[Clinical study on the effect of continuous transarterial infusion chemotherapy of CDDP for metastatic liver tumors].

Thirteen patients with unresectable liver tumors, including 7 colorectal cancers, 5 gastric cancers, and 1 breast cancer, were treated by intraarterial continuous infusion of CDDP using an implantable reservoir. The catheter was placed by direct insertion at laparotomy (7 cases) or through the inferior epigastric artery (6 cases). The clinical antitumor effects were evaluated by CT-scan or ultrasonography. One CR, 6 PR, and 6 NC were observed among the thirteen patients, respectively. The response rate for all cases was 53.8%, which is evidently higher than that of previously reported "one shot" transarterial MMC and 5-FU infusion chemotherapy. Since this treatment method induces little side effects, it can be maintained in outpatient clinics. Thus, we concluded that this treatment could be useful to improve the quality of life of patients with unresectable liver tumors.

[Diagnostic imaging of polysplenia syndrome in the adult].

Polysplenia syndrome is a congenital disorder of situs that is characterized by the presence of multiple spleens and a variable combination of thoracic and visceral anomalies. We present three adult cases of polysplenia syndrome with emphasis on the diagnostic imaging findings. Computed tomography could best determine the exact location and shape of the anomalous organs. The visceral anomalies observed in our patients included multiple spleens, interruption of the inferior vena cava (IVC), azygos or hemiazygos continuation, left-sided IVC, symmetrical liver, anomalous fissure of the liver, anomalous lobe of the liver, median location of the gallbladder, short pancreas, inverted stomach, gallbladder and pancreas, and intestinal malrotation.

Autopsy report of primary CNS B-cell lymphoma indistinguishable from multiple sclerosis: diagnosis with the immunoglobulin gene rearrangements analysis.

We report a case of primary CNS B-cell lymphoma indistinguishable from multiple sclerosis (MS). MRI of the head showed the spontaneous disappearance of the white matter lesions and the progressive cerebral atrophy. The brain biopsy failed to make a diagnosis of CNS lymphoma but rather suggested MS. Although the primary CNS lymphoma was suspected at autopsy, the immunohistochemical study showed the CNS-infiltrating lymphoid cells comprising both T-cells and B-cells. Analysis of the immunoglobulin and T-cell receptor gene rearrangements first provided evidence of primary CNS B-cell lymphoma.

Cystic pineocytoma--case report.

Pineocytoma and pineoblastoma, originating from pineal parenchyma, are rare and usually solid. An unusual case of totally cystic pineocytoma in a 37-year-old female is reported. The tumor showed neuronal differentiation and had a good outcome. Prominent calcification associated with pineocytoma and pineoblastoma is an useful finding to differentiate these from benign pineal cysts.

Study on the change of lung lamellar body to lattice tubular myelin by N-acetylglucosaminidase with special reference to membrane components and calcium.

The possibility of a role of N-acetylglucosaminidase or beta-galactosidase in the morphological change of lung lamellar body to lattice tubular myelin was examined in an in vitro incubation study. Electron microscopic observation of the incubation product and gas-chromatographic analysis of released sugars during incubation were used to understand the mechanism of these enzymes in the change of membrane constitution. N-acetylglucosaminidase proved to play a role in developing membrane of lamellar body toward lattice tubular structure with the support of Ca++. In this process, inositol release was indispensable in the modulation of membrane as well as the release of a small amount of N-acetylglucosamine. However, further reaction of this enzyme caused destruction or fusion of lattice-tubular integration with the release of other membrane components. Beta-galactosidase played no role in this process.

[Effective continuous hemofiltration and plasma exchange for the treatment of subacute type fulminant hepatic failure].

By combination of continuous hemofiltration (CHF) with plasma exchange therapy we successfully treated a patient with subacute type fulminant hepatitis to keep her consciousness alert. The patient was a 55-year-old woman who admitted because of severe jaundice. On the 51st day after the onset she had consciousness disturbance and was transferred to our hospital. We started the therapy of CHF and plasma exchange at the patient's hepatic coma grade 4. On the 5th day her consciousness level recovered to grade 2 and we could keep the level for almost 2 weeks. This combination therapy seemed good not only for the improvement of consciousness of patients in hepatic coma but also to support the hepatic function of almost ahepatic patients.

