Visuo-perceptual impairment and cerebral lesions in spastic diplegia with preterm birth.

Eighteen cases of spastic diplegia (SD) ranging in age from 5 year 4 month to 9 year 5 month with preterm birth were studied to clarify the relationship between visuo-perceptual impairment and their cerebral lesions. All underwent neuropsychological examinations including the Frostig developmental test of visual perception, Tanaka-Binet or Suzuki-Binet intelligence test, and MRI examination. Cerebral lesions were detected in all subjects, and the volume of the peritrigonal white matter of the parietal and occipital lobes was significantly correlated with the visuo-perceptual impairment (r = 0.74, in the axial plane; r = 0.64, in the coronal plane). We propose that visuo-perceptual impairment is caused by cerebral lesions and that the measurement of peritrigonal white matter by MRI is useful for detecting potential visuo-perceptual impairment at an early age.

Experimental Tonsillophilus suis granuloma in rats and guinea pigs.

Rats and guinea pigs were experimentally infected by intradermal and intraperitoneal inoculation of T. suis. These animals were observed for pathologic changes at various stages. Macroscopic observation disclosed visible grayish white soft nodules at the injection site in those inoculated intradermally and many grayish white nodules in the peritoneum of those inoculated intraperitoneally. Histopathological examination revealed the presence of characteristic microbial elements in the centre of the lesions. Various stages of the lesions were recognized; the early stage being abscess, the advanced stage being pus-forming granuloma and the final stage being residual granuloma. The microbial elements were composed of spores, thalli and multilocular tubers. Clubs were formed around microbial elements in rats inoculated intradermally and in guinea pigs inoculated intraperitoneally. The characteristic T. suis lesions in pigs were reproduced by experimental infection of this organisms in rats and guinea pigs, and thus the pathogenicity of T. suis was confirmed.

Studies on the electrical stimulation-induced contractile responses of hamster and mouse gallbladders.

Isolated gallbladders of both hamsters and mice showed a contractile response consisting of initial and secondary contractile responses to electrical stimulation with rectangular pulses (50 volt, 40 Hz) of 6 msec duration for a period of 10 seconds. The duration time of the contractile response was very long. This was because the duration time of the process of the initial contractile response was as short as 5 to 40 seconds, but that of the secondary contractile response was as long as 8 to 15 minutes. Both initial and secondary contractile responses of the gallbladder in both hamsters and mice were little affected by atropine (1 x 10(-6)M, guanethidine (1 x 10(-6) M) and tetrodotoxin (1 x 10(-7) M). In both animals, further, the initial contractile response of the gallbladder was little affected by indomethacin (1 x 10(-5) M) and 5, 8, 11, 14-eicosatetraynoic acid (ETYA) (5 x 10(-6) g/ml), while the amplitude of the secondary contractile response of the gallbladder was significantly reduced by such drugs. From these results, we obtain the following conclusions: 1) In both animals, the intramural nerves of the gallbladder may have no influence on the both initial and secondary contractile responses of the gallbladder. 2) The initial contractile response is probably mediated by a direct effect of the electrical stimulation to the muscle in the wall of the gallbladder, while the secondary contractile response is probably attributable to prostaglandins released from the wall of the gallbladder by a direct effect of the electrical stimulation to the wall.

[Non-verbal auditory agnosia with EEG abnormalities and epilepsy; an unusual case of Landau-Kleffner syndrome].

A case of unusual Landau-Kleffner syndrome was reported. She was an 8 year-old girl and showed non-verbal agnosia, diffuse EEG abnormalities and convulsions. Her responses to both verbal and non-verbal sounds remained inconsistent and unstable. When a continuous spike-wave complexes on EEG was detected, she paid no attention to any sound in spite of her fair consciousness. Auditory brainstem response and magnetic resonance imaging of her brain were normal. Auditory agnosia was correlated well with EEG abnormalities, and valproic acid and clonazepam were effective for EEG improvement. After the EEG improvement, clinical responses to sounds recovered well; firstly she could pay attention to sounds and then she could distinguish between verbal and non-verbal sounds. Finally, she could speak a few words after the learning letters.

