Increased eosinophil oxidative metabolism by treatment with soluble intercellular adhesion molecule-1.

Adhesion molecules may play an important role not only in adherence of inflammatory cells (particularly eosinophils) to an inflamed focus but also in activation of these cells. It is therefore of interest to evaluate eosinophil activation via intercellular adhesion molecule-1 (ICAM-1) and the beta 2-integrin family, namely CR3 (Mac-1), lymphocyte function-associated antigen (LFA)-1 alpha and LFA-1 beta, which are ligands for ICAM-1. Reactive oxygen species generated by eosinophils have also been considered capable of causing airway injury at the inflamed focus. This study examined the effect of recombinant soluble ICAM-1 and its ligands on eosinophil-induced radical oxygen products in terms of luminol-dependent chemiluminescence. Recombinant soluble ICAM-1 augmented eosinophil oxidative metabolism. It was concluded that signaling via adhesion molecules might play an important role in the pathogenesis of allergic inflammation through activation of eosinophils, e.g. an increase in oxidative metabolism.

Adhesion to fibronectin augments eosinophil radical oxygen products.

Fibronectin, an extracellular matrix component, is a ligand for very late activation antigen (VLA)-4, which is one of the beta 1-integrin family of molecules expressed by eosinophils. This study examined the effect of adherence to fibronectin on radical oxygen products from eosinophils. Adhesion of eosinophils to fibronectin resulted in enhancement of eosinophil production of radical oxygen species, as determined by luminol-dependent chemiluminescence of eosinophils stimulated with calcium ionophore. It was concluded that eosinophil adhesion to extracellular matrix via adhesion molecules may be important in the pathogenesis of allergic inflammation through eosinophil activation.

Possible release of eosinophil granule proteins in response to signaling from intercellular adhesion molecule-1 and its ligands.

The presence of a large variety of membrane receptors and the identification of cytotoxic molecules (mainly granule basic proteins) have indicated that eosinophils should br considered as effector cells. It has recently been suggested that adhesion molecules, particularly intercellular adhesion molecule-1 (ICAM-1), play an important role in allergic inflammation, for example in bronchial asthma. This study therefore investigated the possible release of granule protein in response to signaling from ICAM-1 and its ligands. The concentrations of eosinophil cationic protein and eosinophil-derived neurotoxin in supernatants of eosinophils were significantly greater (p < 0.05) in the presence of recombinant soluble ICAM-1 than without it. These results suggest that signaling from ICAM-1 and its ligands might induce eosinophil activation and might be involved in degranulation of eosinophil granule proteins, e.g. eosinophil cationic protein and eosinophil-derived neurotoxin.

[Effect of rantes (regulated on activation, normal T expressed and secreted) on eosinophil adhesion to plasma coated glass using eosinophilic cell line (EoL-1)].

Recently, adhesion molecules expressed on various inflammatory cells, especially on eosinophils are considered to play an important role in allergic inflammation such as bronchial asthma. RANTES (Regulated on activation normal T expressed and secreted) seems to be the most important chemokine which belongs to Platelet factor 4 (PF4) superfamily, since RANTES is revealed as a chemotactic factor for eosinophils as well as memory T cells or monocytes. In this study, we examined the effect of RANTES on eosinophil adhesion to plasma coated glass using eosinophilic cell line named EoL-1. RANTES augmented EoL-1 adherence to Plasma coated glass and EoL-1 adherence to plasma coated glass was inhibited by anti-VLA-4 antibody, anti-CR3 antibody. On the other hand, RANTES did not augment expression of adhesion molecules (VLA-4, CR3) on EoL-1. These findings indicate that RANTES intensify the adhesion of eosinophils without augmentation of their expression, and suggest that it induce qualitative, but not quantitative changes in their adherence. Taken together, our study suggest that RANTES up-regulates the inflammatory reaction through augmentative effect on adhesion of eosinophils in bronchial asthma.

