Degradation of tri-n-butyltin in Ise Bay sediment.

The vertical distribution patterns of the transformation products of tri-n-butyltin (TBT) in sediment cores collected from 6 sites in Ise Bay, Japan indicated that TBT was transformed by two pathways: methylation and debutylation. Anaerobic incubation of sediment spiked with TBT showed that the methylation was mainly supported by sulfate reducing activity and debutylation was mainly supported by nitrate reducing activity. These results showed that contribution of the two transformation reactions of TBT varied according to the dominant microbial activities in the sediment. The two types of vertical distribution of butyltins observed in the sediment cores appear to be the result of different dominant microbial activity in the sediment.

[Hyperfunctioning follicular carcinoma of the thyroid. A case report].

A 76-year-old female patient with a large neck lump of 15-year duration, was accompanied by palpitation and tremor. The lump was 9 x 5cm in size and the lower half of it was not palpable because the lower pole was located in the mediastinum. Physical examination revealed two enlarged lymph nodes in the right supraclavicular area. Chest X-ray film showed a coin lesion in the right lung that seemed to be a metastasis. The patient was apparently thyrotoxic with elevated serum concentrations of 15.7 micrograms/dl thyroxin and 359 ng/dl triiodothyronine. A neck scintigraphy using 123Iodine showed a thyroid hot nodule in accordance with this lesion. A total thyroidectomy with a modified neck dissection was carried out. This was followed by 131Iodine therapy. Specimen of the primary lesion weighed 147 g. Pathology of this tumor was follicular carcinoma of the thyroid. The patient was doing well 14 months after surgery. In the literature, three cases of similar hyperfunctioning thyroid carcinoma have been reported, all of which had a large primary lesion. The histological features proved to be of follicular or papillofollicular type. The treatment in these cases was administration of an antithyroid drug followed by surgical removal.

Periodic paralysis with cardiac arrhythmia.

In 1963, Klein et al. first described two girls with normokalemic periodic paralysis and cardiac arrhythmia. We have observed a 15-year-old girl with cardiac arrhythmia and normokalemia but with some features of hyperkalemic, periodic paralysis. The patient showed a waddling gait and Gower's sign. She had atrophy of the proximal muscles and deep tendon reflexes were reduced. Her ECG showed bigeminy with multifocal premature ventricular contractions. At the start of an episode of muscle weakness, the serum potassium concentration rose from 3.1 to 4.4 mEq/l. Muscle weakness was not provoked by intravenous administration of 45 g glucose or of 72 g glucose followed by 10 units of regular insulin. Muscle weakness was evoked by the ingestion of 4 g potassium chloride. During the provoked muscle weakness, the ECG showed normal sinus rhythm temporarily.

[Two-dimensional echocardiography of coronary artery in Kawasaki disease (MCLS): detection, changes in acute phase, and follow-up observation of the aneurysm (author's transl)].

Two-dimensional echocardiography (2-D echo) was performed before coronary angiography in 213 patients (pts), 4 months to 12 years old, with a history of Kawasaki disease (MCLS). In 10 pts, 2-D echo was performed almost everyday during acute phase. In 23 pts, coronary artery aneurysms were followed up for more than 6 months by 2-D echo. In 32 of 213 pts, the presence of coronary artery aneurysms (9 pts with left, 4 pts with right and 19 pts with both coronary arteries) were proved by coronary angiography. The correct diagnosis was achieved prospectively by 2-D echo in 26 of 32 pts with coronary artery aneurysm (in 24 of 28 pts with left and 15 of 23 pts with right coronary artery aneurysm). In all 10 pts with acute phase of MCLS, an increased echocardiographic density around the coronary artery and of the coronary artery itself continued from the 4th or 6th day to the 10th day or near the second month of the illness. The findings were considered due to acute perivasculitis and vasculitis in the coronary artery. In 5 of 10 pts, the dilatation of coronary artery was demonstrated on about 6th day of the illness and in 3 pts was transient, but in other 2 pts the dilated coronary arteries grew up aneurysms on the 9th and 13th day of the illness. In 23 pts with coronary artery aneurysm followed up by 2-D echo for more than 6 months, 18 pts were started to observe within 6 months and 5 pts beyond 1 year after the onset of illness. In the former, the size of aneurysm was markedly reduced in 7, slightly reduced in 6, and did not change in 5 pts. On the other hand, it did not change in all the 5 pts of the latter. This study suggests that 2-D echo is very useful to diagnose noninvasively coronary artery aneurysm in pts with the history of MCLS and to detect and follow-up it in acute phase.
