Mortality and morbidity after anterior lesser curve seromyotomy with posterior truncal vagotomy for duodenal ulcer.

A survey has been performed of the mortality and morbidity of anterior lesser curve seromyotomy with posterior truncal vagotomy in the elective treatment of chronic duodenal ulcer. There was one death in a series of 605 patients due to a myocardial infarction, an operative mortality of 0.16 per cent. There was no case of ischaemic necrosis of the lesser curvature or fundus of the stomach. Eleven patients had symptoms of delayed gastric emptying (1.7 per cent) and seven of these underwent a drainage procedure (1.3 per cent). Postoperative dumping did not occur, significant diarrhoea was present in two patients (0.33 per cent). This operation is relatively simple, quick and extremely safe to perform. It is suggested that the more widespread use of this type of elective surgery for duodenal ulcer might reduce the mortality from the condition.

Bile-duct cysts--diagnosis and treatment in 14 patients.

Between 1978 and 1982 14 patients were treated for bile-duct cysts. The predominant clinical signs were obstructive jaundice and hepatomegaly in the younger patients, while pain was the most common symptom in the older patients. Ultrasonography was a valuable diagnostic technic but percutaneous transhepatic cholangiography (PTC) and endoscopic retrograde cholangiopancreatography (ERCP) were associated with septic complications; as an alternative, routine preoperative cholangiography is recommended to visualize the type of bile-duct cyst. Excision of the bile-duct cyst was considered the treatment of choice. Excision was performed in 11, and internal drainage in three patients. Cholangitis developed in four children after excision of the cyst. In our series the prognosis was good following surgical treatment, even in younger patients.

An unexpected complication of a liver biopsy.

A case of anaphylactic shock after percutaneous liver biopsy is described. This was due to rupture of a large echinococcal cyst localized in the liver. Diagnosis and treatment with a review of the literature are discussed with emphasis on the complete removal of both the cyst and the surrounding capsule of fibrous liver tissue.

Direct femoral artery pressure measurement at rest and during reactive hyperemia in the evaluation of the aorto-iliac segment.

Direct femoral artery pressure (FAP) studies with reactive hyperemia (RH) were performed on 39 extremities which underwent a proximal or distal reconstruction for angiographically multilevel disease. The half-year clinical results were correlated with the FAP studies. A second group of patients underwent FAP studies and angiography and both studies were correlated. The conclusions from the studies are in the first place that FAP studies with RH are an accurate way to assess the aorto-iliac segment without giving any complications; secondly that observation of single plane angiography tends to overestimate aorto-iliac disease. Finally, criteria are proposed which can help in the clinical decision whether to perform a distal or a proximal reconstruction in multilevel atherosclerotic disease.