[Bronchial and non-bronchial systemic artery embolization for hemoptysis due to non-neoplastic lung diseases. Immediate effect and long-term results].

Forty-four patients with massive or repeated hemoptysis due to nonneoplastic lung diseases were treated by embolization of the bronchial and non-bronchial systemic arteries with Ivalon particles or a mixture of Ivalon and gelatin sponge particles during the past five and a half years. Immediate control of hemoptysis was achieved in 43 patients (98%), and no spinal cord ischemia was observed. Among 30 patients followed for more than 12 months, successful control of hemoptysis was obtained in 22 (73%). Hemoptysis recurred in eight patients (27%); four underwent repeated embolotherapy alone, two underwent surgery alone, and two underwent repeated embolization and surgery. Successful control of hemoptysis was achieved after repeated embolization and/or surgery in all eight patients. It is concluded that embolization of bronchial and non-bronchial systemic arteries is an effective initial treatment for hemoptysis, and good long-term results can be obtained in combination with surgery.

[The management of stage I lymphomas of the B-cell type with special reference to the role of radiation therapy].

Fifty-one patients with clinical stage I B-cell lymphomas were treated between 1980 and 1988. For the entire group, the actuarial 5-year survival rate and 5-year freedom from relapse rate were 78% and 64%, respectively. Primary site, tumor bulk and performance status were the prognostic factors. Twenty-three patients were intensively treated with local radiotherapy alone, because (1) they had histologically low grade (7 patients) or follicular large cell (1 patients) tumors, (2) Waldeyer's ring was the site of the primary (11 patients) or (3) the tumor was less than 4 cm in maximum diameter (4 patients). Among them, only one patient died due to lymphoma (5-year survival rate: 95%), and 19 remained disease free (5-year freedom from relapse rate: 80%). In contrast, of the remaining 23 patients treated with both radiation therapy and combination chemotherapy (VEMP or CHOP), the 5-year survival rate and 5-year freedom from relapse rate were 70% and 54%, respectively. These results suggest that high-risk patients should be treated with more aggressive combination chemotherapy as well as radiotherapy. However, low-risk patients with stage I B-cell lymphomas can be treated by local radiotherapy alone.

[Magnetic resonance imaging of idiopathic trigeminal neuralgia].

Seventeen patients with idiopathic trigeminal neuralgia (ITN) and seven subjects of control group were evaluated with magnetic resonance imaging (MRI). Vascular contact at the proximal portion of the preganglionic segment (PGS) of the trigeminal nerve was observed in 100% of ITN on the affected side, 76% of ITN on the non-affected side, and 21% of control group. Deformity of the PGS was observed in 65% of ITN on the affected side, and in none of ITN on the non-affected side or control group. Non-surgical treatments were effective in all of the 6 patients without deformed PGS, but they failed to control trigeminal neuralgia in 4 of 11 patients with deformed PGS. This preliminary study suggests that MRI could be used for the clinical assessment of trigeminal neuralgia before treatment.

Pain-related somatosensory evoked potentials in syringomyelia.

Pain-related somatosensory evoked potentials following CO2 laser stimulation (pain SEPs) and conventional electrically-stimulated SEPs (electric SEPs) were examined in 8 patients with syringomyelia who showed various forms of dissociated sensory loss. Unlike clinical examination using a pin or needle, pain SEP is considered to be an objective and quantitative test to investigate functions of peripheral and central sensory pathways responsible for pain-temperature sensation (A delta fibres and the spinothalamic tract). Pain SEPs were abnormal in all patients. The results were generally compatible with the degree of a clinical impairment of pain-temperature sensation. Subclinical abnormality was detected in 3 patients. Electric median nerve SEPs using the scalp reference (Fz) were normal in 6 out of 8 patients. However, anterior and posterior cervical responses using a noncephalic reference were absent or small in 7 patients. Electric SEPs following tibial nerve stimulation were normal in 7 patients. These findings suggest that the function of the ascending fibres through the dorsal columns is intact in most patients, whereas the dorsal horn, where a fixed cervical potential is generated, is impaired. Pain SEPs combined with electric SEPs therefore appear to be extremely useful for investigating physiological function in the sensory pathways in patients who show 'dissociated sensory loss' such as in syringomyelia.

