Simultaneous and continuous measurement of free concentration of valproate in blood and extracellular space of rat cerebral cortex.

Free concentration of valproate (VPA) was measured simultaneously and continuously in blood and in the extracellular space of cerebral cortex of rats by VPA-selective microelectrodes. Constant amounts of VPA were injected into the femoral vein with differing duration of injection. Immediately after drug application, the concentration of free VPA in blood and brain increased to a peak value, the degree of which increased with the speed of injection. Ten to 15 min after VPA injection, a plateau value was reached. This plateau value was equal in the extracellular space of cortex and in blood. The data indicate that VPA can "freely" cross the blood-brain barrier (BBB).

Digital subtraction radiography for monitoring dental demineralization. An in vitro study.

The aim of the present study was to evaluate the assessment of progression of demineralization by digital subtraction radiography. In each of 14 extracted human teeth, 2 approximal enamel demineralization lesions were induced in vitro to simulate dental caries. A modified tunnel preparation with glass ionomer fillings was performed on one lesion of every tooth as a model of caries inhibition. Every week, radiographs were obtained under standardized conditions over a period of 42 days. The radiographs were digitized and calibrated for grayscales. Reference landmarks were chosen and aligned for the different pictures by computer-assisted imaging to adjust the images for projective distortions. The images of the 7th, 14th, 21st, 28th, 35th and 42nd day were subtracted from the baseline radiograph. The mean value of gray values of the subtraction images was calculated and ANOVA tests for repeated measurements and paired t tests were used for statistical analysis. The results of the present study indicate that statistically significant gray level changes due to progression of demineralization could be detected in the radiographic images by subtraction analysis. Differences between glass ionomer-filled and nonfilled lesions failed to reach significance. The introduced method may have the potential to detect and document minute caries progression.

Organizing for effective managed care contracting.

While many forums have debated the fairness and ethical implications of managed care arrangements, it is unlikely that physicians will escape practicing within fixed budgets in the future; the economics of health care simply requires it. Although a backlash has developed against managed care, it is actually more recognition of how pervasive it has become, rather than any threat to its existence. Currently managed care comprises the majority of commercial insurance, is making substantial inroads into Medicaid, and is challenging the reductions in Medicare reimbursement by dropping plans at a time when the Federal government's entire strategy for controlling Medicare costs is based upon managed care through its Medicare+Choice program.

E-prescribing hits the medical world: what physicians need to know to get the right technology.

Electronic prescribing is an emerging technology that allows physicians to write appropriate, legible, and safe prescriptions quickly and easily, using a handheld computer unit linked to their office computer. The early physician adapters using this innovation had little objective information upon which to base their choice, so cost was their main criterion. Now, a variety of vendors and options are available, so the discriminating physician is able to select the electronic prescribing system most complementary to his or her practice's needs and unique style.
