The case of a 38-year-old female with primary argyrophil cell carcinoma of the uterine cervix is reported. Two years after operation the patient developed widespread metastases with typical Cushing's syndrome. Microscopically, the tumor consisted of solid anaplastic cells, adenocarcinoma, and squamous cells. The plasma levels of ACTH and cortisol were elevated. Many cells of both the primary and metastatic tumors showed argyrophilia. Almost all the cells of the metastases contained numerous round secretory granules measuring about 117 micrometers in diameter. Small rod-shaped or larger round secretory granules, measuring 250 and 430 micrometers respectively, were also found in a few of these cells. The tumors in the right lung, pancreas, and kidney contained high levels of ACTH, beta-MSH, serotonin, histamine, and amylase. This is the first report of ectopic production of these five substances from argyrophil cell carcinoma of the uterine cervix.
Eight dissolution methods (beaker, rotating basket, oscillating basket, solubility simulator, rotating flask, and column) were evaluated using 21 commercial film-coated chloramphenicol (I) tablets and a nondisintegrating benzoic acid (II) tablet. The relative agitating intensities obtained from different dissolution methods were compared through the relative zero-order nondisintegrating tablet dissolution rate constants. Correlation coefficients between I dissolution rate parameters (lag time, T20, T50, and T80) were determined. Significant correlation was observed for the lag time among seven methods, and all pairwise regression lines passed through zero except one. The regression line slopes reflected the relative destructive force intensities produced by each dissolution method on the coated I tablet films. The seven dissolution methods could be classified into two main groups according to correlations of four dissolution rate parameters. The classification criterion agreed well with that based on the agitation method. However, dissolution methods may not be interchangeable even though they belong to the same dissolution method group.
The relationship between chloramphenicol (I) tablet bioavailability and in vitro dissolution rates was examined. The effect of solid food on the I tablet and powder bioavailability was also studied. Five tablets of I were selected for bioavailability testing on the basis of the dissolution rates of 18 I tablets (250 mg) determined by several methods. Compound I, 500 mg, was administered orally to five subjects, following overnight fasting, according to a crossover design. The bioavailability parameters were obtained from urinary I excretion. Among the five formulations studied, only one tablet (F) showed significantly poorer bioavailability. The dissolution rates at pH 1.2 did not give the same rank order as the bioavailability. The dissolution rate of Tablet F showed remarkable pH dependency. The dissolution rates at pH 4 showed good correlation with in vivo bioavailability data. The bioavailability of I powder was not affected by solid food. Tablet F, which had poor bioavailability in the fasting state, showed good bioavailability when administered just after the standard breakfast.

This reports concerns a 9 years old boy, who had situs solitus and atrioventricular concordance with double outlet left ventricle, pulmonary stenosis, tricuspid stenosis, ventricular septal defect, secundum atrial septal defects and hypoplastic right ventricle. Previous Blalock-Taussig shunt was created at the age of 11 months. Total correction consisted of closure of atrial septal defect and tricuspid valve orifice, and direct anastomosis of right atrial appendage to pulmonary trunk. Though atrial pacing was required for the immediate post operative period, the patient regained sinus rhythm 3 days after operation. He is doing well at present, one year after the operation.
An infected graft and a mycotic pseudoaneurysm were successfully resected by employing an ascending aortasupraceliac abdominal aorta bypass graft in a 19-year-old man. He had formerly undergone graft replacement surgery for traumatic aneurysm of the descending thoracic aorta, with the aid of a temporary external bypass graft. After this first operation, the patient had suffered from septicemia due to Psudomonas aeruginosa, which resulted in formation of mycotic pseudoaneurysms at the distal anastomotic site of the prosthetic graft and at both stumps of the formerly employed external bypass graft.
A model of experimental renal osteodystrophy was established in the rats with chronic renal failure induced by partial nephrectomy and therapeutic effects of 1 alpha-hydroxyvitamin D3 (1 alpha-OH-D3) were studied. Male Wistar rats weighing 180 g were 5/6 nephrectomized and fed a normal diet (Ca and P : 1%) for 6 months. After the surgery, serum creatinine levels increased 60% and thereafter continued to rise gradually with their growth for 4 to 5 months, followed by rapid increase. The serum phosphorus levels were also elevated concomitantly and the serum calcium concentrations were normal. Marked bone resorption accompanied with hypertrophy of parathyroid glands was observed by histological examinations (Tetrachrome-Fuchsin stain, contact microradiography and H-E stain). The bone resorption seemed to be due to secondary hyperparathyroidism. Treatment with 0.25 micrograms/kg/day p.o. of 1 alpha-OH-D3 for 10 days in the uremic state resulted in remarkable new bone formation which was confirmed by histological examinations. These results clearly demonstrated that the reduction of nephron mass play a critical clue of renal osteodystrophy and 1 alpha-OH-D3 appears to have a good potential for clinical use in patients with renal failure and metabolic bone diseases.
Bone changes in 138 patients under hemodialysis were evaluated by radiological methods. Bone mass as was assessed by clavicular score was reduced in patients with chronic renal failure compared with control subjects, especially in male patients, and showed the gradual decrease with aging in both sexes. The incidence of disappearance of lamina dura and that of vascular calcification increased with aging in chronic renal failure. Although serum PTH did not show much difference among various age groups, serum CT and calcium were significantly elevated in 20--24 years of age group compared with other age groups. The etiology of decrease of bone mass in male subjects was discussed from the standpoint of decreased gonadal activity.
Eighty-two patients underwent curative resection for squamous cell carcinoma of the esophagus. The 5-year actuarial survival rate was 33 per cent. Sixteen of 34 patients who developed secondary nodal metastases of the neck and mediastinum were irradiated with a dose of 40 to 60 Gy over 4 to 6 weeks (81 in average TDF), which probably prolonged the survival.
We performed immunohistopathologic studies on biopsied lung tissue obtained from two patients with lupus pneumonitis using immunofluorescence, immunoperoxidase, electron microscopy, and acid-microelution. In both patients, immunofluorescence showed granular deposits of IgG, the third component of complement (C3), and DNA in the alveolar walls. The immunoperoxidase technique in both and electron microscopy in one showed that these deposits were in the interstitium of the alveolar walls and in the alveolar capillary walls. The eluates obtained from cryostat sections of the biopsied lungs contained antinuclear factor of IgG class in one patient and showed anti-DNA antibody activity in both. We suggest that the deposits are immune complexes composed of DNA, anti-DNA antibody, and complement and that deposits of DNA-anti-DNA immune complex may play a role in lupus pneumonitis.