[Effect of cytokine RANTES on induction of activated eosinophils].

The cytokine RANTES (regulated on activation, normal T expressed and secreted) is a member of the platelet factor 4 superfamily and is a chemoattractant for eosinophils. To study the effect of RANTES on induction of activated eosinophils, flow cytometry was used to measure the expression of intracellular EG2 in eosinophils. The concentration of eosinophil cationic protein in the culture supernatant of eosinophils stimulated with RANTES was also measured. Expression of EG2 and the mean fluorescence intensity ratio were greater in eosinophils stimulated with RANTES than in control eosinophils. Eosinophils stimulated with RANTES also produced more eosinophil cationic protein than did control eosinophils.

Changes in serum amylase level following hepatic resection in chronic liver disease.

OBJECTIVE: Factors that were likely to cause hyperamylasemia following hepatic resection were studied. DESIGN: Case-comparison study. SETTING: Institutional practice. PATIENTS: Seventy-one patients who underwent hepatic resection because of primary or secondary liver tumors and other benign disease, and who had no history of pancreatitis, were divided into four groups. Patients were divided into groups according to the presence or absence of underlying liver disease and the vascular occlusion methods used during hepatic resection (Pringle maneuver or the hemihepatic vascular occlusion technique). The Pringle maneuver was chosen for patients in whom the duration of liver parenchymal transection was expected to be short and/or the hepatic hilum had severe adhesion precluding the safe dissection of the hepatic artery and portal vein. MAIN OUTCOME MEASURES: Serum amylase levels were measured on the preoperative day and on postoperative days 1, 2, 4, 6, 7, 10, and 14. Preoperative liver function, operative blood loss, operative time, and vascular occlusion time were examined. RESULTS: Patients with chronic liver disease (CLD group) had high serum amylase levels during the preoperative and postoperative periods. Patients in the CLD group who underwent the Pringle maneuver (Pringle-L group) had significantly elevated postoperative serum amylase levels in comparison with their preoperative serum amylase levels. Pancreatitis developed in two patients in the Pringle-L group--one of them died. CONCLUSION: These results strongly suggest that prolonged complete occlusion of the portal vein for hepatectomy in patients with chronic liver disease has a serious influence on postoperative serum amylase levels.

Induction of beta 2 integrin expression on an eosinophilic cell line (EoL-1) by the supernatant of mononuclear cells stimulated with specific allergen from asthmatic patients.

Adhesion molecules recently have been considered to play an important role in inflammatory processes in bronchial asthma. Our previous study revealed high expression of beta 2-integrin family (CR3, LFA-1 alpha, CD18) on hypodense eosinophils. Thus, from the point of view of cell-to-cell interaction between mononuclear cells and eosinophils, we examined whether the supernatant of mononuclear cells obtained from mite-allergic asthmatic patients cultured with specific allergen mite-allergen is involved in adhesion molecule expression using an eosinophilic cell line (EoL-1). These characteristics of beta 2-integrin family expression (high expression of beta 2 integrin) were induced by the supernatant of mononuclear cells from mite-allergic asthmatic patients stimulated with mite-allergen as well as with a combination of the recombinant eosinophilopoietic growth cytokines (IL-3, GM-CSF and IL-5). Thus, we could conclude that some cytokines produced by specific allergen stimulated mononuclear cells in asthmatics might be involved in allergic inflammation through the induction of adhesion molecule expression on eosinophils in asthma or allergic disorders.

RANTES augments radical oxygen products from eosinophils.

RANTES, which is released from thrombin-stimulated platelets, is a member of the 8-kD cytokine family that has been shown to possess chemotactic activity for eosinophils. Thus, in this study, we examined the effect of RANTES on radical oxygen products from eosinophils. RANTES treatment resulted in the enhancement of peak value and integrated value of productivity of eosinophil-mediated radical oxygen products determined by luminol-dependent chemiluminescence of EoL-3 cells stimulated with A23187. In addition, the radical oxygen products of EoL-1 or eosinophils showed similar results. Thus, we could conclude that platelets might play an important role in the pathogenesis of allergic inflammation through the involvement in the selective eosinophil infiltration and eosinophil activation by releasing RANTES.

