Bipolar electrocautery in laparoscopic cholecystectomy.

Laparoscopic cholecystectomy was undertaken in 100 patients using bipolar electrocautery for hemostasis. Cholecystectomy was completed laparoscopically in 94 patients. No deaths or common bile duct injuries occurred. In this single surgeon's experience, bipolar electrocautery is a safe, effective, and economical modality in laparoscopic cholecystectomy.

Laparoscopic appendectomy: a simplified technique.

Laparoscopic appendectomy can easily be performed by a surgeon familiar with laparoscopic cholecystectomy. The technique described employs the same spatial relationships which are used in laparoscopic cholecystectomy. By maximizing the similarities between the two operations, surgeon and assistants can rapidly become comfortable with laparoscopic appendectomy. Of the laparoscopic appendectomies attempted 88% were successfully completed. The technique and results are presented and discussed.

Laparoscopic cannulation of an accessory hepatic duct: a case report.

Anomalies of the cystic duct are commonly encountered by the operating surgeon. Accessory hepatic ducts directly entering the gallbladder are among the most difficult of these to identify. Because of magnification, laparoscopy provides oftentimes superior visualization of the biliary tree. Herein, we describe a case in which an accessory hepatic duct was identified, cannulated, and evaluated with cholangiography enabling appropriate operative treatment.

Inhibition and recovery of DNA synthesis in host tissues and sensitive and resistant B16 melanoma after 1-(2-chloroethyl)-3-(trans-4-methylcyclohexyl)-1-nitrosourea, a predictor of therapeutic efficacy.

Single doses of 1-(2-chloroethyl)-3-(trans-4-methylcyclohexyl)-1-nitrosourea caused transient suppression of [3H]thymidine incorporation into DNA in bone marrow and gastrointestinal mucosa and more prolonged inhibition of such incorporation in B16 melanoma. A single dose of 1-(2-chloroethyl) -3- (trans-4-methylcyclohexyl) -1-nitro-sourea, 16 mg/kg, doubled the mean life-span after treatment of C57BL times DBA/2F1 male mice bearing 12-day-old B16 melanomas. Subsequent doses timed to minimize toxicity and maximize antitumor effect, however, produced no further prolongation of survival, and studies with B16 melanoma previously expsed to 1-(2-chloroethyl)-3-(trans-4-methylcyclohexyl)-1-nitrosourea demonstrated that the suppression of [3H]thymidine incorporation into DNA was no longer prolonged beyond that seen with normal host tissues. The loss of clinical efficacy was accompanied by a loss of differential suppression of [3H]thymidine incorporation into DNA between the tumor and host tissues.
