Role for the S-layer of Campylobacter rectus ATCC33238 in complement mediated killing and phagocytic killing by leukocytes from guinea pig and human peripheral blood.

OBJECTIVE: The role of the S-layer of Campylobacter rectus ATCC 33238 in complement-mediated killing and interaction with leukocytes of the intraperitoneal cavity from guinea pigs and human peripheral blood was studied in vitro. MATERIALS AND METHODS: Rabbit polyclonal anti-serum to whole C. rectus cells, a monoclonal antibody which recognizes 150 KDa S-layer protein antigen and a monoclonal antibody against lipopolysaccharide (LPS) were prepared. Sensitivities of C. rectus cells against complement mediated killing and phagocytic killing by peritoneal leukocytes of guinea pig and human peripheral leukocytes were examined in the presence or absence of a specific antibody. RESULTS: C. rectus ATCC 33238 cells were moderately sensitive to complement mediated killing in the presence of rabbit polyclonal antibody against whole cells, and slightly sensitive in the presence of monoclonal antibody against S-layer. Ingestion and phagocytic killing of C. rectus cells by leukocytes were enhanced by the rabbit anti-serum and monoclonal antibody against S-layer, but not by the monoclonal antibody against LPS, pre-immune rabbit serum or control ascites. Viability of leukocytes was dependent on the number of ingested C. rectus cells. CONCLUSIONS: The present study demonstrates that S-layer possessing C. rectus cells are resistant to complement mediated killing and phagocytic killing by leukocytes in the absence of specific antibody.

Responses of peri-implant tissues to undisturbed plaque formation in dogs: clinical, radiographic, and microbiological findings.

The present investigation evaluated the clinical and microbiological effects of long-term undisturbed plaque formation on peri-implant tissues. Four mongrel dogs were used in the study. The mandibular 3rd and 4th premolars were extracted. Three months later, four Brånemark implants (BR, n = 8) or Integral implants (IN, n = 8) were placed in the edentulous area of each dog. After another 3 months, an abutment connection was performed. Plaque control regiment was maintained for 30 days until the superstructure was placed (Day 0: baseline). Then the plaque control program was terminated to allow gross plaque accumulation. Clinical, radiographical and microbiological data were recorded at the baseline, and at 90 and 180 days after the termination of the plaque control. Values for the clinical indices such as probing depth, plaque index and bleeding on probing increased at day 90 on both the implant and tooth sites, and remained unchanged at day 180. Significantly more implant sites showed bleeding on probing than did tooth sites. Neither implants nor tooth sites showed clinically significant changes in mobility. No distinct bone resorption was observed over time by radiography. In microscopic morphological observations, cocci dominated at the baseline, and decreased by day 180, while the proportion of motile bacteria increased. In a culture study, black-pigmented anaerobic rods, which consisted predominantly of Porphyromonas gingivalis-like bacteria, were consistently found from the baseline to day 180. No Actinobacillus actinomycetemcomitans-like organisms were isolated. These results indicate that, although the long-term plaque accumulation did not cause marked periodontal destruction, the peri-implant tissue may be more susceptible to plaque accumulation than the periodontal tissue, and that teeth may serve as a reservoir for the bacterial colonization of the implant sulcus.

Urinary basic fetoprotein in the diagnosis and follow-up of patients with urothelial carcinoma.

BACKGROUND: Urothelial carcinoma is one of the most common malignant neoplasms of the urinary tract and is characterized by a high local recurrence rate. However, no specific and reliable tumor marker has been identified for the diagnosis and follow-up of patients with urothelial carcinoma. METHODS: Urinary basic fetoprotein (U-BFP) levels were measured by enzyme immunoassay in 119 patients with urothelial carcinoma, 64 patients with other urologic malignancies, 144 patients with benign diseases, and 166 healthy individuals to evaluate it clinical efficacy as a marker of urothelial carcinoma. The histologic logic localization of BFP was also studied. A cutoff value was set at 14 ng/mL based on the maximum diagnostic efficiency. RESULTS: The specificity and sensitivity of U-BEP for urothelial carcinoma was 91.7% and 58%, respectively. THe U-BFP positive rate increased significantly with the histologic grade. The U-BFP positive rate was higher in both Tis and T2-4 than in Ta, 1 disease. The U-BFP level exceeded the cutoff value before curative therapy in 32 patients; U-BFP was normalized in 97% of these patients within 2 weeks after therapy. There was a total of 11 recurrences and the U-BFP level returned to abnormally high levels in all instances. Histologic localization of U-BFP was demonstrated in the cytoplasm of the tumor cells in 80% of the patients. CONCLUSION: Based on these results, U-BFP is a potential tumor maker of urothelial carcinoma. In particular, the test could be used to monitor patients whose U-BFP value is positive before therapy.