Dural arteriovenous malformation associated with occlusion of the superior sagittal sinus--case report.

The authors report a rare case of dural arteriovenous malformation (AVM) associated with occlusion of the superior sagittal sinus. A 78-year-old female developed transient aphasia, followed by generalized convulsion. Common carotid angiography showed a dural AVM fed by the bilateral middle meningeal arteries, draining to the superior sagittal sinus, and sinus occlusion. 123I-single photon emission computed tomography demonstrated decreased blood flow in the bifrontal-parasagittal regions. Transcatheter embolization via the feeding arteries improved the cerebral blood flow around the lesion, and the symptoms disappeared.

Case report: computed tomography findings in Williams-Campbell syndrome.

Williams-Campbell syndrome is a rare type of cystic bronchiectasis that is due to defective cartilage of fourth-to sixth-order bronchi. The diagnosis has been made either at autopsy or by bronchographic findings. The article describes an adult case of this syndrome with CT findings that are characteristic of this entity. CT showed areas of emphysematous lung parenchyma distal to the dilated bronchi, suggesting bronchiolitis obliterans secondary to proximal cystic bronchiectasis. These findings are useful in differentiating Williams-Campbell syndrome from other causes of cystic bronchiectasis, thus obviating bronchography.

[Trans-arterial infusion chemotherapy through the inferior epigastric artery for malignant abdominal tumors without laparotomy].

A new technique of intra-arterial catheterization through the inferior epigastric artery using a subcutaneously-implanted silicon reservoir was tried for arterial infusion chemotherapy in patients with cancer located in specific lesions. Eight surgically uncontrollable tumors, consisting of 6 hepatic metastasis, one surgically incurable gastric cancer and one recurrent rectal cancer, were treated by continuous arterial infusion of cis-diamminedichloroplatinum (CDDP) using this new method. Clinical antitumor effect was evaluated by CT scan, sonography or operation. CR, PR and NC were observed in 3, 3 and 2 patients, respectively. No significant difference in CDDP levels in serum was found between CR plus PR and NC cases. Since this method can be done without laparotomy, more patients may benefits from arterial infusion chemotherapy.

[The management of stage I lymphomas of the T-cell type].

Between 1980 and 1988, we treated 68 patients with clinical stage I non-Hodgkin lymphomas. Fifteen patients had lesions classified as T-cell type lymphomas, and the remainder as B-cell type lymphomas. The actuarial 5-year survival rate was 42% for patients with T-cell type and 78% for patients with B-cell (p less than 0.03), and freedom from relapse was 25% and 67%, respectively (p less than 0.01). The main relapse sites for patients with T-cell type were extranodal (5 patients) and multiple sites (2 patients). These results suggest that patients with T-cell type lymphomas should be treated by aggressive combination chemotherapy as well as radiotherapy, even though their disease may appear to be limited to one region.

[Radiology of trigeminal neuralgia--with special reference to CT findings].

CT findings in ninety-nine patients with trigeminal neuralgia were reviewed. Brain tumors including three trigeminal neurinomas, three meningiomas, one epidermoid, one acoustic neurinoma, were found in eight cases as a cause of symptomatic trigeminal neuralgia. Among seventy-nine patients with idiopathic trigeminal neuralgia, four cases had vascular decompression surgery because of the tortuous, ectatic or anomalous vertebrobasilar artery. Among the other seventy-five non-surgical cases, seventeen cases showed the tortuous and/or ectatic vertebrobasilar artery on CT, and this group of patients showed slightly higher recurrence rate of trigeminal neuralgia after nerve block or medication compared with other non-surgical patients. CT is thought to be a useful screening imaging modality for evaluation of patients with trigeminal neuralgia, and angiography is required for precise evaluation of the compressing vessels when surgical treatment is contemplated.

Magnetic resonance imaging of an intraventricular craniopharyngioma.

A case of a craniopharyngioma confined entirely to the third ventricle is reported. MRI revealed an isointense and high signal lesion on T1- and T2-weighted images, respectively, with homogeneous enhancement by Gd-DTPA. The tumor was reduced by 90% in 4 months after 60 Gy radiation therapy. Intraventricular craniopharyngioma is unusual and is not associated with tumoral calcification or cyst formation.
