After the discovery that Clomiphene-citrate (Clomid) increased the secretion of gonadotropins, it was first used to induce ovulation and later on it was also used in males in certain cases of infertility. The effects of Clomid on the hypothalamic-pituitary level made it also possible to use it for a dynamic evaluation of the hypothalamic-pituitary gonadal axis in both females and males. The results obtained with Clomid are reviewed. The results are widely divergent because of the fact that the selection of the patients, the dosage, and the length of the therapy are not always appropriate. However, the experience with Clomid appears to show sufficient effectiveness in oligozoospermic men, who have a low or normal FSH-level.
Congential absence of the vas deferens is a frequently occurring cause of infertility. Such a disorder can be accompanied by renal agenesis and therefore pyelography should be performed. That such a disorder in man could have genetic aspects has not been mentioned in the literature, but there are reports that this disorder is hereditary in some animals. A family of nine children is described in which four of the five sons were infertile as a result of congenital aplasia of the vasa deferentia. Since there was no evidence that these men had cystic fibrosis and since they did not have the mosaic form of Klinefelter's syndrome, an unrelated developmental defect is postulated. The genetic aspects are discussed and the conclusion is made that the inheritance pattern most probably is autosomal recessive. Finally, the therapeutic problems are discussed. At this time, surgical treatment of this disorder is not encouraging.

Here are reported results of a study based on the administration during 13 weeks of Pergonal-Profasi, in cases of oligozoospermia. When comparing these data to others obtained previously, conclusion can be drawn that administration of these gonadotropic hormones is mainly indicated to patients showing, on the one hand, oligozoospermia combined, on the other hand, with subnormal FSH-values. Though no investigation was made on this point, it can be expressed that specifically in cases of patients with very low FSH levels a treatment consisting of prolonged administration with higher dosages has to be preferred.
