Kinetics of 201Tl uptake in adenomas and well-differentiated carcinomas of the thyroid. A double isotope investigation with 99Tcm and 201Tl.

A visually increased uptake of 201Tl chloride corresponding to a 'cold' (131I or 99Tcm ) thyroid nodule is mostly seen in well-differentiated carcinomas but also often in follicular adenomas. Since a visually increased uptake of 201Tl can be due to an increased initial uptake and/or a delayed elimination, an extended dynamic investigation was performed in patients with well-differentiated carcinomas or with follicular adenomas. Data were collected in a dynamic simultaneous double isotope ( 99Tcm + 201Tl) study up to 50 min after intravenous administration. Adenomas could be significantly separated from carcinomas by the elimination (p = 0.0001), but not by the initial uptake.

[Immunocoagulopathies as a possible cause of acquired hemorrhagic diathesis].

Spontaneous factor VIII inhibitors may be responsible for severe life-threatening haemorrhages. It is reported on two patients with inhibitors directed against factor VIII: C. In one female patient a rheumatoid arthritis could be made evident as basic disease. Diagnostics, therapy and course are reported and discussed.

[Gated blood pool scanning in the diagnosis of coronary disease. A prospective study to compare cineventriculography and 2-dimensional echocardiography].

38 patients with clinically suspected coronary artery disease were studied by contrast ventriculography, 2-dimensional echocardiography and multiple gated blood pool imaging (MUGA) without stress. The results were compared with each other and with the final diagnosis confirmed by coronary angiography. The left ventricular ejection fraction is evaluated nearly identically and with sufficient accuracy by both non-invasive methods, local motion abnormalities are on the other hand diagnosed in the best way by MUGA imaging in our own cases.

[Quantitative lung perfusion scintigraphy in lung function diagnosis].

A new method is described for quantitative evaluation in lung perfusion scintigraphy, which allows to conclude from the increased perfusion of normal lung-tissue to a virtual total lung perfusion. The relationship between this and the real total lung perfusion leads to the pulmonary perfusion defects in per cent. The findings of a comparison of 40 patients between the perfusion scan method and the lung function tests is presented, which proves the validity of the new method. It is shown that restrictive lung diseases have a good and obstructive have a bad correlation. It is demonstrated furthermore that the perfusion defects of bronchogenic carcinoma can be quantified very well by the method.

Preoperative scintigraphy with correlation to cytology and histopathology in carcinoma of the thyroid.

At preoperative scintigraphy of 83 consecutive histopathologically confirmed thyroid carcinomas a solitary reduced uptake was observed in 70 per cent. The minimum detectable size was 10 mm for differential carcinomas showing this image, but in the 'screening group' of hereditary medullary carcinoma several scintigrams were considered normal despite a tumour size of 10 to 15 mm. In the imaging of the undifferentiated carcinoma a solitary reduced uptake with disappearance of the entire affected lobe was often demonstrated. Multifocal areas in the dot scintigram representing tumours were often missed at fine needle aspiration biopsy. Computer-processed scintigrams were superior in showing multifocal lesions.

Treatment of ovarian cancer in the Southern Swedish Health Care Region during the five-year period 1974-1978.

A total of 555 cases of malignant epithelial ovarian tumour diagnosed in vivo was reported to the Regional Tumour Registry for Southern Sweden, Lund during 1974-1978. Of these 428 (77%) were referred to the Department of Oncology, Lund, which serves the Southern Swedish Health Care Region. Patients not being referred were mostly elderly with advanced tumours. The selection of patients will thus affect the evaluation of the treatment results. The standard surgical procedure was bilateral salpingo-oophorectomy with or without hysterectomy, and infracolic omentectomy. 95% of the patients were treated according to a randomized trial including postoperative chemotherapy, radiotherapy or a combination of both. Preoperative radiotherapy in primarily inoperable patients increased the operability. 34% of all stage IIb, IIc, III, and IV-patients were thus operated or reoperated with only small residual tumour or none at all following surgery. Stage, grade and size of postoperative residual tumour were the most important prognostic factors. The five-year cumulative survival rate for all patients referred to the Department of Oncology was 32%.

[Skeletal scintigraphy in the care of breast cancer . Long-term follow-up over 8 years].

In a retrospective study 1704 whole-body bone scans of 807 female and 7 male patients with breast cancer performed from 1974 to 1981 were reevaluated considering the incidence of bone metastases in staging and follow-up, the response of metastatic uptake to treatment as well as sensitivity and specificity of the diagnostic procedure. The yield of positive bone scans in staging was 23.7% at all and depended on T stage with a comparably high incidence of 8.8% in stage T1 and T2. Subsequently within 6 years follow-up further 26% were developing bone metastases. The long time response of known metastases to treatment was ambient, 65% of the scans found without some essential change. The incidence of bone metastases combined with a calculated sensitivity of 93% and specificity of 91% is justifying the use of bone scans in every stage and every phase of assessing breast cancer.

[Emergency endoscopy in patients with hereditary hemophilia].

Haemorrhages from the upper gastrointestinal tract are a particularly threatening complication for patients with hereditary haemorrhagic diathesis. Though in the individual case under conservative therapy a temporizing attitude is possible, the source of the haemorrhage should be localized. When the peculiarities of this group of patients are taken into consideration the emergency endoscopy does not evoke a new danger of bleeding, on the contrary it is a decisive prerequisite for the individual therapy according to indication.

Soft-tissue sarcomas and exposure to chemical substances: a case-referent study.