Induction of expression of adhesion molecules on an eosinophilic cell line (EoL-1) by the supernatant of lymphocytes stimulated with specific allergen from asthmatic patients.

Recently, adhesion molecules have been considered to play an important role in inflammatory processes in bronchial asthma. To extend our understanding of the high-intensity expression of adhesion molecules (CR3, LFA-1 alpha, LFA-1 beta, ICAM-1) on hypodense eosinophils, which was observed in our previous study, we examined whether the supernatant of lymphocytes from mite-allergic asthmatic patients is involved in adhesion molecule expression using an eosinophilic cell line (EoL-1). These characteristics of adhesion molecule expression were induced by the supernatant of lymphocytes obtained from mite-allergic asthmatic patients cultured with specific allergen as well as a combination of recombinant cytokines (IL-3, GM-CSF, IL-5). Thus, we could conclude that some lymphokines produced by specific allergen in asthma might be involved in the high-intensity expression of adhesion molecules on hypodense eosinophils in asthma or allergic disorders.

Effect of platelet-activating factor and platelet factor 4 on eosinophil adhesion.

The effect of platelet-activating factor (PAF) and platelet factor 4 (PF4) on the adhesion of isolated human eosinophils or eosinophilic cell lines (EoL-1, EoL-3) was examined. Both PAF and PF4 augmented eosinophil adhesion to plates coated with AB plasma or recombinant soluble intercellular adhesion molecule-1 (r-sICAM-1). These findings suggest that PAF and PF4 not only modulate chemotactic activity of eosinophils but also intensify the function of eosinophil adhesion. Since PAF and PF4 induce the expression of adhesion molecules (LFA-1 alpha, LFA-1 beta, CR3) on eosinophils, we could conclude that PAF or PF4 are closely related to eosinophil accumulation not only as chemotactic agents but also as augmentative agents for eosinophil adhesion through involvement in functional eosinophil adherence as well as surface expression of adhesion molecules on eosinophils.

Radical operation after portal embolization for tumor of hilar bile duct.

The clinical outcomes of patients with carcinoma of the proximal bile duct who underwent portal embolization of the right lobe followed by right extended lobectomy with (n = 3) and without (n = 9) pancreatoduodenectomy, are described. Preoperative embolization of the portal venous branch of the right hepatic lobe, which was performed with the intent of preventing postoperative hepatic failure, induced a significant increase in the volume of the future remnant liver (left lobe) (from 437 +/- 88 to 544 +/- 97 milliliters, p < 0.01). No patients died in the hospital after the resectional operation. One patient, who had poorly differentiated adenocarcinoma, died as a result of tumor recurrence 12 months after operation, and in another patient, distant lymph node metastases were found. The remaining ten patients are alive and well with no signs of recurrence. The three who underwent hepatopancreatoduodenectomy are leading a normal life 14, 21 and 22 months postoperatively.

[Effect of platelet-activating factor (PAF) on eosinophil adhesion to plasma-coated glass using eosinophilic cell lines (EoL-1, EoL-3)].

Adhesion molecules on various inflammatory cells, especially on eosinophils are now considered to play an important role in allergic inflammations such as bronchial asthma. In this study, we examined the effect of platelet-activating factor (PAF) on eosinophil adhesion to plasma-coated glass using eosinophilic cell lines (EoL-1, EoL-3), since PAF might be an important mediator as an eosinophil chemotactic agent in bronchial asthma. PAF markedly augmented adherence to plasma-coated plates. This finding suggests that PAF might up-regulate the inflammatory reaction in bronchial asthma through its action as an augmentative agent for eosinophil adhesion as well as its action as an eosinophil chemotactic agent.

Resection for multiple metastatic liver tumors after portal embolization.

