Metacarpal and phalangeal lengthening by callus distraction.

Twelve metacarpals and two phalanges in 14 patients (nine males and five females) were lengthened between August 1992 and March 1999 by the callus distraction technique using a small external fixator (Orthofix). The indications were traumatic amputation (10 cases), aplasia (three cases) and hypoplasia owing to premature epiphyseal closure of the metacarpal (one case). All patients were reviewed with a median follow-up time of 39 (range, 9-88) months. The median age of the patients was 15 (range, 4-39) years. The median lengthening was 21 (range, 13-34) mm which was a median of 82% (range, 38-162) of the original bone length. The median times for callus distraction and consolidation were 4.5 and 7.5 weeks respectively. All except one patient tolerated the procedure well and were satisfied with the results. Functional improvement was seen in 13 out of 14 patients, but non-union was observed in one patient.

[Fireworks-caused injuries in Denmark].

The number and type of fireworks injuries occurring around New Year's Eve as well as type of fireworks used was investigated with the aim of guiding future prophylactic efforts. All patient contacts to emergency rooms throughout Denmark that were due to fireworks were prospectively registered for twenty-four hours before and after New Year's Eve 1995/1996 and 1996/1997. The number of fireworks injuries increased by 13.9% from 381 to 433, and the number of serious injuries increased by 50.8%. In 1996/1997 50% of the injuries were hand injuries, 19% were eye injuries, 8% were injuries to the head and neck, and the remaining 23% were hearing injuries and miscellaneous injuries. Seventy percent of the serious injuries were hand injuries, leading to some form of amputation in a third of cases. Sixty-one percent of the admissions were due to illegal fireworks. It is concluded that the number of serious fireworks injuries is on the increase, and that the main part of these injuries are hand injuries. Over half of the the serious injuries were due to illegal fireworks. Prophylactic efforts should be aimed against hand-held and illegal fireworks.

Observer variability in measurements of carpal bone angles on lateral wrist radiographs.

Determinations of carpal bone angles are used in the clinical evaluation of carpal malalignment. Eleven frequently referred radiological measures in lateral projection of the wrists in 23 wrists were assessed using different definitions of axes. Interobserver- and intraobserver variations were calculated. The standard deviation of the interobserver variation ranged from 2.60 degrees to 18.15 degrees, and the intraobserver variation from 1.89 degrees to 4.66 degrees depending on the angles measured. The use of three angles for the least observer variability in assessment of carpal alignment is recommended. These angles were defined from the following carpal bone axes: radius, the line through the center of the medullary canal at 2 cm and 5 cm proximal to the radiocarpal joint; lunate, the line perpendicular to the tangent of the two distal poles; scaphoid, the tangent of the palmar proximal and distal margins, and capitate, the tangent of the dorsal margin of the diaphysis of the third metacarpal bone (substitute axis).

Gentamicin-collagen sponge for local applications. 10 cases of chronic osteomyelitis followed for 1 year.

A bovine collagen sponge containing gentamicin was applied locally in 10 patients with chronic osteomyelitis who were then followed for 1 year postoperatively. No adverse effects or recurrence of infections was observed. The release of gentamicin from the sponge was more complete and rapid than has been reported with polymethyl methacrylate (PMMA) beads. Compared with previous reports on PMMA beads, a much higher gentamicin concentration in the wound exudate can be expected after implantation of the collagen sponge, which is easy to apply and does not require a second operation for removal.

Principles of evaluation and results in microsurgical treatment of major limb amputations. A follow-up study of 26 consecutive cases 1978-1987.

A total of 26 microvascular extremity replantations/revascularizations with a survival rate of 85% were followed for an average of 4.5 years. The material consisted of 11 upper arms, 11 lower arms, 1 wrist and 3 feet. In this comparatively small and inhomogenous material there was no significant influence found on the functional results regarding level of amputation, time of ischaemia or age of the patients. Sharp injuries had the best outcome. The functional results were evaluated following the principles outlined by Chen & Yu and by Tamai. Good and excellent results were found in 63% of the upper extremities according to these classifications and in 47% according to our own classification. Some of the difficulties in establishing indications and contraindications to replantation are outlined. To facilitate comparison of materials a modified and more realistic version of the score system of Tamai is recommended as a standard for evaluation of functional results following above-wrist-replantation. This will enable early detection of progress in the treatment of extremity amputations.