In 1977 several patients were seen with soft-tissue sarcomas and previous exposure to phenoxy acids. This clinical observation resulted in a cases-referent (case-control) study being undertaken which showed that exposure to phenoxy acids or chlorophenols, which are chemically related, gave a roughly six-fold increase in the risk for this type of tumour. A further case-referent study of soft-tissue sarcomas has now been performed to confirm these earlier findings and also to obtain further information on the effects of different phenoxy acids. This new investigation gave an increase of the same magnitude in the risk for soft-tissue sarcomas after exposure to phenoxy acids or chlorophenols, but this risk related also to exposure to phenoxy acids free from impurities, such as polychlorinated dibenzodioxins and dibenzofurans.

Some aspects of prophylactic oophorectomy and ovarian carcinoma.

The early diagnosis of ovarian carcinoma is as difficult and rare as ever before. Despite the appearance of sophisticated drugs and combinations of these with surgery and radiotherapy the prognosis still remains the same as during the last 30 years, i.e. overall survival between 20 to 30%. This study concerns the impact of prophylactic oophorectomy in Sweden. 4.6% of ovarian carcinoma patients had received pelvic surgery at some time prior to the diagnosis of ovarian cancer and after the age of 40. With the help of statistical data it is estimated that about one hundred out of the approximately thousand women per year who now develop ovarian cancer in Sweden could be saved if prophylactic oophorectomy were practised in our country. Various aspects of prophylactic oophorectomy and its effects are discussed. It is our opinion that every gynaecologist/surgeon should seriously take into consideration and discuss the possibility of prophylactic oophorectomy in the perimenopausal patient before operation. Not to perform prophylactic oophorectomy during pelvic surgery after the menopause seems to us to be an unreasonable practice.

Rapid high pressure liquid chromatography of patulin in apple juice.

A rapid method has been developed for screening patulin in apple juice. The sample is extracted with ethyl acetate and the extract is cleaned up by extraction with a sodium carbonate solution. Patulin is determined after high pressure liquid chromatography on a Spherisorb 5 micrometer ODS column with an ultraviolet detector set at 276 nm. The detection limit of the method is 5 micrograms/L and recovery is 70-75%.

[Early diagnosis of stomach neoplasms].

The primary prevention of the carcinoma of the stomach promises little success in near future. Therefore the effort is directed to early recognition and healing (secondary prevention). The optimum method for this is the combination of X-ray-examination, gastroscopy and aimed biopsy. But also by early diagnosis and therapy in the invasive carcinoma the chances of survival are to be improved. At present only 5-6% of the patients with gastric carcinoma survive longer than 5 years. The author adopts a definite attitude to the groups with risk of carcinoma. The search for further factors of persons endangered by carcinoma (habit of nutrition, kind of constitution and preparation of food, other exogenous factors) must be continued intensively in order to get selected groups which may be controlled consequently.

High pressure liquid chromatographic determination of ochratoxin A and zearalenone in cereals.

A high pressure liquid chromatographic (HPLC) method has been developed for determining ochratoxin A and zearalenone in cereals. The sample is extracted with phosphoric acid and chloroform. The extract is cleaned by washing on a silica gel column with cyclohexane-ethylene dichloride-ethyl ether. After eluting zearalenone with chloroform, ochratoxin A is eluted with chloroform-formic acid. Zearalenone is extracted into alkaline solution, washed with chloroform, the pH is adjusted, and the zearalenone is extracted back into chloroform. Ochratoxin A is purified by chromatography on aqueous sodium biarbonate-Celite. The mycotoxins are determined by using a liquid chromatograph with 2 columns in series packed with Spherisorb ODS 10 micrometer and 5 micrometers, respectively. Ochratoxin A is detected with a speftrophotofluorometer, coupled in series with an ultra-violet detector for estimation of zearalenone. Detection limits are 1-5 micrograms/kg for ochratoxin A and 2 micrograms/kg for zearalenone.

Continuous treatment of non-Hodgkin's malignant lymphoma with prednimustine (Leo 1031).

The effect of Prednimustine was evaluated in 37 patients with generalised non-Hodgkin's lymphomas. The patients were divided into three groups according to dosage and previous treatment. Totally, in all three groups, 22 complete and 10 partial remissions were observed. During follow-up, five of the complete responders and all partial responders have relapsed. Leucopenia and thrombocytopenia were induced in several patients, but were always moderate and reversible after withdrawal of the drug. In some patients with a history of peptic ulcer or diabetes mellitus, these conditions were aggravated.

En-bloc irradiation of unresectable bronchogenic carcinomas and their regional lymphatics.

Patients with unresectable or inoperable bronchial carcinoma were treated with en-bloc irradiation of the tumour and the mediastinal lymph nodes to a total target absorbed dose of 40 Gy in 20 fractions. The first 52 patients were treated in one series and the last 45 patients in two series (split-course). Radiation adverse effects were only mild. The two regimens gave the same palliative results. The median survival was the same for the 2 groups (8 months). Most patients died in disseminated disease.

Present status of forensic psychiatry in Iceland.

This paper gives a brief survey of forensic psychiatry in Iceland at the present time. Because of few inhabitants and limited financial resources there are no special forensic psychiatric institutions in the country, nor any forensic psychiatrists; this kind of work is mainly carried our by general psychiatrists, but also to a certain extent by general practitioners, Formalities around compulsory admission and detention of mental patients, uncooperative and lacking insight, are briefly described. The psychiatric examination of probably mentally ill offenders is also accounted for. Such investigations are without doubt the main task of forensic psychiatry in Iceland to-day. Finally the main faults of forensic psychiatric services in Iceland and very limited educational facilities in this domain are referred to. Some future plans are also mentioned.