BACKGROUND: Five patients with bilateral multiple liver metastases (3 to 12 lesions) from colorectal cancer who underwent extensive liver resection after portal embolization are described. METHODS: Portal embolization of the right portal branch was performed 9 days to 8 months before hepatic resection. The location and number of metastases were determined by intraoperative ultrasonography at the time of liver resection to accomplish complete resection of the tumors. Extended right lobectomy was carried out in four patients, two of whom underwent additional wedge resection of nodules located in the left lateral segment. The other patient underwent right lobectomy associated with local resection of the tumor in the left lobe. RESULTS: The postoperative course in the five patients was uneventful, with no serious complication or liver dysfunction. Although one patient died of recurrence 28 months after liver resection, the remaining four patients were alive and free of cancer between 36 and 74 months after hepatectomy. CONCLUSIONS: The presence of bilateral multiple (four or more) metastatic liver lesions from colorectal cancer is not considered a contraindication for hepatic resection if thorough examination of the liver is performed with intraoperative ultrasonography and the surgical risk is minimal. Portal embolization appears effective for increasing the safety of hepatectomy for patients with small metastases who require major right-sided resection combined with wedge resection of the left lobe.

Intraportal US with 20-MHz and 30-MHz scanning catheters. Work in progress.

Intraportal ultrasonography (US) with scanning catheters was attempted in nine patients with various biliary tract diseases, to evaluate the portal venous system for tumor invasion. Intraportal US was performed successfully in seven patients. Intraportal US scans were compared with images obtained with arterial portography, direct portography, and intraoperative US. Intraportal and intraoperative US revealed cancer invasion into the wall of the portal venous trunk in two patients. Negative findings for cancer invasion with intraportal US were verified with intraoperative US and laparoscopic examination in five cases. Arterial portography was unable to delineate the fine portal venous wall configuration in six of the seven patients, and direct portographic images were not fully diagnostic of tumor invasion in three cases. The improved detection of tumor invasion to the portal venous system with intraportal US was helpful in determining the appropriate treatment for biliary tract disease in these patients.

[A case of acetaminophen-induced pneumonitis].

A case of acetaminophen-induced pneumonitis is described. A 63-year-old woman took 2350 mg of acetaminophen for fever. She subsequently developed a non-productive cough and dyspnea. Chest X-ray revealed diffuse reticulonodular shadows in bilateral lung field and she was admitted to our hospital. PaO2 was 45.0 Torr and PaCO2 35.7 Torr while breathing room air. Bronchoalveolar lavage examination showed an increase in the percentage of lymphocytes and a decrease of CD4/CD8 ratio. Microscopic examination of a transbronchial lung biopsy specimen showed alveolar septal thickening. The lymphocyte stimulation test was positive for acetaminophen, with a stimulation index of 237%, while other drugs used were negative. Because drug-induced pneumonitis was suspected, all drugs were stopped and she was administered methylprednisolone. Consequently her symptoms, laboratory data and chest X-ray findings improved. Based on these findings, we diagnosed this case as acetaminophen-induced pneumonitis. To our knowledge, there has been only one previously reported case of acetaminophen-induced pneumonitis.

Repeat hepatic resection for recurrent hepatocellular carcinoma.

During the last 12 years, hepatic resection was carried out on 268 patients with a diagnosis of hepatocellular carcinoma. Of these, 78% developed tumor recurrence in the remaining liver within 5 years. The indications for repeat hepatectomy were evaluated in the same way as for the first operation. Eighteen second hepatic resections and six third hepatic resections were performed 4 to 63 months after the first hepatectomy, with no operative deaths. The cumulative survival rate of these 24 patients was 87.1% at 3 years, 87.1% at 5 years and 72.6% at 7 years after the first hepatic resection, respectively. The present results show that repeat hepatectomy can be safely indicated for recurrent tumors in patients whose liver functional status has been relatively stable since the first hepatic resection.
