Users and "teasers": failure to follow through with initial mental health service inquiries in a child and family treatment center.

This article examines the experience of families who contacted a child and family community mental health treatment center, but then did not follow through to receive even one session of diagnosis or treatment at the agency. The paper reports the base rate for this behavior in a large (N = 2,358) sample of cases who either only made inquiry or who received treatment at the agency. The inquiry group, or "teasers," tended to have children who were older than client groups, to have relatively more behavioral and fewer personality problems, to have demonstrated problems earlier than client groups, and to have parents who were slightly older. A follow-up study was conducted with 100 inquiry and 100 client cases. The relative efficacy of telephone and written questionnaire methodologies are compared, demonstrating that the telephone approach was more effective. Client-group children were reported to be only moderately better off at the time of follow-up than the inquiry group, but conclusions about the effects of treatment are limited by the finding that many members of the inquiry group had sought treatment elsewhere after their brief contact with the agency. Implications of this study for community mental health administration and for future research in this area are discussed.

Types of immune complexes in the ascitic fluid of women with carcinoma of the ovary.

Immune complexes with C1q-fixing properties and those precipitable by polyethylene glycol (PEG) were detected in ascitic fluid from patients with advanced ovarian cancer. The ascitic fluid from 42 of 58 patients (72%) contained these complexes. A positive result with the C1q assay was obtained in 41% of patients, whilst with the PEG assay a positive result was obtained in 59% (50% in the IgM, 36% in the IgG, and 14% in the IgA fraction). The highest mean level of PEG-precipitable complexes was in the IgG fraction (11.4 mg/100 ml) and lowest in the IgA fraction (3.3 mg/100 ml). These results indicate that gram quantities of the immune complexes may be isolated from the large volumes of ascitic fluid usually present in ovarian cancer. Further studies of ascitic fluid may thus provide data on the nature of the immune responses in these patients.

Early pregnancy factor as a monitor for fertilization in women wearing intrauterine devices.

Measurement of early pregnancy factor (EPF) by the rosette inhibition test was performed on serum samples from 14 women using intrauterine devices (IUDs). Serial blood samples taken during the luteal phase of 23 cycles demonstrated in six of the cycles a transient appearance of EPF during the 6- to 8-day period following ovulation. In contrast, no EPF activity was found in sera obtained from women in whom fertilization was prevented, either by sexual abstinence or tubal ligation. These observations support the postulate that the IUD functions by the prevention of implantation of the fertilized ovum, rather than by preventing fertilization.

Validation of the rosette inhibition test for the detection of early pregnancy in women.

The authors validated use of the rosette inhibition test for the detection of early pregnancy factor (EPF) in human pregnancy, first by optimizing conditions for the assay, using known pregnant and nonpregnant sera, and second, by examining the performance of this assay in three clinical situations: a "blind" study involving coded first-trimester sera showed 80% correlation with pregnancy status; serial assay of EPF activity in sera collected from normal women attempting to conceive, correlated with human chorionic gonadotropin beta-subunit (beta-hCG) levels and pregnancy status; a longitudinal study of serial serum samples through two normal pregnancies showing the continued presence of EPF until the early third trimester in each case. It was concluded that with the rosette inhibition assay, consistent demonstration of a pregnancy-associated substance (EPF) could be obtained.

Fertilization and early pregnancy loss in healthy women attempting conception.

The rosette inhibition test was used to monitor the activity of an early pregnancy factor (EPF) in sera collected from 18 healthy women who were attempting to conceive. Serial blood samples collected during the luteal phase of 21 menstrual cycles demonstrated the appearance of EPF in 14 of the cycles, of which six showed only transient EPF activity over a 5-10 day period following ovulation. These results indicate a 67% rate of fertilization and a 36% rate of early conceptus losses.

Detection of an immunosuppressive factor in human preimplantation embryo cultures.

Within 48 hours of fertilization of human preovulatory oocytes in vitro, an immunosuppressive factor was detected in the culture medium by a rosette inhibition test. Cultures in which fertilization of the oocyte was unsuccessful were indistinguishable from control medium. Release of an early pregnancy factor from the preimplantation embryo could be of clinical significance to the establishment of pregnancy after embryo transfer.

Early pregnancy factor: its role in mammalian reproduction--research review.

An immunosuppressive early pregnancy factor associated with mammalian reproduction is currently attracting considerable interest. Research into its detection, site of production, distribution, immunosuppressive property, characterization, and application is in progress in a number of laboratories. This review aims to crystallize the current research findings and to identify significant areas for further investigation and application.

Detection of cell-mediated immunity to spermatozoa in mice and man by the leucocyte adherence-inhibition test.

A modification of the leucocyte adherence-inhibition test was developed to study cell-mediated immunity to spermatozoa. The test is highly reproducible and requires very low numbers of leucocytes. Deposition of spermatozoa in the genital tract of the female mouse by one normal mating is sufficient to sensitize the female to sperm antigens and the degree of sensitization is independent of the number of matings and the strain of male, and is not enhanced by subsequent pregnancy.

Specificity and possible origin of anti-N antibodies developed by patients undergoing chronic haemodialysis.

Studies were performed on two anti-N sera from patients who had formed anti-N antibodies during their period of haemodialysis. No specific inhibition of these antibodies was obtained by extracts prepared from a new and a used dialysis unit, and it was concluded that there was no evidence of a compound in the units which would stimulate production of the anti-N. The specificity of the anti-N's was shown to be against a precursor of MN antigens, since all activity in the sera could be removed by neuraminidase-treated M cells, which expressed MN precursor. Since the anti-N's would react with untreated N cells only at 4 degrees C, but with formaldehyde-treated cells at 4, 25 and 37 degrees C, it was concluded that the specificity of the anti-N's was directed against an antigen developed by red cells after formaldehyde treatment. It is proposed that the production of the anti-N antibodies is stimulated by formaldehyde-treated red cells passing into the patient during re-use of a dialysis unit sterilized with formaldehyde.

Two esterases common to seminal plasma, other external secretions and leucocytes in man.

Two esterases previously identified in seminal plasma, on the basis of their electrophoretic mobility, immunogenicity and sensitivity to organophosphorus esters were detected in various human external secretions and leucocyte extracts. alpha-Esterase was found to be abundant in urine and milk whereas this enzyme occurred in a low concentration in seminal plasma, leucocyte extract, cervical mucus and sweat. The concentration of betagamma-esterase was high in seminal plasma and leucocyte extract. The enzyme was present in small amounts in urine, milk, gastric juice, saliva, sputum, tears, cervical mucus and sweat.

The isolation and characterization of lactoferrin from sow milk and boar seminal plasma.

Lactoferrin isolated from sow milk (about 0-6 mg/ml) was shown to be chromatographically homogeneous, an observation supported by electrophoresis and by reaction against monospecific anti-lactoferrin antiserum. Isoelectric focusing showed multiple forms of the protein (i.e.p., 9-3 to 10-0) converted by neuraminidase to one form (i.e.p., 9-65). Boar seminal plasma contains immunologically identical lactoferrin (0-1 to 0-5 mg/ml) which binds strongly to boar spermatozoa.
