups revealed that changes in physical activity were significantly more frequent in the schizophrenics (62%) than in the depressives (25%).

Application of the immunohistochemical method as a predictive assay in radiotherapy of squamous cell carcinoma of oropharynx and hypopharynx.

We analyzed various subsets of lymphocytes infiltrated into cancer tissues from 15 patients (10 cases of oropharyngeal cancer and 5 of hypopharyngeal cancer) before treatment and after irradiation with approximately 10 Gy (1,000 rad) and 30 Gy by the method of biotin-avidin-horseradish peroxidase using mouse monoclonal antibodies. The infiltration was graded (marked) to - (none). In 3 cases, tumor-infiltrating lymphocytes were remarkably increased at the delivery of a small dose of irradiation (approx. 10 Gy): 2 of them showed remarkable radiosensitivity, and the lymphocyte subpopulation of the cancer tissue was mainly composed of Leu-3a + 3b-positive and Leu-8-negative lymphocytes (helper inducer T lymphocytes or delayed-type hypersensitivity cells). On the other hand, the third one proved to be rather radioresistant, and the lymphocyte subpopulation was mainly composed of Leu-14-positive lymphocytes (B lymphocytes). These findings indicate that the analysis of the lymphocyte subpopulation infiltrating into cancer tissues at the delivery of small doses of irradiation is applicable as a predictive assay in radiotherapy of squamous cell carcinoma of the oropharynx and hypopharynx.

[Improvement in the effects of radiation therapy with BRM].

The effects of radiation therapy and Sonifilan (SPG; a glucan produced by Schizophyllum commune) on the control of the primary lesion of human uterine cervix cancer and the experimental metastasis with mice were studied. SPG had excellent effects on the uterine cervix cancer compared with radiation therapy alone. Furthermore, immunohistochemical study showed that tumor infiltrated monocytes in good responders were mainly occupied with helper/induced T lymphocytes. SPG also suppressed the experimental lung metastasis especially with radiation therapy. Metastatic tumor infiltrated monocytes were many macrophages, which was the same as in inoculated local tumor.

[Analysis of brain CT in 120 patients with human cysticercosis].

A study on brain CT was made in 120 patients of human cysticercosis, which is a rare disease in Japan and clinical symptoms and laboratory data for the diagnosis were also discussed. From the point of therapeutic view, we proposed a new differentiation on brain CT of human cysticercosis, which is divided into two groups according to the alive or dead parasite. Furthermore, we proposed a new type named multiple large and small cysts type on brain CT. The idea of diagnostic standard was made integrating brain CT image, clinical symptoms and laboratory data.

[Experimental study of the antitumor effect of Z-100 in the treatment of MM46 tumor transplanted in C3H/He mice. 2. Effect of the combination therapy of radiation and long term administration of Z-100].

Female C3H/He mice aged 14 weeks with transplanted MM46 tumor were used to investigate the effect of an immunomodulator, Z-100 (an arabinomannan lipid extracted from Mycobacterium tuberculosis strain Aoyama B) combined with local irradiation of 30 Gy (3,000 rad). Daily doses of 5 micrograms, 50 micrograms and 500 micrograms/kg of Z-100 were injected intramuscularly for 14 consecutive days after irradiation, and 2 times a week for 6 weeks thereafter. The antitumor effect was evaluated by the changes in tumor volume and survival curves. In groups administered 50 micrograms and 500 micrograms/kg of Z-100, tumor growth decreased significantly compared with the control group (radiotherapy group). Concerning survival rates of each group of mice, there were no marked differences between Z-100 administered groups and the control group. To clarify the mechanisms of action of Z-100, the changes in the lymphocyte subsets infiltrated into tumor tissue after Z-100 treatment were analysed immunohistochemically using monoclonal antibodies, anti-Thy 1.2, anti-Lyt-1, anti-Lyt-2, anti-L3T4, MAS034b and MAS053c and avidin-biotin-peroxidase complex method (ABC method). In the findings of immunohistochemical studies, differences were hardly observed between groups administered Z-100 and groups treated with radiation only. From these results, it was concluded that immunological effects of Z-100 resembled that of radiotherapy on the topical tumor tissue.

The RT1.G locus in the rat encodes a Qa/TL-like antigen.

A new antigenic system in the rat homologous to the Qa/TL antigen system in the mouse has been characterized. It was detected by antibodies raised in donor-recipient combinations that were matched for the RT1.A, B, D, E loci in the major histocompatibility complex (MHC): (R11 X BN)F1 anti-BN.1L(LEW), (R18 X BN)F1 anti-BN.1L, and BN.1LV1(F344) anti-BN.1L. Absorption analyses using these antisera and a variety of inbred, congenic, and recombinant strains identified three alleles, RT1.Ga, Gb, Gc, of which Gc is a null allele. The strain distribution of these alleles was determined using 37 strains of rats representative of all of the prototypic haplotypes and a number of congenic and recombinant strains. The use of the congenic and recombinant strains showed that the RT1.G locus was linked to the MHC and that the most probable gene order was A-E-G. Testcross analysis showed that the map distance between A and G was 1.4 cM (4/285 recombinants). The RT1.G antigen has a heavy chain of Mr 46,000 and is present on both T and B cells.

Origin of müllerian duct and its later developmental changes in relation to wolffian duct in bovine fetuses.

At the 1.6-1.8 cm stages in both male and female bovine fetuses, the anlage of the Müllerian duct was first observed as an elevation of mesothelium on the cranial tip of the mesonephros. The duct extended toward the urogenital sinus in parallel with the Wolffian duct. Its caudal end was solid and was fused with the Wolffian duct at about the level of the middle of the gonadal anlage. Thereafter, the Müllerian duct became gradually independent of the Wolffian duct throughout its whole length. Therefore, it may be that the main part of the Müllerian duct is established by budding from the Wolffian duct. At the 4.3-4.5 cm stages the male Müllerian duct began to degenerate and the female one continued to grow. This suggests that the male Müllerian duct is affected by the Müllerian inhibiting substance at these stages. At the 12.0 cm stage, the female Wolffian duct disappeared. The male Wolffian duct was also much reduced in diameter at this stage, but began to grow again from the 14 cm stage onward.

Influence of adrenalectomy on the development of the neonatal thymus in the rat.

Adrenalectomy of 1-, 6-, 11- and 16-day-old male and female rats did not cause any changes in the weight of thymus 5 days later. Adrenalectomy on days 21, 26, 31 and 81 of age induced a significant hypertrophy of the thymus 5 days later. These results indicate that after about the time of weaning, the thymus is under some inhibitory regulation by the adrenal gland.

Origin of müllerian duct and its later development in relation to Wolffian duct and anogenital distance in the rat.

On day 14 of fetal life in the rat, Müllerian (paramesonephric) duct in both sexes was first observed close to Wolffian (mesonephric) duct. Its caudal end joined Wolffian duct at about the level of the middle of gonadal anlage. Thereafter, Müllerian duct became independent of Wolffian duct through its whole length. Therefore, the main part of Müllerian duct may be formed by budding from Wolffian duct. On day 16, the anogenital distance in male started to increase with thickening of Wolffian duct and disappearance of Müllerian duct. Castration of male fetuses on day 19 stopped later masculinizing development. Therefore, it is clear that the fetal testis is an organ crucial for the masculinization of the urogenital tract as it activates the secretion of masculinizing hormone(s).