[Interhemispheric choroidal epithelial cyst associated with partial agenesis of the corpus callosum: case report and review of the literature].

A case of interhemispheric choroidal epithelial cyst is reported. The patient is a 9-month-old female who was transadmitted to our hospital for further examination because of the enlargement of her head. She had no neurological deficits nor symptoms of increased intracranial pressure. CT scanning performed on admission showed multiple cystic lesions in the right frontoparietal interhemispheric space, whose circumference was partially enhanced with contrast medium. Metrizamide CT cisternography demonstrated no communication between the lesions and the ventricular system. The signal intensity of the cysts was higher than that of cerebrospinal fluid on both T1-weighted and T2-weighted MR images. Sagittal T1-weighted images showed partial agenesis of the corpus callosum. The surgical exploration was performed via interhemispheric approach. The cyst wall was found to be white, relatively rich in vascular components, and was removed as much as possible. The examination of the cyst fluid showed total protein levels of 1250 to 3440 mg/dl, and sugar contents of 43 to 99mg/dl. Callosal agenesis was confirmed at operation. The light microscopic examination revealed that the cyst wall was composed of a single layer of columnar or cuboidal epithelium with occasional papillary configuration and thick collagenous connective tissue. The epithelial cells contained PAS-positive granules in the cytoplasm. Electron microscopy showed numerous club-shaped microvilli with no coating materials, continuous basement membrane, tight junction, interdigitation, and multiple fenestrations of endothelium of stromal vessels. On the basis of these findings, the lesion was diagnosed as choroidal epithelial cyst. In the literature, interhemispheric choroidal epithelial cyst associated with partial callosal agenesis, confirmed ultrastructurally, has not, to out knowledge, been reported.

[A case of cervical myelopathy due to atlanto-axial subluxation with athetoid cerebral palsy].

We reported cervical myelopathy due to atlanto-axial subluxation of athetoid quadriplegia of cerebral palsy. This lesion was present from upper cervix to lower medulla oblongata, and appeared on MRI. The changes of sSEP was useful to evaluate his spinal function. We think that pediatricians need to understand this pathophysiology, particularly for multihandicapped children.

Myoclonic astatic epilepsy presenting eyelid myoclonic seizures induced by closing of eyes.

A male infant with myoclonic astatic epilepsy showed myoclonic seizures of the eyelids by closing of the eyes. There was no photosensitivity in EEG with flash-stimulation and he could induce seizures even in total darkness. The latency between a lid closure and onset of seizure discharges on EEG was relatively long (0.3-0.5 second). Epileptic seizures in this patient may be induced via a brainstem reticular formation, and elevated Bell's position seems to be the necessary stimulus to produce seizures.

Studies on the intramural nerves of the hamster and mouse gallbladders.

We first carried out microscopic observation of the intramural nerves of hamster and mouse gallbladders which were fixed and stained with a solution of OsO4 and ZnI2. For this microscopic observation, the guinea-pig gallbladder was used as a control. We then investigated the relationship between the motility and intramural nerves of the isolated gallbladder in both the hamster and mouse. The following results were obtained: 1. The ganglionated plexus and the perivascular nerves as the intramural nerves of the guinea-pig gallbladder were observed in the subserosal layer. These perivascular nerves were not possessed of ganglia. On the other hand, the nerve plexus and the perivascular nerves as the intramural nerves of the gallbladders of hamster and mouse were observed in the subserosal layer. These nerves, that is, the nerve plexus and the perivascular nerves, were not possessed of ganglia. That is to say, no ganglionated plexus or ganglia were observed in the subserosal layer of the walls of hamster and mouse gallbladders. 2. The isolated hamster and mouse gallbladders evoked spontaneous motility. This spontaneous motility was little affected by either atropine (1 x 10(-6) M) or tetrodotoxin (1 x 10(-7) M). 3. The hamster and mouse gallbladders showed a contractile response to electrical stimulation with rectangular pulses (50 volt, 40 Hz) of durations of 0.5, 1, 2, 3, 4, 5 or 6 msec for a period of 10 sec. These contractile responses were little affected by either atropine (1 x 10(-6) M) or tetrodotoxin (1 x 10(-7) M). 4. The long duration of the contractile response was mediated by electrical stimulation with rectangular pulses (50 volt, 40 Hz) of durations of 4, 5 or 6 msec for a period of 10 sec. That is to say, its duration time was approximately from 8 to 15 minutes for both hamster and mouse. These results suggest that the hamster and mouse gallbladders may not contain the nerves that take part in the movement of the gallbladder. That is to say, the contractile response of the hamster and mouse gallbladders seems to be myogenic rather than neurogenic in its control. It is, however, possible that some substance other than the neurotransmitter in the walls of these gallbladders may also play a part in the control of the contractile response, although further study will be necessary to verify this suggestion.

