A survey of 1006 surgical clinics in the Federal Republic of Germany in 1984 showed that the general surgical procedure for duodenal ulcers was vagotomy in 65.9%. 21.9% of the clinics checked the effectiveness of vagotomy intraoperatively by either performing electrostimulation (69.6%), leucomethylene-blue staining (20.9%), monitoring of the intragastric pH-value (7.3%), or Congo-red test (1.4%). Postoperative vagotomy controls were performed in 51% of the hospitals. Judging by the present results, application of intraoperative effectiveness control is not necessarily advisable. Evaluating the pre- and postoperative acid production allows the establishment of a quality control, however without any prognostic relevance.
Pre-operative computed tomograms were obtained in 52 patients with histologically confirmed peripheral bronchial carcinomas and possible involvement of the pleura and thoracic wall was analysed. The results were compared with the operative and histological findings. Thickening of the pleura and of the sub-pleural fat line was observed in 24 cases. Amongst these patients there was only one case with histologically confirmed pleural infiltration. In 17 patients the sub-pleural fat line was obliterated, or could not be defined. Amongst these, 11 showed tumour extension to the pleura or soft tissues of the chest wall. It is concluded that the most suspicions sign of pleural tumour extension is absence of the sub-pleural fat line. The more frequently observed pleural thickening cannot be considered as a reliable sign for pleural involvement.
The aim of these examinations was a comparison of the tensile strength and the histology of single and two-layer sutured gastrotomies applied to rats. Independent of the applied suture material and suture technique all experiments resulted in the same tensile strength of the suture between the 5th and 7th postoperative day as in the control group. Differences of anastomotic dehiscence did not exist. The single-layer can be loaded as well as the two-layer suture. Any difference of the tensile strength of anastomosis depending on the suture material does not exist.
Sensory component, reflexes and motor components are responsible for anorectal continence. The most important factors are the extraperitoneal part of the rectum, the M.m. sphincter ani internus and externus, the levator ani muscles and the corpus cavernosum recti. The diagnostic of anal incontinence is usually evident from history and physical examination. Additionally intraluminal pressure recordings and electrical measurements are helpful.
26 patients suffering from nonresectable adeno-carcinoma of the rectum have been treated since Sept. 1980. The mean age was 72 years. The temperature of the probe tip was -160 degrees C. A closed system with liquid nitrogen was used. In 12 patients there was an indication of systemic inoperability, 13 had an irremovable tumour or metastases. 1 patient refused curative operation. Cryosurgery was aimed at reducing the mass of rectal tumours and preventing colostomy. Colostomy could also be avoided in 12 of 14 patients suffering from stenosing tumours. The advantages of cryotherapy are outpatient treatment, no anaesthesia and minimal complications. Generally, this method will yield good results in palliative treatment of inoperable rectal carcinomata.

After the operation of hyperparathyroidism not only the level of calcium but also the postoperative course of the parathormone is a very informative parameter. In contrast to the serum calcium there is a very delayed normalisation of the values of parathormone in single cases of primary hyperparathyroidism. In cases of recurrent primary hyperparathyroidism the value of parathormone increases much earlier than the value of serum calcium and before the patient has any complaints. So the parathormone enables the recurrent or persistening hyperparathyroidism to be early recognized or differentiated respectively. In the course of the operative therapy in secondary hyperparathyroidism the recrudescence of the level of parathormone points to a successful replantation of the epithelial bodies. In general increased postoperative values are not to be equated with the recurrence of the disease.
A squamous-cell carcinoma of the anorectal junction had developed from a chronic anal fistula in a 27-year-old woman with Crohn's disease who had been suffering from anal and rectovaginal fistulas for many years. In view of their cancerogenic potential, chronic fistulas associated with Crohn's disease must be followed up closely by clinical examination, endoscopy and biopsy. If surgery is indicated, resection of such fistulas should be considered at an early stage.
