Rabies antibody titers in vaccinees: protection, failure and prospects.

Treatment of symptomatic rabies is a challenging task as no effective drug is yet available. Hence, prophylactic immunization is an important step. The development of adequate level of antibody (greater than or equal to 0.5 IU/ml serum) is necessary for protection against the disease. In contrast to the nerve tissue vaccines, tissue culture vaccines particularly human diploid-cell strain type is highly potent and least reactogenic. To minimize the costs of immunization by this expensive vaccine, small dose intradermal administration has been advocated for developing countries with endemic rabies. Concurrent use of serum and antimalarials may interfere with adequate immune response. Efforts are in progress for the development of more potent subunit and recombinant antirabies vaccine for future human use. This review makes an attempt to study the antibody levels in the vaccinees regarding its role in the protection and failure against clinical and experimental rabies.

Schmidt's syndrome.

Schmidt's syndrome is a condition where there is auto-immune Addison's disease associated with primary hypothyroidism. We present one such case in view of its rarity in Indian literature.

Nasal cartilaginous tumour.

A cartilaginous tumour of nasal region in a 6-year-old child is described. The problems of histological interpretation and treatment are discussed.

Chronic myeloid leukaemia with massive lymphadenopathy.

A 35 year old patient with chronic myeloid leukaemia presented with massive lymphadenopathy and unilateral limb oedema, when still in the chronic phase. Four weeks later, she passed into frank blast crisis, despite treatment with busulphan and palliative radiotherapy to the lymph nodes.

Haemorheology in diabetes.

The rheology of blood is disturbed in diabetes due to increased viscosity of blood and decreased deformability of red blood cells. This disturbed haemorheology contributes to diabetic vasculopathy and is reversible with treatment with insulin.

Peripelvic extravasation and formation of perinephric urinoma after cystoscopy.

A hitherto undescribed complication of cystoscopy is reported wherein peripelvic extravasation and perinephric collection of urine resulted from conventional cystoscopic examination in a patient with a small-capacity bladder and vesicoureteral reflux. Prompt recognition, institution of indwelling catheter drainage, and massive antibiotic therapy led to spontaneous resolution of the flank mass. In similar cases, we suggest the use of an Iglesias resectoscope with a continuous flow and suction evacuation, even for cystoscopy and bladder biopsy.
