Clinical features of small contraction wave recorded by an external tocodynamometer.

Clinical features of the small contraction wave recorded by a guardring tocodynamometer were examined retrospectively. This study included 578 patients and 6363 cardiotocographs ranging from 20 to 42 weeks of gestation. The small wave was observed in 7.5% of the cardiotocographs examined, and the rate of small-wave appearance in each gestational week tended to decrease gradually as the pregnancy progressed. This was not observed after 41 weeks of gestation. The small wave-positive group had relatively poor obstetric parameters and fetal outcome, and the small wave was frequently observed in cases of effective beta 2-stimulant intravenous infusion for the treatment of preterm labor. These results suggest that the small wave represents some degree of contractility and is ominous in general; however, the appearance of the wave does not lead to a poor prognosis in preterm labor if large phasic contractions can be abolished by beta 2-stimulant treatment.

Malignant pericardial effusion in endometrial adenocarcinoma.

A case of endometrial adenocarcinoma in a 62-year-old woman with malignant pericardial effusion is presented. The patient underwent a total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic node dissection, and paraaortic node biopsy. Postoperatively, she was placed on a combination chemotherapy regimen of cisplatin, doxorubicin, and cyclophosphamide. The patient developed cardiac tamponade during the course of chemotherapy. Although we first suspected cardiotoxic effect of doxorubicin, cytologic examination revealed adenocarcinoma cells in the pericardial fluid. A review of the literature revealed no other cases of cardiac metastasis from endometrial carcinoma diagnosed during life.

Gliomas with dural invasion: computed tomography and angiography.

This report is a review of the findings of computed tomography and angiography of eight supratentorial gliomas with dural invasion. Computed tomography revealed localized increases in the thickness of ring enhancements at the sites of dural invasion in four cases, which was attributed to increased blood supplies by the meningeal vessels. Angiographically, feeders from the meningeal vessels entered the tumors mostly through relatively broad areas of tumor-dura attachment. A sunburst type of neovascularity from a meningeal artery was seen in only one case. In most such cases the differential diagnosis from meningioma was possible by the computed tomographic and angiographic findings. Selective external carotid angiography was the most important examination for diagnosing dural invasion by gliomas.

Characteristics of action potentials and contractions evoked by electrical-field stimulation of pregnant human myometrium.

Evoked electrical and mechanical activities of pregnant human myometrium were recorded by the single sucrose-gap method. Each muscle strip has an individual rhythm of membrane excitability and the evoked activity was affected by spontaneous membrane fluctuation. However, high-frequency stimulation caused changes in the rhythm of fluctuation and the pattern of action potential converted from plateau to spike type. Twitch contractions evoked by spike potentials were summated and became fused tetanus in accordance with shortening of the pulse interval. This method is suitable for clarifying the excitation-contraction relationship in human myometrium.

Preoperative diagnosis of ovarian malignancies with intravenous digital subtraction angiography.

Intravenous digital subtraction angiography was performed on 59 patients with ovarian tumors. Dilatation of the uterine artery and its ovarian branch was relatively frequent in both benign and malignant tumor groups, while dilation of the ovarian artery, and hypervascularity from the uterine artery or the ovarian artery were fairly specific to the malignant tumor group. These five abnormal vascular patterns were evaluated by a scoring system and approximately 75% of malignant and 87% of benign ovarian tumors were correctly diagnosed preoperatively by digital subtraction angiography alone. Intravenous digital subtraction angiography can be applied as an easy, safe, and valuable diagnostic method for ovarian malignancies and can replace conventional pelvic angiography.

Bronchoscopic and angiographic comparison of bronchial arterial lesions in patients with hemoptysis.

In seven patients with nonmalignant disease, we bronchoscopically observed various types of bronchial arterial lesions that may have caused hemorrhage. Five of the seven showed a bulging lesion, and the other two demonstrated an intrabronchial mass. We also examined these seven patients using selective bronchial arteriography. Herein we report our comparative study of the bronchoscopic findings and the bronchial arteriograms of these bronchial arterial lesions. The bulge observed in bronchoscopy corresponded either to an aneurysm or to a hypervascular area in the bronchial arteriogram. The mass lesions corresponded to a hypervascular area or a focal dilatation in the bronchial arteriogram. The intrabronchial lesions observed bronchoscopically either disappeared or were significantly diminished by bronchial arterial embolization for management of the hemorrhage. A histologic examination in two patients who underwent surgery revealed vascular lesions corresponding to the intrabronchial lesions in bronchoscopy. The results of this comparative study have important application in the bronchoscopic examination of bronchial arterial lesions in patients with hemoptysis.
