[A clinical study on S6472, sustained release preparation of cefaclor, dermatological area].

We conducted a clinical investigation on the use of S6472, an sustained release preparation of cefaclor, in the dermatological area. Thirty-two patients serving as subjects were divided into group I (14 cases, including folliculitis and acne pustulora), group II (16 cases, including furuncle, furunculosis and carbuncle) and others (2 cases). One or 2 wrappers (375 or 750 mg) of S6472 were orally administered to patients twice daily after meals. In group I, the efficacy rate was 100%. Clinical results were as follows: Excellent (6 cases), good (8 cases), fair (0), and poor (0). In group II, the efficacy rate was 93.8%: Excellent (10 cases), good (5 cases), fair (1 case), and poor (0). A decrease of the bacteriological effect was observed in 1 case with Staphylococcus epidermidis. A superinfection was noted in 1 case Staphylococcus aureus. The causative bacteria were all eradicated in the remaining 24 cases. No side effects were detected. From the above results, S6472 appears to be a useful preparation, twice daily oral administration has excellent effects against dermatological infections.

Lewy body-like hyaline inclusions in sporadic motor neuron disease are ubiquitinated.

Round eosinophilic hyaline inclusion bodies with halos in the somata of anterior horn cells from a case of sporadic lower motor neuron disease (MND) were intensely immunostained with the monoclonal anti-ubiquitin antibody (DF2). A few similar, DF2-positive inclusions were also observed in the nerve cell processes of anterior horn cell or in the neuropil. Most inclusions showed intense homogeneous staining of the entire inclusion, whereas a few had intense staining of their periphery with no or pale staining of the central areas. Other DF2-positive structures in the somata of anterior horn cells included cytoplasmic granular structures, eosinophilic thread-like or reticular structures, and small eosinophilic profiles different from Bunina bodies. The DF2-staining intensity of Bunina bodies and spheroids did not exceed the background level. These results suggest that ubiquitination is associated with a pathological process of anterior horn cell degeneration in this MND case.

Intrahepatic biliary cysts after hepatic portoenterostomy in four children with biliary atresia.

We report our experience with 4 cases of cystic dilatation of intrahepatic bile ducts following hepatic portoenterostomy for biliary atresia. Two of the cases did not achieve satisfactory bile excretion and all four cases developed recurrent cholangitis after hepatic portoenterostomy. The attacks of cholangitis seemed to be associated with the presence of intrahepatic cysts. Although one case resulted in death from hepatic failure, three other cases are now outpatients. Patients who develop recurrent cholangitis following hepatic portoenterostomy, should be examined to exclude the presence of intrahepatic biliary cysts. Ultrasonography, computed tomography and percutaneous transhepatic cholangiography were all effective in detecting cysts and provided valuable information for planning treatment. Percutaneous transhepatic or surgical drainage of the bile ducts was effective in reducing jaundice, and recurrent cholangitis.

Heterogenous expression of endothelial cell markers in infantile hemangioendothelioma. Immunohistochemical study of two solitary cases and one multiple one.

Three cases of infantile hemangioendothelioma were immunohistochemically studied with the use of antibodies against von Willebrand factor (vWF), Ulex europaeus I lectin (UEA I), vimentin, thrombomodulin (TM), and actin, as endothelial cell (EC) markers. Because of a broad variety of histologic features, the growth pattern of the tumor cells was subclassified into the following four subtypes: capillary, sinusoidal, cavernous, and myxomatous parts. The solitary tumor from patients 1 and 2 was composed of these four components, but the multiple tumor in the patient 3 consisted of capillary and sinusoidal parts. Immunohistochemically, vWF and vimentin were dominantly expressed in the ECs of the cavernous and myxomatous parts. UEA I had strongly positive results in all histologic types, except the myxomatous part. Expression of vWF and vimentin in neoplastic EC suggests that functional differentiation of the tumor tissue occurs in the myxomatous and cavernous parts and may be related to the spontaneous regression of the infantile hemangioendothelioma.

Malignant histiocytosis involving pancreas at initial presentation.

A 10-year-old boy with malignant histiocytosis presented with fever, hepatosplenomegaly, and diffuse pancreatic enlargement, mimicking acute pancreatitis. Although malignant histiocytosis involving pancreas at initial presentation is exceptional, this entity should be included in the differential diagnosis of acute pancreatitis, especially when hepatosplenomegaly and pancytopenia are present.

Juvenile laryngeal papillomatosis with pulmonary parenchymal spread. Case report and review of the literature.

A 6.5-year-old boy developed laryngeal papillomas at 20 months of age and pulmonary parenchymal spread at age 3.5 years, the lung lesions beginning as solid nodules which rapidly cavitated. The cavitary lesions have stabilized in size but no appreciable improvement has resulted from trials of bleomycin, methotrexate, or interferon. A review of 14 other cases indicates that the lung lesions may develop many years after the onset of laryngeal papillomatosis, and the lung lesions do not tend to regress spontaneously as uncomplicated juvenile laryngeal papillomas often do. Three of 15 patients developed carcinomas in their lung lesions without prior therapeutic irradiation.