Microwave ablation of the adrenal gland: experimental study and clinical application.

OBJECTIVE: To evaluate the heating effect of microwave irradiation on the dog adrenal gland and to assess the clinical feasibility of microwave ablation of adrenal tumours. MATERIALS AND METHODS: The thermal distribution and histology were evaluated after dog adrenal glands were irradiated at different intensities with microwaves. Irradiation at 70 W for 30 s was also performed after laparotomy, on the adrenal tumour of a patient with primary aldosteronism. RESULTS: Microwave irradiation at 70 W for 30 s provided a sufficiently high temperature to produce coagulation within a 5 mm radius of the microwave electrode. The adrenal tumor in the patient with primary aldosteronism was coagulated successfully by this dose of microwave irradiation. CONCLUSION: Microwave ablation of the adrenal gland may be clinically applicable in the treatment of adrenal tumours.

Laparoscopic microwave thermotherapy on small renal tumors: experimental studies using implanted VX-2 tumors in rabbits.

OBJECTIVE: To investigate the effectiveness of microwave thermotherapy on implanted renal VX-2 tumors. METHODS: Fifteen rabbits with implanted renal VX-2 tumors were divided into three groups according to the type of therapy: nephrectomy, laparoscopic microwave thermotherapy, and no treatment. Both the anticancer effect and safety were compared among these groups. RESULTS: The survival rate in the laparoscopic microwave thermotherapy group was significantly higher than that in the no-treatment group, and was the same as that in the nephrectomy group. The serum creatinine level did not increase after microwave therapy. CONCLUSION: Laparoscopic microwave thermotherapy may be useful for a nephron-salvaging treatment in small implanted renal VX-2 tumors in rabbits.

Ultrasound-guided microwave thermotherapy on a VX-2 carcinoma implanted in rabbit kidney.

A temperature greater than 60 degrees C was maintained for 60 s within a 5-mm radius of a microwave electrode implanted in a rabbit kidney using 2450 MHz at 100 W for 30 s. Histological examination revealed complete coagulation necrosis in that area. VX-2 cells were inoculated into the kidneys of 10 rabbits. One week later, five rabbits received ultrasound-guided microwave thermotherapy, and the remaining five rabbits received no treatment. All rabbits with microwave thermotherapy had a complete response to treatment while all rabbits without therapy died of cancer within 6 weeks of the VX-2 implantation. These results indicate that ultrasound-guided microwave thermotherapy has the potential of being a nephron-salvaging treatment for small renal tumors.

Distribution of Porphyromonas gingivalis and Treponema denticola in human subgingival plaque at different periodontal pocket depths examined by immunohistochemical methods.

Localization of Porphyromonas gingivalis and Treponema denticola in different areas of subgingival plaque from advanced adult periodontitis patients was studied immunohistochemically using sensitive immunogold-silver staining and immunoelectron microscopy. Fourteen periodontally diseased teeth were extracted without damaging the subgingival plaque, fixed, and embedded. The subgingival plaque samples were sectioned according to four different pocket depths (0-2 mm, 2-4 mm, 4-6 mm and > or = 6 mm). Serial thin sections were stained using specific antisera to P. gingivalis or T. denticola and then with secondary antibody labelled with colloidal-gold. Cells of both P. gingivalis and T. denticola were predominantly found in subgingival plaque located at depths of more than 4 mm in periodontal pockets. T. denticola cells were found in the surface layers of subgingival plaque, and P. gingivalis were predominant beneath them. However, in the deeper subgingival plaque, the coexistence of P. gingivalis and T. denticola was observed. The present findings suggest that P. gingivalis and T. denticola play important roles in the pathogenicity of periodontal disease and provide the useful information for elucidating the pattern of colonization of microorganisms in the periodontal pocket.

Experimental study of microwave coagulation of a VX-2 carcinoma implanted in rabbit kidney.

This paper describes the results of an experimental study of the microwave coagulation of VX-2 renal tumors implanted in rabbits. The rabbits undergoing microwave treatment exhibited a satisfactory survival rate and a complete response to treatment, as verified by histological examination. All the rabbits receiving no treatment died within 6 weeks of implantation of the VX-2 carcinoma. These results indicate that microwave coagulation may be a curative method of treatment for a relatively small renal tumor. Intraoperative real-time ultrasonic scanning permits the percutaneous microwave coagulation of renal cancer in a clinical situation.

[Pregnancy obtained by in vitro fertilization (IVF) with epididymal spermatozoa in obstructive azoospermia: report of two cases].

