Thyroid amyloidosis with recurrent subacute thyroiditis-like syndrome.

We studied 2 men with a subacute thyroiditis-like syndrome (STLS) associated with systemic amyloidosis. Both had very tender, diffuse, firm goiters, low thyroidal radioactive iodine uptake values, and increased erythrocyte sedimentation rates. Glucocorticoid therapy resulted in dramatic improvement. Compared to 18 patients with subacute thyroiditis, these 2 men had 1) persistence of goiter even in remission, 2) repeated exacerbation of STLS, 3) pain always localized in the same site, and 4) gastrointestinal, renal, and cardiac abnormalities. Histological examination of the patients' thyroid glands revealed amyloid deposition and no evidence of subacute thyroiditis. In addition, 1 man had low T3 thyrotoxicosis with an elevated rT3/T3 ratio, suggesting impaired peripheral conversion of T4 to T3, and immunological and histological evidence of Hashimoto's thyroiditis. These findings suggest that thyroid amyloidosis may be associated with STLS. When patients with clinical features of subacute thyroiditis have an unusual course, the possibility of thyroid amyloidosis should be considered.

Reevaluation of the thyroidal radioactive iodine uptake test, with special reference to reversible primary hypothyroidism with elevated thyroid radioiodine uptake.

The clinical significance of the thyroidal radioactive iodine uptake (RAIU) test was reevaluated in patients with various thyroid disorders. Compared with 262 normal subjects or 194 patients with euthyroid diffuse goiter with normal serum TSH levels, RAIU values were significantly higher in 100 patients with latent primary hypothyroidism (serum TSH, 5-40 mU/L). In 126 patients with overt primary hypothyroidism (serum TSH, greater than 40 mU/L), RAIU values were either extremely high (49 patients with reversible hypothyroidism and 10 patients with postpartum hypothyroidism) or low (67 patients with irreversible hypothyroidism). The increase in RAIU values in latent, or reversible overt hypothyroidism was TSH dependent, and there was a good correlation between RAIU values and serum TSH levels (r = 0.6203; P less than 0.001). In overt primary hypothyroidism, spontaneous recovery of thyroid function during iodide restriction alone occurred in 52 of 53 patients with RAIU values above 35%, in only 7 of 23 patients with RAIU values between 10-35%, and in none of 50 patients with RAIU below 10%. Thus, recovery was predicted by high RAIU values (P less than 0.001; prediction rate, 91.4%). Goiter was found in about 80% of the patients with reversible hypothyroidism, compared with only 34% of the patients with irreversible hypothyroidism. Recovery of thyroid function during iodide restriction also occurred in 71% of the patients with latent hypothyroidism. However, RAIU measurements did not predict the prognosis of patients with latent hypothyroidism. We conclude that iodine-induced reversible hypothyroidism is common in our patient population, and RAIU measurements may be helpful in determining the prognosis of patients with overt primary hypothyroidism.

Sarcoidosis with multiple calcification.

Sarcoidosis, having a twenty-nine years of clinical course, associated with multiple calcified deposits in neck, mediastinum, liver, spleen, kidney, abdominal lymphnodes and lung was reported. Calcified opacity, which was firstly detected during initial two years on plain chest and abdominal x-ray films, increased in calcified mass insidiously. Though, the examination on this admission failed to reveal pathogenesis of multiple calcification, immunological derangement seemed to initiate and promote the calcification.

Immunopharmacological actions of the new antiallergic drug butyl 3'-(1H-tetrazol-5-yl)oxanilate. 2nd communication: inhibitory effects on histamine release from rat mast cells and lung fragments.

