[Embolization of carotid-cavernous fistulae. Technique and results of surgery].

Seventy two patients with carotid cavernous fistula (CCFs) were managed at our hospital during fifteen year period. Transarterial treatment of CCFs using the detachable balloon technique was performed in all patients resulting in clinical and angiographic cure in 68 patients. Fatal outcome in two patients. Surgical ligation of the internal carotid artery was needed also in two patients.

[Post-traumatic cerebral artery thrombosis].

In the years 1965--1978 thrombosis of the intracranial vessels of different aetiology was diagnosed in 204 patients treated at the Department of Neurosurgery, Medical Academy in Warsaw. The authors selected from this group 8 cases described in greater detail in which there was an evident temporal relationship between trauma and the development of the disease. The severity of the sustained trauma was without a significant influence on the development of post-traumatic thrombosis. Clinical Manifestations increased in intensity usually within the first 48 hours and in only one case they appeared 2 weeks after trauma. In the clinical status of the patients of considerable interest was usually absence of disturbances of consciousness with presence of serious signs of focal central nervous system damage. Of decisive diagnostic significance was cerebral angiography which localized the site of thrombosis and excluded presence of post-traumatic intracerebral haematoma. In 7 cases thrombosis affected the main trunk or branches of the middle cerebral artery. The prognosis as to survival of such patients is generally good but in most cases disability of different degree developed following thrombosis.

[Endovascular balloon treatment of a giant aneurysm of the basilar artery].

A case of giant saccular aneurysm of basilar artery successfully treated by endovascular balloon occlusion is presented. The authors stress the effectiveness of this method in the treatment of an aneurysm which was considered inaccessible for direct surgery due to its localization. Endovascular procedure afforded protection against bleeding as well as regression of neurological deficits.

[Teratoma of the posterior cranial fossa].

Two cases of posterior cranial fossa teratomas are reported. The rarity of these lesions is stressed. In the material of surgically treated cases of brain tumours from a period of 12 years in only two patients teratomas of this location were found. In adults they are extremely rare. In both cases teratoma finding during the operation was a surprise for the neurosurgeons and this was confirmed by literature reports.

[Stereotaxic ventroposteromedial thalamotomy in the treatment. Clinical observations and physiopathological considerations].

In 6 patients the authors performed stereotaxic ventroposteromedial thalamotomy for abolition or reduction of pain. The operation produced lesions in the centrum medianum and parasfascicular nucleus. In 2 cases unilateral operation was performed contralaterally to the side of pain, in the remaining cases the operation was done bilaterally as a one-step procedure. Postoperative follow demonstrated that in 1 case improvement was completely absent, in 4 initial good effect was transient and pain returned after several weeks and in only 1 patient very good effect persists for over 12 months. In the light of these experiences and a survey of the pertinent literature the authors reach the conclusion that limited stereotatic lesion of the so called non-specific pain-transmitting thalamic nuclei produces permanent abolition or alleviation of pain in few cases only. The pathophysiology of pain is discussed in the light of these data.

[A case of tumor of the pineal body treated with gamma rays of Co-60].

A 21-year-old patients was admitted to hospital with symptoms suggesting a tumour in the area of the epiphysis. Computer tomography confirmed the initial diagnosis. Since signs were present of increased intracranial pressure a Pudenz valve was implanted and in the second stage of treatment radiotherapy with radioactive cobalt was applied. A very good result was obtained with control CT failing to demonstrate presence of the tumour and hydrocephalus.

[Atypical pain syndrome after amputation of the lower limb].

A very unusual case if described of ischialgia coexistent with phantom pains in the amputated extremity. The cause of pain was prolapse of a lumbar intervertebral disc diagnosed only during operation. The case points out the necessity of differentiating between postamputation pains and pains not directly related to amputation. This differentiation may help in successful treatment.

[Remote results of treatment of dorso-medial intervertebral disk displacement].

The paper contains a clinical analysis of 23 cases of dorsomedial intervertebral disc prolapse in the lumbar spine treated surgically with stress laid on late results, which were evaluated in follow up. The results were compared with the rate of increase of clinical manifestations, clinical conditions of the patients, and time from the onset of prolapse to the operation. The methods of operation were not analysed. The results were compared with those reported in the literature and were found unsatisfactory. Particularly unsatisfactory results were found in the group of patients with sudden onset of the disease and with development of serious neurological manifestations in a short time. It is stressed that early surgical intervention in such cases can prevent development of irreversible changes and can improve significantly the results of surgical treatment of dorsomedial intervertebral disc prolapse with full blockade of the cerebrospinal fluid spaces in the lumbar part of the spine.

[Surgical treatment of meningiomas of medial part of sphenoid ala and en-plaque meningiomas of perisellar area].

The authors present some clinical aspects of their own experience in the operative treatment of clinoidal and en-plaque suprasellar meningiomas which account for 5.6% of all intracranial meningiomas in Department of Neurosurgery treated in the years 1987-93. Visual loss was the first symptom of disease. In conclusion, the authors stress the still unacceptably long period between the onset of symptoms and correct diagnosis. Possibilities of total removal are limited by the size of tumour and compression of adjacent structures, and the chance for visual improvement is low for that reasons. Visual improvement was observed in only one case after surgery, however in 5 cases worsening of visual acuity was noted in postoperative period. Mortality rate was 6.2%. Adjuvant radiotherapy was performed in non patient. Long term results in ophthalmological and neurological aspects are discussed.

[Posttraumatic carotid-cavernous fistula: diagnosis and treatment--exemplified by a patient with a fistula following a blunt head injury].

Carotid-cavernous fistula is a rare posttraumatic complication. The authors described the patient in which a blunt cranial injury caused the carotid-cavernous fistula. In diagnosing the fistula Doppler examination proved very helpful. It showed the vein-arterial flow in the ophthalmic artery on the fistula side. The arteriography of the common carotid artery fully confirmed the diagnosis. Successful treatment was achieved by means of percutaneous endovascular balloonization of the fistula ring.