Epididymal spermatozoa aspiration was performed in two cases of obstructive azoospermia. After the procedure, in both cases, the patient's wives obtained twin pregnancies by this method in conjunction with in vitro fertilization (IVF) and embryo transfer (ET). The patient's wife in one case had a normal delivery. The patient's wife in the other case, however, aborted artificially because she had cerebral infarction. Epididymal spermatozoa aspiration proved efficacious for male sterility in cases of obstructive azoospermia.

Ureteral compliance and histology in partial obstruction in a canine model.

Standardized partial ureter obstruction was achieved in 35 mongrel dogs in whom ureteral compliance and its relationship to histologic changes were studied over a period of 26 weeks. The partially obstructed ureter became dilated with thickened wall and showed muscle hypertrophy and connective tissue proliferation. The normal stellate configuration of the ureteral lumen gradually flattened and became entirely circular by the eighth week. The compliance of the obstructed ureter increased gradually after one, 2, 4, and 8 weeks but decreased thereafter at 12 and 26 weeks. At 8 weeks, the ureter was significantly more compliant than both, the unobstructed control ureter (p less than 0.005) and the 12 week and 26 week partially obstructed ureters (p less than 0.005 & p less than 0.05 respectively). The increase in ureteral compliance was proportional to the amount of muscle hypertrophy during the first 8 weeks while the subsequent decrease in compliance (at 12 and 26 weeks) was proportional to the amount of connective tissue proliferation. This study documents ureteral response, both functional and histologic, to partial obstruction in a canine model over 26 week duration.

Microwave coagulation therapy on VX-2 carcinoma implanted in rabbit urinary bladders.

In order to evaluate the effectiveness of microwave coagulation therapy on urinary bladder carcinoma, we conducted a series of experiments using carcinoma VX-2 cells designed for the application on animal hosts. Three days after implantation of VX-2 cells into the bladder of the rabbits, microwave coagulation therapy was performed. The antitumor effect, i.e. the survival rate, the histological study and the immunological response of the microwave therapy was examined in comparison with the control group (no treatment), the partially cystectomized group and the sham-operated group (no tumor cell implantation). The results were obtained as follows. (1) The survival rate in the microwave group was greater than that in the control group. (2) The stimulation index value (SI), which represents humoral immunity, decreased postoperatively in all groups. In the microwave group, SI increased gradually beginning 21 days after the transient decrease. (3) Histological findings revealed severe degeneration, necrosis and complete eradication of the cancer cells of the bladder wall in the microwave group, however, perforation of the urinary bladder could not be detected. The results indicate that microwave coagulation therapy is an effective procedure for urinary bladder tumors. Furthermore, microwave therapy may also accelerate the inactivation of immunological suppressors in the carcinoma host, an additional benefit of the microwave procedure.

Simultaneous recording of urethral pressure and cross sectional area profile.

A new technique for simultaneously recording urethral closure pressure and cross sectional area profile is described. The cross sectional area of the urethra could be measured using the field gradient principle. The procedure was performed readily in male patients and the recordings can be graphically presented during measurement. A high degree of reproducibility using this method was obtained. Typical recordings of control subjects was well as patients with pathology were obtained using this technique. The results of this study show that the maximum urethral closure pressure obtained was 60.8 + 3.4 mmHg and functional profile length, 4.8 + 0.4 cm. Internal meatus cross sectional area was 0.67 + 0.04 cm2 and the minimum urethral cross sectional area was 0.12 + 0.02 cm2. This method has been shown to be readily applicable in measuring urethral closure pressure in males and can potentially be used as a tool of comparing the functional parameters of pressure with the anatomical value of diameter.

The effect of alpha-2 agonists and antagonists on the upper urinary tract of the rat.