The effects of butyl 3'-(1H-tetrazol-5-yl)oxanilate (WP-833), a new antiallergic drug, on its ability to inhibit histamine release from both peritoneal mast cells and lung fragments of rats were investigated. WP-833 inhibited in a dose-dependent fashion immunoglobulin E (IgE)-mediated histamine release from mast cells. Such potent inhibition was observed in glucose-free as well as complete Tyrode's solution but neither in Ca2+-free nor D2O-supplemented Tyrode's solution. In addition, WP-833 significantly increased intracellular cyclic adenosine monophosphate (AMP) levels in purified mast cells. On the other hand, WP-833 inhibited compound 48/80-but not calcium ionophore A23187-induced histamine release from normal mast cells. Also both IgE- and IgG-mediated histamine release from lung fragments were inhibited by WP-833. WP-833 showed non-competitive inhibition of cyclic AMP-dependent phosphodiesterase derived from lung preparations.

Immunopharmacological actions of the new antiallergic drug butyl 3'-(1H-tetrazol-5-yl)oxanilate. 3rd communication: inhibitory effects on histamine release from lung fragments, Schultz-Dale reaction in isolated tracheal muscle and experimental asthma in guinea pigs.

The inhibitory effects of butyl 3'-(1H-tetrazol-5-yl) oxanilate (WP-833), a new antiallergic drug, on histamine release from lung fragments, Schultz-Dale (SD) reaction in isolated tracheal muscle and experimental asthma were investigated in guinea pigs passively sensitized with homologous immunoglobulin E (IgE) serum. WP-833 dose-dependently inhibited antigen-induced histamine release from lung fragments. In addition, synergism as to inhibition of histamine release was observed with a combination of WP-833 and isoprenaline but not with that of WP-833 and theophylline. WP-833 also inhibited antigen-mediated SD reaction of isolated tracheal muscle without showing antagonistic actions upon histamine- and leukotriene D4-induced contraction of normal tracheal muscle. Experimental asthma was clearly inhibited in a dose-dependent fashion by oral administration of WP-833.

Immunopharmacological actions of the new antiallergic drug butyl 3'-(1H-tetrazol-5-yl)oxanilate. 4th communication: effects on histamine release from human leukocytes and from human and monkey lung fragments.

The effects of butyl 3'-(1H-tetrazol-5-yl)oxanilate (WP-833), a new antiallergic drug, on histamine release from human leukocytes and from human and monkey lung fragments were investigated. WP-833 showed no inhibition of antigen, anti-immunoglobulin E (IgE)- and calcium ionophore A23187-induced histamine release from leukocytes of atopic patients allergic to house dust mite. In addition, no influences of WP-833 on the levels of cyclic adenosine monophosphate and guanosine monophosphate in leukocytes from healthy subjects were observed. However, antigen- or anti-IgE-mediated histamine release from human and monkey lung fragments passively sensitized in vitro with human serum IgE sensitive to mite antigen was clearly inhibited by WP-833. Interestingly, prolonged pretreatment of lung fragments with WP-833 resulted in a decay in inhibition of anaphylactic histamine release. Spontaneous histamine release from human leukocytes and monkey lung fragments were not inhibited by WP-833.

Immunopharmacological actions of the new antiallergic drug butyl 3'-(1H-tetrazol-5-yl)oxanilate. 1st communication: effects on type I to type IV allergic reactions in animal models.

The effects of butyl 3'-(1H-tetrazol-5-yl)oxanilate (WP-833), a new antiallergic drug, on type I to type IV allergic reactions were investigated by employing various animal models. WP-833 (i.v. and p.o.) dose-dependently inhibited homologous or heterologous passive cutaneous anaphylaxis (PCA) mediated by rat or mouse immunoglobulin E (IgE) in rats. Homologous PCA caused by guinea pig IgE was also inhibited by WP-833. In addition, WP-833 had inhibitory actions upon homologous PCA induced by rat or guinea pig IgG. However, WP-833 showed no inhibition of rat skin reactions caused by histamine, serotonin and bradykinin, contrasting with the inhibition of prostaglandin E1-induced skin reaction. Furthermore, both adrenalectomy and propranolol treatment exerted no influences on the inhibition of IgE-mediated homologous PCA in rats by WP-833. In contrast to above findings demonstrating that WP-833 clearly inhibited type I allergic reaction, systemic Forssman shock in guinea pigs and reversed cutaneous anaphylaxis in rats (type II), passive Arthus reaction in rats (type III), and contact dermatitis and tuberculin reaction in mice (type IV) were unaffected by WP-833 even in higher doses than in those capable of completely inhibiting type I allergic reaction.

