[A case of erythropoietin-producing renal cell carcinoma proved by immunohistochemistry].

A case of erythropoietin-producing renal cell carcinoma is reported. A 59-year-old woman was referred to our hospital for further examination of erythrocytosis accompanied with an elevated serum erythropoietin level. Ultrasonography and computerized tomography scan showed a left renal tumor. She underwent a left radical nephrectomy. Postoperatively, serum erythropoietin level and erythrocytosis were normalized. Histopathology was renal cell carcinoma, clear cell subtype, and immunohistochemistry revealed erythropoietin in cancer cells. Furthermore, the cancerous tissue contained a high concentration of erythropoietin.

Spontaneous rupture of a bladder diverticulum: ultrasonographic diagnosis.

We report a rare case of spontaneous rupture of a large bladder diverticulum that was diagnosed accurately and promptly using abdominal ultrasonography. Instillation of saline and air into the bladder on ultrasonography demonstrated jetting from the wall of the bladder diverticulum. We recommend this useful imaging technique, which can be performed easily in the emergency room, in all suspected cases of bladder rupture.

[Preventive chemotherapy after transurethral prostatectomy: a randomized study].

The usefulness of preventive chemotherapy was studied in transurethrally prostatectomized patients with no prior urinary tract infections. They were divided into two groups (A and B) randomized by the envelope method. A was administered intravenously piperacillin (PIPC) 2 g once after surgery, and twice on the following day. B was similarly administered PIPC intravenously, followed by oral administration of tosufloxacin tosilate (TFLX) 300 mg twice daily from the second to the fifth day. Of the total of 135 cases, 104, 45 in A and 59 in B, conformed to the patient standards established for analysis. Efficacy was assessed by the primary physician and also according to a unified standard in which the onset of a post-surgical infection and a bacteriuria of 10(4) or greater CFU/ml observed ten days after surgery were regarded as ineffective. An intergroup statistically significant difference was not observed in the efficacy rate assessed by the primary physician. However, according to the unified standard assessment, B showed a significantly higher efficacy rate (88.1%) than A (62.2%). The administration of TFLX following PIPC was useful. With an indwelling catheter, B exhibited a significantly greater efficacy rate when the retention period was four days or longer. The efficacy was greater in both groups if lavage was not performed, and this effect was greater in A. Consequently, the administration of TFLX was considered more useful for long-term indwelling catheter cases. Neither serious side effects, nor clinical test abnormalities were observed in this study.

Emphysematous cystitis: report of a case.

Emphysematous cystitis is characterized by gas collection within the bladder wall and lumen. A case of emphysematous cystitis in a 74-year-old diabetic male is reported. Radiogram demonstrated an enlarged bladder with intraluminal gas.

[Two cases of congenital urethral stenosis complicated with vesicoureteral reflux].

Secondary vesicoureteral reflux (VUR) is not uncommon, but is rarely accompanied by congenital urethral stenosis in children. We report 2 cases. Case 1 was in a 3-year-old boy who had congenital anterior urethral stenosis accompanied by VUR. Urethroplasty by Johanson's operation was performed and 6 years later VUR disappeared. Case 2 was in a 7-year-old girl, who had congenital peripheral urethral stenosis accompanied by VUR. The urethra was dilated by a balloon-dilator and about 1 year later VUR disappeared.

[Cavernous hemangioma of the penis: report of a case immunohistochemically studied].

An 18-month-old boy was admitted in April, 1988 because of a penile mass which had been first noted at the age of one month. From nine months after birth, transient bleeding from the mass accompanied by pain had been occasionally noted. At the age of 14 months, the mass seemed to grow markedly. On physical examination, there was an irregularly shaped mass on the right side of the prepuce. It was palpated subcutaneously, soft in consistency and was 10 mm in diameter. It had a light-purple color. Upon surgery, the mass was found not to have originated from the cavernous body of the penis, so that it could be excised without difficulty. Histopathological diagnosis was cavernous hemangioma. Electron microscopic examination revealed findings suggestive of newly formed blood vessels such as abnormal cuboidal shape of endothelial cells and a high nucleocytoplasmic ratio. Immunohistochemical examination of factor VIII-related antigen was in homogeneously positive in vascular endothelial cells. To our knowledge, this is the twenty-eighth case reported in Japan.

