Cardiac and thoracic vascular injuries: anesthetic considerations.

Heart and thoracic vascular injuries account for 1% to 4% of trauma, but the potential for mortality and morbidity makes preanesthetic evaluation important. A rational management plan initiated from the time the patient is first seen in the emergency room will guide anesthetic management. Our review of 65 patients with cardiac and intrathoracic vascular injuries showed the preoperative evaluation facilitates selection of airway and anesthetic management. Extensive monitoring and placement of large intravenous lines allow prompt detection and treatment of hemodynamic deterioration.

Modular approach to CPR training.

Basic and advanced cardiopulmonary resuscitation training is worthwhile yet time consuming. Anesthesiologists should be among the most qualified leaders in CPR teaching programs. Using a modular self-teaching presentation for basic and advanced cardiac life support courses, minimal instructor time is required to accomplish proficiency in both didactic and mannequin performance for participants. This method can be adapted to various educational levels and time constraints without compromising quality of teaching.

Anesthetic management of neck trauma.

In a retrospective review of one year's experience in the management of neck trauma, 88 cases comprising 42 gunshot wounds, 29 stab or slash injuries, and 17 injuries by blunt trauma were reviewed. Multiple injuries were noted in all groups, but a higher percentage was found in the blunt trauma series. Anesthetic management is reviewed, and emphasis is placed on careful evaluation of the patient's airway, using physical and roentgenographic examination to plan adequate airway management.