Synthesis and pharmacological activity of stereoisomers of 1,4-dihydro-2,6-dimethyl-4-(3-nitrophenyl)-3,5-pyridine-dicarboxylic acid methyl 1-(phenylmethyl)-3-piperidinyl ester.

1,4-Dihydro-2,6-dimethyl-4-(3-nitrophenyl)-3,5-pyridinedicarboxylic++ + acid methyl 1-(phenylmethyl)-3-piperidinyl ester 1, a highly potent calcium antagonist, was separated into stereo- and optical isomers to investigate the differences of antihypertensive activities between them. Fractional crystallization of the hydrochlorides of 1 gave alpha- and beta-diastereoisomers. The alpha-isomer (benidipine hydrochloride, KW-3049) showed very strong hypotensive effect, but little activity was observed in the beta-isomer. From optically resolved 1,4-dihydro-5-methoxycarbonyl-2,6-dimethyl-4-(3-nitrophenyl)-3- pyridinecarboxylic acids 2, and 1-benzyl-3-piperidinols 3, four optical isomers of 1 were synthesized, and their absolute configurations were deduced. The hypotensive activity of (+)-alpha, namely (S)-(S)-1, was 30 to 100 times stronger than that of (-)-alpha in intravenously administered spontaneously hypertensive rats.

Synthesis of expected metabolites of benidipine hydrochloride.

(+/-)-(R*)-2,6-Dimethyl-4-(m-nitrophenyl)-1,4-dihydropyridine-3, 5-dicarboxylic acid (R*)-1-benzyl-3-piperidinyl ester, methyl ester hydrochloride (benidipine hydrochloride, KW-3049, 1), a highly potent and long-acting calcium antagonistic 1,4-dihydropyridine derivative, is now under clinical study as an antihypertensive and as an antianginal agent. In order to confirm the structures of the metabolites of KW-3049, 19 compounds were prepared. Among them 11 compounds were found to be metabolites (the compounds 2, 3, 6, 8, 14, 15, 16, 28, 31, 32 and 34) of KW-3049 in rats and/or dogs.

Immunopharmacological actions of the new antiallergic drug butyl 3'-(1H-tetrazol-5-yl)oxanilate. 5th communication: effects on immunological reactions in mice and humans.

The effects of butyl 3'-(1H-tetrazol-5-yl)oxanilate (WP-833), a new antiallergic drug, on immunological reactions in mice and humans were studied in view of possible side effects. Hemagglutinin formation in mice immunized with sheep red blood cells was not suppressed by WP-833 administered i.p. or p.o. for 3 days either before or after immunization. In addition, the same treatments with WP-833 showed no suppression of hapten-specific immunoglobulin E (IgE) or immunoglobulin G (IgG) antibody formation in mice immunized with dinitrophenyl-conjugated ovalbumin mixed with alum. The proliferation of human peripheral blood lymphocytes induced by concanavalin A, phytohemagglutinin-P, pokeweed mitogen, protein A or Cowan I was not suppressed by WP-833. Nitroblue tetrazolium reduction test, phagocytosis and chemotaxis in human neutrophils were not inhibited by WP-833.

In vitro investigations with selective adsorbents for IgE and IgM.

In vitro studies were carried out with IM-T. This adsorbent consists of polyvinyl alcohol joined with tryptophan side chains and was delivered by ASAHI Medical Co., Ltd., Tokyo/Japan. It was found that IM-T binds immunoglobulins G and M and also immune complexes only moderately but IgE was adsorbed in remarkable amounts. A clear dose-dependent was adsorbed in remarkable amounts. A clear dose-dependent manner of the IgE adsorption could be stated. Kinetic studies revealed that the binding was only slow and gradual.