[Functional results after replantation/revascularization of major parts of the extremities].

Twenty-five patients who were submitted to replantation or revascularization of a total of 11 upper arms, 11 forearms, one wrist and three feet were followed-up with the object of assessing the functional status. 85% of the replantated/revascularized parts survived. The follow-up examination included assessment of the level of amputation, mechanism of the injury, duration of ischaemia and the age of the patient in relation to the joint mobility achieved, strength, sensibility, cosmetic inconveniences and the return to previous employment. Out of 19 surviving upper limbs and three feet, the results were classified as excellent and good after Tamai's and Chen's methods. The best results were found after sharp lesions but the investigation revealed that even in severe avulsion lesions, it proved possible to obtain good results. It is thus not possible to establish definite directives for the indications for replantation. The final decision concerning reconstruction cannot be made until after contact with one of the departments in which replantation can be undertaken.

Occurrence of methicillin-resistant Staphylococcus aureus in a department of orthopedic surgery 1970 to 1986.

The incidence of methicillin-resistant (and multiply-resistant) strains of Staphylococcus aureus in the Department of Orthopedic Surgery at Odense University Hospital fell from 15% of all strains in 1970 to less than 0.5% in 1986. This decrease occurred more slowly in the orthopedic department than in the rest of the hospital. A retrospective survey of 30 episodes of infection due to methicillin-resistant Staphylococcus aureus from 1979 to 1986 indicates that this slower decrease may be due to the relatively frequent occurrence of chronic infections in orthopedic surgery.

Early mobilization after flexor tendon grafting for isolated profundus tendon lesions.

After reconstruction of a flexor profundus tendon with a free graft passed through or around an intact superficialis decussation, impaired function may result if adherences develop. In an attempt to avoid adherences, early mobilization using Kleinert's method was applied in 20 grafts. At follow-up, on average 36 months after operation, all the patients had improved ROM except one patient with tendinous infection one year postoperatively. On average the gain in DIP-joint flexion was 34 degrees, while six patients had a loss of 17 degrees and 12 patients a gain of 22 degrees in PIP-joint motion. Only nine patients required a few weeks of occupational therapy. It is concluded that early mobilization a.m. Kleinert after tendon grafting is a safe method for prevention of loss of PIP-joint motion due to the formation of adherences.

Experience with microlymphaticovenous anastomoses for congenital and acquired lymphoedema.

Ten unselected, consecutive patients were operated upon for lymphoedema of a limb. In order to evaluate the effect of microlymphaticovenous anastomoses no conservative treatments were used concomitantly. Seven patients had some relief of symptoms postoperatively, while six patients had a decrease in limb circumference. Three patients with primary lymphoedema had no effect of this treatment. Lymphoscintigraphy is advocated as an aid to the selection of suitable cases for microsurgery. Our future indications for microlymphaticovenous surgery for lymphoedema will be limited to patients with secondary lymphoedema with a duration of less than five years. The disease will be diagnosed by clinical examination as well as by lymphoscintigraphy.

Ulnar thumb in cleft-hand-type congenital deformity. Case report.

A twelve-year-old boy with a two-digit cleft hand was successfully treated by stabilization, rotation and activation of the floating ulnar digit. This ulnar thumb proved to be a useful tool for daily life, but an even better result might have been achieved if the treatment had been given at an earlier age.

Conservative treatment of fingertip injuries.

In a prospective investigation of 81 consecutive patients with fingertip injuries conservative treatment was evaluated. Fingertip injuries were defined as lesions greater than or equal to 1 cm2 in the terminal phalanx without injury to the tendons or joints. All fingertip injuries were cleaned and covered by Vaseline gauze and left to heal. If less than 2 mm of soft tissue covered the bone a few millimetres of bone were nibbled away to allow good cover with soft tissue. The majority of injuries (64 per cent) occurred at work. The average healing time was 25 days. The main later complaints were intolerance of cold (36 per cent), numbness (36 per cent) and tenderness (26 per cent). None of the patients had stiff joints. On average, the two-point discrimination had increased by 1 mm in the injured fingertip. Conservative treatment is recommended as a safe and simple treatment of fingertip injuries, even when bone is exposed in the wound.