MR imaging of spastic diplegia. Comparative study between preterm and term infants.

Eighteen children (eleven preterm birth and seven term birth) with clinical evidence of spastic diplegia (SD) were studied to clarify the differences of the lesions between preterm and term SD on MR imaging. All with preterm birth showed similar abnormalities of the periventricular white matter represented by high intensity in T2-weighted imaging and low intensity in T1 imaging. It seemed that the lesions were periventricular leukomalacia (PVL) and dysmyelination pathologically and correlated well clinically with spastic diplegia. SD with term birth group showed various lesions, two brain anomalies (schizencephaly and corpocephaly), one PVL, four showed no lesions. We suggested that SD with preterm birth is not only a clinical but also a pathological entity.

Changes in the mixed venous blood oxygen saturation during the acute phase of myocardial infarction and administration of dobutamine.

Clinical significance of the continuous monitoring of mixed venous oxygen saturation (SvO2) by Catheter Oxymetry System was evaluated in 45 patients with acute myocardial infarction (AMI). Patients with AMI were classified into 4 groups according to Forrester's hemodynamic classification. The mean SvO2 value for each group was 70.4 +/- 4.1% for subset I, 54.7 +/- 6.9% for subset II, 55.8 +/- 9.8 for subset III, and 47.0 +/- 8.0% for subset IV. Subsets II, III, and IV showed significantly low values compared to subset I (p less than 0.01). The relationship between PCW and SvO2 level was expressed by the negative regression line y = -0.8x + 73.8 (r = -0.64, p less than 0.01). The curve representing the relationship between CI and SvO2 level was expressed by the equation y = 53.8 logx + 42.7 (r = 0.68, p less than 0.01). The SvO2 level of the 14 patients who showed a significantly low value of 56.9 +/- 8.2% before administration of 5 micrograms/kg/min dobutamin, increased to a mean value of 66.8 +/- 7.9%, as well as the CI after 60 min. However, PCW demonstrated no pronounced changes because of dobutamine administration. Since the SvO2 level highly correlated to the cardiac pumping function in acute phase of myocardial infarction, polygraphic monitoring of SvO2 was found to be a useful index for evaluating changes in the hemodynamics. It was also a very useful measure in evaluating the effects of dobutamin.

[Disturbances of visual perception and the lesions on spastic diplegia].

In a study of 12 cerebral palsy with spastic diplegia, the relationship between their disturbances of visual perception and their lesions was discussed. All of them were performed Frostig Developmental Test of Visual perception and Tanaka-Binet Intelligence Test. Six of them showed disturbances of visual perception on Frostig Test. Subtest II, figure-ground, was the lowest of all subtests. The mean of perceptual quotient was under that of intelligence quotient significantly (p less than 0.05 t-test). MRI examination was performed to all and the lesions were detected at 10 of them. Six who showed disturbances of visual perception had the lesions and two who showed good visual perception had no lesions. The lesions were periventricular white matter and extended to semioval center and subcortical white matter at parietal and occipital lobes. Anatomically, their lesions correspond to superior longitudinal bundle and vertical occipital bundle and their bundles have important functions as the pass way of visual perception. Therefore, it was suggested that the disturbances of visual perception detected at spastic diplegia were based on the lesions and the mechanism was disconnection of higher visual pass way.

