Measuring the impact of advanced practice nursing on achieving cost-quality outcomes: issues and challenges.

This article describes the issues and challenges that are inherent in conducting cost-quality outcomes research related to the advanced practice nurse role. As consumers of this research, nurse administrators need to be aware of these issues in order to make informed, organizationally rational decisions. Following discussion of the issues, recommendations are offered for factors to consider when using results of published research findings.

Cost-Benefit Analysis

Establishing a nurse-managed wellness clinic in a predominantly older African American inner-city high rise: an advanced practice nursing project.

With funding from a U.S. Department of Housing and Urban Development contract awarded to a private university, advanced practice nurse faculty established a nurse-managed wellness clinic in an apartment building populated by predominantly African American older people. Ethnographic methods were used to ensure culturally competent care. The clinic provided nursing, pharmacy, and health sciences faculty and students with community-focused clinical experiences as interdisciplinary team members.

Black or African American

The advanced practice nurse role in implementing and evaluating two nurse-managed wellness clinics: lessons learned about structure, process, and outcomes.

By the year 2000, the baby boom generation will be reaching its sixth decade and 34 million U.S. citizens will be over the age of 65. One strategy for promoting healthy aging is having older persons participate in health promotion programs. Nurse-managed wellness clinics (NMWCs) staffed by advanced practice nurses (APNs) offer an option for health promotion among older individuals. This article describes the APN role in implementing and evaluating two NMWCs for older people living in urban, high-rise apartment buildings subsidized by the U.S. Department of Housing and Urban Development. Lessons learned about structure, process, and outcomes in a community-based wellness center are identified and discussed.

Aged

Nurse executive job loss: trauma or transition.

Since the enactment of TEFRA, the health care industry has initiated cost-cutting measures that have resulted in increased job vulnerability for all levels of health care managers. Research on managers and professionals suggests that there are categories of factors that moderate the stress of job loss such that career growth can occur. Interviews with 10 nurse executives who had experienced involuntary turnover suggest a variety of coping strategies that can help to turn job loss into a successful career transition.

Adaptation, Psychological

Building on the past to challenge the future.

The Council on Graduate Education for Administration in Nursing (CGEAN) was established to further the development and improvement of graduate education for administration in nursing. The Council seeks to identify the nature and direction of education for administration in nursing in various healthcare systems, providing guidelines for programs offering administration. A major goal of CGEAN is facilitating dialogue between nursing service administrators and graduate level educators who are engaged in teaching and research related to administration in nursing. This column, sponsored by members of the Council, will analyze and respond to position statements and trends related to the delivery of health services and graduate education for administrators in nursing.

Curriculum

Role deprivation in baccalaureate nursing students pre and post curriculum revision.

This study documents that the Role Deprivation of senior generic baccalaureate nursing students (n = 194) in a newly revised conceptually based, 8-term, health-oriented curriculum was significantly greater (p less than .05) than the role deprivation of senior baccalaureate nursing students (n = 174) in the same school when the program was based on a 10-term, medical model, illness-oriented curriculum. Role deprivation was measured by the Corwin (1960) Role Conception-Role Deprivation (RC-RD) Scale which was used extensively by Kramer in her studies (1969, 1970, 1974) of nurse's adjustment to the hospital work world. In these studies, Kramer used role deprivation as an indirect measure of reality shock. Unresolved reality shock leads to anger, frustration, and disillusionment that eventually can lead to exit from the profession (Kramer, 1974). Kramer has demonstrated that changes in learning experiences in an educational program can affect new graduates' adaptation to the hospital work world.