[Pneumocystis carinii pneumonia in renal transplant recipients treated with cyclosporin].

Four cases of pneumocystis carinii pneumonia occurred among 38 renal transplant recipients. Diagnosis was confirmed by cyst concentration technique in 2 cases. The other 2 cases were clinically, from the rapid improvement of fever, pulmonary infiltrates and hypoxia following a therapeutic trial of high dose sulfamethoxazole-trimethoprim. All patients responded to treatment with high dose sulfamethoxazole-trimethoprim. Three patients survived pneumocystis carinii pneumonia, but i died due to aspergillosis. One patients showed serological evidence of concomitant cytomegalovirus infection.

Retroperitoneal seminoma with simultaneous occurrence in the prostate.

We report a case of retroperitoneal seminoma with simultaneous occurrence in the prostate. The testes were normal on physical and ultrasound examination. In this case a multicentric occurrence of seminoma was considered rather than metastasis from the retroperitoneum to the prostate. Systemic chemotherapy with vincristine, peplomycin and cisplatin was effective for the prostatic as well as the retroperitoneal lesion.

Vaginal and phallic urethra with prominent clitoris in female pseudohermaphroditism.

Idiopathic female pseudohermaphroditism associated with a vaginal urethra, prominent clitoris and an accessory phallic urethra is a rare syndrome, with no apparent cause to account for the masculinization of the genitalia and exhibit of similar characteristic features. We report such a case and review the pertinent literature.

[Two cases of nonfunctioning adrenocortical adenoma].

Two cases of benign nonfunctioning adrenocortical adenoma found incidentally by computerized tomography (CT)scan are presented. One patient had some abnormal values in subsequent endocrine studies, but did not have any recognizable clinical syndromes. The adenomas were removed surgically.

[Clinical effects of goshajinkigan for male infertility].

No study has been reported of the effects of Goshajinkigan on male infertility. Thirty infertile male patients were orally given Goshajinkigan at a daily dose of 5.0 g for three months or more. Sixteen of these patients showed significant improvement in sperm motility. Ten cases showed effective increase in sperm count. Only one patients became pregnant during the period of administration of this drug. Laboratory examination showed no significant change in serum LH, serum FSH or prostaglandin E in seminal fluid. These results suggest that administration of TSUMURA-Goshajinkigan is effective as therapy for male infertility, but how this drug may be involved in promoting fertility should be investigated in future.

[Clinical efficacy of S-6436 in urinary tract infection (author's transl)].

S-6436 was orally given to the patients mainly with uncomplicated acute cystitis at two divided doses for 7 days with the following results: 1) One or 2g/day of S-6436 were administered to the patients with 93.7% of effectiveness. There is almost no difference in effectiveness between 1 g and 2g dosages. 2) Bacteria in urine disappeared in 74% of the patients treated and superinfection occurred in 22%. The disappearance of the bacteria was observed in 83.3% of the patients with E. coli and 60% with Staph. epidermidis. The superinfection however was observed in 16.5% with E. coli and 33.3% with Staph. epidermidis. The disappearance of bacteria was observed in each one case with Klebsiella, Staph. aureus and Ps. aeruginosa. The superinfection was observed in 2 cases with Proteus. 3) Sensitivity tests on the organisms indicated that 94.4% of E. coli, 40% of Staph. epidermidis, Klebsiella and Staph. aureus were susceptible to the drug but Proteus and Ps. aeruginosa were not susceptible. 4) No remarkable side effects were observed, though digestive was used in several patients. From the above results, S-6436 is considered to be a useful drug which is expected to have as much effectiveness as regular cephalexin at two divided doses after breakfast and dinner.