We examined the effect of the selective alpha-2 agonist dexmedetomidine and antagonist atipamizole on the upper urinary tract, renal pelvic pressure and ureteral peristalsis. Experiments were performed on twelve Sprague-Dawley female rats weighing 275-323 grams, with administration of urethane (1.2 micrograms/kg). Ventilatory support was provided through a tracheotomy. A continuous normal saline infusion was maintained through the left iliac vein at a rate of 2.5 ml/hr. Arterial pressure was measured at the left iliac artery, which was cannulated with a PE-100 tube connected to a pressure transducer. A mid-line incision was then made from the xyphoid to the symphysis to expose the left kidney, both ureters, and the bladder. The bladder was intubated and drained to avoid bladder pressure increase. Measurements of urine output rate were made from the right ureter and renal pelvic or ureteral pressure was measured using a nephrostomy placed into the pelvis. A ureterostomy was produced by introducing another catheter, into the upper segment of the left ureter for ureteral pressure measurements. The rats were divided into two groups as follows: 1) dexmedetomidine group (n = 6); injected intravenously with 2 micrograms/kg of dexmedetomidine dissolved in 0.5 ml saline. 2) atipamizole group (n = 6); injected intravenously with 2 micrograms/kg of atipamizole dissolved in 0.5 ml saline. Ureteral peristaltic frequency, baseline pressure, and contraction amplitude were compared before, after, and between the bolus injections of 2 micrograms/kg dexmedetomidine (n = 6) or 2 micrograms/kg atipamizole (n = 6) in 0.5 ml saline. The results showed that dexmedetomidine at 2 micrograms/kg produced a significant decrease in arterial pressure and an increase in urine output from 1.2 + 0.8 to 3.6 + 1.2 ml/min. There was no effect on the baseline pelvic pressure of 6.8 + 1.2 cmH2O or amplitude of the renal pelvic contractions: 3.5 + 0.6 cmH2O. The frequency of pelvic contractions was reduced from 0.37 + 0.03 to 0.27 + 0.02 Hz. Atipamizole at 2 micrograms/kg produced a significant reduction in urine flow rate of 1.1 + 0.8 to 0.6 + 0.2 ml/min. Atipamizole also showed no significant effects on baseline pelvic pressure or frequency, but increased the amplitude of pelvic contractions from control values of 3.0 + 0.9 to 3.4 + 0.9 cmH2O. Dexmedetomidine reduced both the baseline ureteral pressure of 8.5 + 2.4 and peristaltic contraction pressure of 11.5 + 2.3 cmH2O in 4/6 rats. Atipamizole reduced base-line ureteral pressure and increased peristaltic rate.(ABSTRACT TRUNCATED AT 400 WORDS)

[Microwave therapy for diseases of the genitourinary organs].

Microwave energy had become a valuable component in the electromagnetic spectrum. Because of its unique energy source, the microwave radiation can penetrate several centimeters into biologic tissue. The energy is absorbed and converted into heat within the tissue. Clinical applications of microwave irradiation fall into two categories; thermotherapy and hyperthermia. Microwave therapy has generated great interest in the field of urology because of its safety, efficiency, and its simplicity as a surgical tool. This review analyses the fundamentals of the science of microwave energy as a therapy and presents an in-depth discussion of its theoretical principles, unique characteristics, appropriateness of application, and its potentiality as a long-term modality for the treatment of urologically-related diseases.

[Transurethral intracavitary irradiation for carcinoma of the prostate].

This paper describes a new technique and preliminary clinical results of remote after-loading transurethral irradiation for cancer of the prostate. As of January 1986, twelve patients with adenocarcinoma of the prostate have been treated by our radiotherapy technique. Clinically, 3 patients were in stage B2, 3 in stage C, 3 in stage D1 and 3 in stage D2. These patients have been followed up for 13 to 33 months with a median follow-up period of 20.6 months. The dose of transurethral irradiation was 9-10 Gy. to the prostatic capsule and about 2 Gy. to the rectum in one procedure. We repeated this radiotherapy 3 to 4 times within an about 1-month period. Three patients in stage D1 and one patient in stage C received an additional external beam radiation (40 Gy.) to the entire pelvis. A needle biopsy was also performed every 4-6 months after irradiation. Local tumor response proved rapid and satisfactory as verified by a rectal examination and ultrasonography. The biopsies revealed a 70% negative rate within one year. The most common side effect was transient frequency observed in 7 patients. Severe complications such as incontinence, urethral stricture, or proctitis were not evident. This study suggests that intracavitary irradiation of cancer of the prostate is effective and safe. This method may have wider application.

[A case of renal oncocytoma with multilocular cyst].

A 63-year-old woman was admitted to our hospital for a left renal tumor with multiple cystic lesions. Computed tomography and renal angiography revealed a hypovascular tumor 9 cm in diameter and multilocular cyst with a thick wall. Renal cell carcinoma and multilocular cystic nephroma were suspected, and left nephrectomy was performed. Histopathological diagnosis of this tumor was renal oncocytoma.

Intraoperative real-time ultrasonic scanning for microwave coagulation of the prostate.

Nine patients with severe dysuria caused by prostatic cancer received prostatic microwave coagulation under intraoperative real-time ultrasonic scanning. During the procedure, the microwave applicator was clearly observed, and the coagulation region appeared as an hyperechoic area compared to the noncoagulated region. Intraoperative real-time ultrasonic scanning allowed safer and more efficacious application of prostatic microwave coagulation.