[The relationship between severity of spastic diplegia and laterality of the cerebral function].

Seventeen children with clinical evidences of preterm birth and spastic diplegia (SD) were studied about the relationship between clinical severity and laterality of cerebral function. The result showed the patients with the left-handed SD were more serious than those with the right-handed in their intelligence and activity of daily living. As the severity increased, motor difficulty was also observed at upper limbs, and especially spasticity was detected at the right hand. The dichotic listening test showed the right ear dominance only in the patients with the right-handed SD. MRI examination was performed in 10 cases, and periventricular leukomalacia (PVL) was detected in all cases. There was few correlation between PVL and clinical laterality. We speculated that right-hand spasticity which was seen in severe cases was due to the fragility of periventricular structure in the left hemisphere.

[Prognosis of three-year-old children with developmental speech delay: reading and writing abilities at eight years of age].

Of 1761 three-year-old children born in 1979-80 and lived at Yonago city, Japan, 28 (1.6%; 25 boys and girls) were diagnosed as developmental speech delay by two pediatric neurologists. Reading and writing skills were evaluated in twelve of the speech delayed children at 8 years by ITPA, Token test and their compositions. Their results showed that reading or writing skill was delayed in many of them, especially in those with behavioral disorders, such as hyperkinetic behavior and poor personal relationship at 3 years.

[Evaluation of increasing digital blood flow during early period of air-cooled cold test].

The present study was performed to elucidate peripheral hemodynamic changes, especially, digital blood flow, caused by an air-cooled cold test. Experiments were carried out by placing the subject's left hand in a box that was kept at a temperature of about 18 degrees C by air-cooling. At the same time, the digital blood flow, digital blood pressure, compliances of the peripheral resistance and capacitance vessels were measured. These parameters were measured on the left forefinger of the cooled side, and also on the opposite side according to Kato's method at 3 points, 1) at normal condition (before cooling stated). 2) 30 seconds after the cooling began and 3) 10 minutes after the cooling began. The following results were obtained; 1) The systemic blood pressure, digital blood pressure and heart rate showed no statistically significant differences in measurements taken at the above three stages. 2) The mean value of the digital blood flow was found to have increased after 30 seconds, and to have decreased after 10 minutes of cooling. Statistically, significant differences were noted at the above three stages. 3) The mean value of the peripheral vascular resistance was found to have increased after 30 seconds, and to have decreased after 10 minutes. 4) Compliances of the peripheral resistance vessel and capacitance vessel showed no significant changes on either side except between normal condition and after 10 minutes of cooling.

[A case of type C Niemann-Pick disease].

We diagnosed a 3-year-old boy as type C Niemann-Pick disease by studies on cultured fibroblasts; sphingomyelinase activity was almost normal, but esterification of exogenously administered cholesterol was deficient. Also sparse cultures of his fibroblasts developed a relatively intense fluorescence with filipin that was not observed either in normal or type B Niemann-Pick fibroblasts. We tried to treat him with dimethylsulfoxide (DMSO), 100 mg-120 mg/day for a year, but it had clinically insufficient effect on our case.

Type C Niemann-Pick disease: clinical and biochemical studies on 6 cases.

Clinical and biochemical studies were performed on 6 cases of type C Niemann-Pick disease. Neurological symptoms started in early infancy in 3 cases, and in childhood in the other 3 cases. However, no clear discriminations were possible with regard to neurological and general somatic manifestations between these two groups. All patients showed normal or slightly low sphingomyelinase and beta-glucosidase activities in fibroblasts, and a defect of esterification of exogenous cholesterol. The extent of these abnormalities was not correlated with the clinical course or severity of this disease.
