How can CRNAs receive third party reimbursement as recognized providers of anesthesia service?

To remain professionally viable, CRNAs must receive fair and adequate reimbursement for services rendered. Denial of payment by third party insurers can result in frustration, loss of income, and potential negative impact on the right to practice. The insurance industry and other health care organizations categorically and conditionally deny reimbursement for a variety of reasons. Categorical denial is the greatest threat to practice. Conditional denial can usually be resolved. When discussing issues with insurers it is important to deal with those in a position to make or influence policy. It is important for individuals and their respective state associations to be familiar with pertinent insurance laws and regulations because they identify practitioner rights and claims that can be made against the insurer. Any categorical denial should be brought to the state association's attention and investigated for possible illegal action.

Ericksonian hypnosis and strategic interventions for sexual orientation confusion.

Erickson's utilization approach provides a model of hypnotic and strategic intervention for persons seeking psychotherapy because of sexual orientation confusion. Case studies outline examples of hypnotic and strategic short-term interventions. These cases include the use of metaphor, utilizing resistances, anchoring, and synesthesia.

Group psychotherapy for bisexual men and their wives.

A significant number of men and women experience conflict surrounding homosexual expression within marriage. As the media focuses more attention on these relationships, more bisexual men and their wives seek counseling or psychotherapeutic assistance. Based upon the author's research and clinical experience, this article outlines a group psychotherapy strategy for bisexual men and their wives. Relevant research and background data on mixed-orientation marriages are summarized. Treatment strategy explores intervention processes, resistances to treatment, the unique supportive experiences of the group, and qualities of the co-therapist who may provide assistance to bisexual men and their wives.

Sexual orientation: a multi-variable dynamic process.

Theory and research concerning sexual orientation has been restricted in its scope and influence by the lack of clear and widely accepted definitions of terms like heterosexual, bisexual, and homosexual. In an attempt to better demarcate and understand the complexities of human sexual attitudes, emotions, and behavior, the Klein Sexual Orientation Grid (KSOG) was developed and administered. The KSOG is composed of seven variables that are dimensions of sexual orientation, each of which is rated by the subject as applying to the present, past, or ideal. Analysis of the data from subjects who filled out the KSOG in Forum Magazine indicated that the instrument was a reliable and valid research tool which took into consideration the multi-variable and dynamic aspects of sexual orientation.

Marriages of bisexual men.

This study examined the marriages of 26 couples in which the husband was bisexual. The subjects were a non-clinical sample married for at least two years and intending to continue their marriages. The sample was, overall, highly educated and earned concomitantly high incomes. Subjects were administered the Klein Sexual Orientation Grid and a research questionnaire to determine successful or problematic aspects of their marriages. Subjects were, for the most part, satisfied with the quality of their marriages, sexually active within the marriages, and open about the husband's homosexual behavior. A high-level of sexual activity within the marriage, open and direct communication, a valued friendship, previous counseling or psychotherapy, cognitive flexibility, and financial independence contributed to the success of these marriages. The husbands reported a great deal of ambiguity about their homosexual behavior, and the couples reported intense conflict dealing with their open marriage styles.