We reviewed 28 cases of primary retroperitoneal tumors (1972 to 1982). We found 22 malignant and six benign tumors. The tumors revealed no special features. Cardinal symptom was a palpable abdominal mass in 21 of 28 patients (75%). Since 1978 we have at our disposal a modern radiological technique, the computerized tomography, which allows a statement of tumor localization, tumor size and infiltration of neighbouring organs. In our patients no definite statement could be made about resectability and type of the tumors. In spite of computerized tomography there is no change the road-minded operation indication. Surgical therapy is the treatment of choice because of lacking alternatives. Complete extirpation to cure, respectively exploratory laparotomy and biopsy to determine the specific nature of the tumor by histological examination should be aspired. In individual cases palliative gastrointestinal anastomoses are necessary to treat intestinal obstruction. All six benign, but only six of 22 (27%) malignant tumors were completely removable. Although in some cases neighbouring organs were respected, no patient died after operation. For a curative therapy the early diagnosis of the tumors is necessary.
After total gastrectomy because of gastric malignancies 31 oesophagojejunostomies performed in end-to-side fashion using a standard manual technique were compared with oesophagojejunostomies performed in end-to-end fashion by mechanical stapling (EEA). After manual suturing there was a leak in 29% and 25.8% out of all patients died. After automatic mechanical suturing anastomotic failure was seen in 12.9% with a postmortality rate of 3.2%. These results show, that by mechanical stapling a secure oesophagojejunostomy is possible and the risk of gastrectomy can be reduced.
Anterior colorectal end-to-end anastomosis by stapler was achieved in 61 patients between 1979 and 1981. The clinical occurrence of anastomotic leakage was 8,2%. The main advantage of the stapler is in anastomosis low in the pelvis where it allows easier access to the extraperitoneal part of the rectum. In patients with colorectal carcinoma in the middle part of the rectum the number of abdomino-perineal excisions decreased from 10,2% in 1979/80 to 3% in 1981. Recurrence of carcinoma took place in three cases after radical resection of rectosigmoidal tumor. Therefore repeated follow-up examinations with endoscopy in these cases are imperative.
Between 1974 and 1981 27 patients underwent parathyroid exploration for hyperparathyroidism at the Department of Surgery, University of Cologne. 18 patients could be reexamined postoperatively. Parathormone levels, serum calcium, serum phosphates and alkaline phosphatase in serum were measured. Recurrence of hyperparathyroidism did not occur. In our patients the excision of a single parathyroid adenoma and subtotal respectively total parathyroidectomy for parathyroid hyperplasia was the adequate therapy of hyperparathyroidism. Up to 82 months postoperatively the patients were without complaints. The determination of parathormone is an important factor in the follow-up program.

The diagnosis of congenital megacolon can only rarely be established in adult cases with dilatation of the rectum and sigma, since the narrow aganglionic segment and increased levels of plasma acetylcholinesterase activity are usually lacking in adult cases,--these findings being typical for congenital megacolon. Idiopathic megacolon thus is the correct diagnosis in most of these adult cases. Surgical removal of the dilated parts of the intestine and end-to-end anastomosis yield satisfactory results, thus a colostomy is not necessary.
Sequelae of vagotomy in peptic ulcer are dysphagia, dumping syndrome, gastric emptying disturbances, diarrhoea, and functional disturbances of liver, bile ducts and pancreas. All these disturbances, except dysphagia, are less pronounced after high selective vagotomy than after surgical procedures including drainage techniques. Relapsing ulcer is the most frequent complication; this is rarely a direct consequence of vagotomy, but it is more likely due to non-complete transection of the nerve or to transections not indicated.
Complaints after cholecystectomy are caused by bile duct disturbances (concrements in the choledochus, stenosis of the papilla, anatomic anomalies of the ducts), by diseases of neighbouring organs (pancreas, liver, stomach) or by functional disturbances. Diagnosis of organic disease can be established by appropriate means, that of functional disturbances by exclusion of organic causes. Therapy depends upon the underlying disease. Improvement of endoscopic procedures has let to the point, that in many cases secondary operations have become unnecessary. Close cooperation is necessary between internist, surgeon, and endoscopist, if an appropriate therapeutic procedure is to be found.
