Ultrasound-guided pigtail catheter drainage of malignant pericardial effusions.

Anterior pericardiocentesis was performed under ultrasound guidance using indwelling pigtail catheters in nine patients with malignant pericardial effusions and cardiac tamponade. Symptoms were alleviated in all cases and there were no deaths attributable to the procedure. Catheters remained in place for a median of 24 h (range 8-168 h). Complications were minor: asymptomatic left-sided or bilateral pleural effusions (n = 3), atrial fibrillation (n = 2), transient pericardial pain (n = 1) and erratic drainage (n = 1). Recurrence of the pericardial effusion was limited to a single case and occurred after 5 months of effective palliation. The procedure was successfully repeated with no further recurrence. One patient who went on to surgical fenestration 11 days after pericardiocentesis died 24 h post-operatively. The remaining patients died of the underlying malignant disease without further cardiac complication. Survival following the procedure ranged from 3 to 8 months. Ultrasound-guided pigtail catheter drainage is a minimally invasive, safe and effective method for the palliation of cardiac tamponade in patients with malignant pericardial disease.

Real-time ultrasonic characteristics of the breast.

An ultrasonic study of the breast of a group of 250 patients attending a breast screening clinic has been performed. Four types of breast can be recognised by their ultrasonic characteristics and the study shows how appearances alter with increasing age. The ultrasonic appearances correlate with the X-ray mammographic patterns described by Wolfe and have increased our understanding of some of the radiographic features. The implications for the use of ultrasound in the diagnosis of lumps and the assessment of mammographic abnormalities, and its potential role as a screening tool, are also briefly discussed.

Contemporary mediastinal tuberculosis.

Mediastinal lymphadenopathy is a prominent feature of adult tuberculosis in immigrant groups in the United Kingdom. Chest radiography of 95 tuberculous immigrants showed mediastinal gland enlargement in 29 cases, whereas none of the 42 Europeans showed this feature. An analysis has been made of the distribution of the disease in all groups together with a more detailed evaluation of mediastinal involvement in the immigrants. Pericardial effusions were seen in eight patients together with the following three unusual complications: (1) broncho-oesophageal fistula; (2) bronchial erosion; (3) superior mediastinal obstruction.

Radiology in early slipped femoral capital epiphysis.

In a study of 38 hips showing only minor abnormality on the AP radiograph, it was found that the most reliable sign of an early slip was blurring of the junction between the metaphysis and the growth plate. The diagnosis could not be made on the AP film alone in 11% of cases and it is recommended that a routine lateral radiograph is obtained when slipped epiphysis is a clinical possibility. The importance of the radiologist in the early diagnosis of the condition is stressed and the causes of diagnostic delay are considered.
