[Results of microscopic image analysis of the thyroid gland fine needle biopsies].

In previous cytomorphometrical studies applying a microscopic image analysis system it was shown that follicular adenomas and highly differentiated follicular carcinomas of the thyroid gland can be distinguished. Basing on these results, the usefulness of the method was examined in selected cases of routine materials. 143 fine needle biopsies were analysed by means of the image analysis system. Moreover, in 52 cases the results could be checked by histological investigations of surgical samples. In comparison with the cytomorphological findings, the results of the cytomorphometrical analysis demonstrate a higher grade of assurance of the diagnostic statements. It was confirmed, that it is possible to diagnose highly differentiated follicular carcinomas of the thyroid gland with the method demonstrated above.

[Radionuclide nephro-urologic diagnosis - its value in clinical diagnosis].

Nuclear-medical nephrological diagnostic methods increasingly obtain significance for the judgment of the function of the nephrological system on account of their non-invasive character and on account of improved technical possibilities which made their way into the nuclear-medical routine diagnostics. As to the technique a graduation is the result, beginning with examination methods which at present allow only a qualitative separated according to the sides judgment of the renal function (isotope nephrography and renoscintigraphy) up to the techniques which render possible quantitative evidence (renal sequence scintigraphy and determinations of the renal clearance). Using different nuclear pharmaceutics which underlie a different kinetics, blood supply as well as glomerular and tubular partial function can be demarcated from each other and simultaneously determined, respectively. The use of modern nuclear pharmaceutics, especially of 123I-hippurate from the Central Institute for Nuclear Research Rossendorf introduced into the diagnostics 1982 brings a further reduction of the as it is very low radiation exposure for the patient. The selection of the methods to be used and the nuclear pharmaceutics should always be performed depending on the clinical questioning. Isotope nephrography and renoscintigraphy are used in the preliminary diagnostics on suspicion of functional and morphologic changes of the kidneys. The renal sequence scintigraphy remains reserved to special questionings. It renders possible to establish according to the sides prerenal, intrarenal or postrenal disturbances of the functions of the kidneys globally as well as regionally and to perform a quantitative side-separated judgment of the function.(ABSTRACT TRUNCATED AT 250 WORDS)

[Kidney radionuclide diagnosis in the evaluation of clinical problem cases in pediatrics and nephrology].

An internistically and nephrologically as well as paediatrically well preexamined and selected group of patients in particular cases needs for the judgment of the renal function and capacity a nuclear-medical diagnostics which passes these preexaminations. Renal sequence scintigraphy including the trend scintigraphy and examinations of the renal clearance with side-separated judgment of the renal clearance with side-separated judgment of the function are at our disposal. The indication of these techniques and the value of evidence are demonstrated with the help of problem cases. As problem cases are demonstrated a child with glomerulonephritis, a female patient with renal failure post partum before and after haemodialysis including the observation of the course a female patient with localized inflammatory disease as well as a patient with kidney tumour and cirrhosis of the kidney. It is shown that nuclear-medical techniques recognize relatively discrete, but also severe pathological changes and are necessary within a complex judgment of the function, the quantitative establishment of the functional and compensatory capacity of the kidneys as well as for the planning of therapy and the control of the course.

[Experimental studies of the distribution of intravenously injected iodine-labeled cytonal (diethylstilbestrol diphosphate)].

After intravenous injection the distribution of iodine-labelled diethylstilbestral-diphosphate (cytonal) was investigated in various organs of the rat. For this purpose the concentration of activity in the organs of the animals were established by measurements of specimens in the bore-hole counter and scintigrams were made by means of a small animal scanner. The result was an unequivocal temporary dependence of the enrichment of the substance in the organs. The greatest enrichment was found 10 minutes after injection in the liver, then followed blood, kidneys, adrenal glands, seminal vesicles, prostate, abdominal wall and the fatty tissue surrounding the prostate. The blood activity relatively decreased. 7 days after injection an unambiguously higher activity was to be proved in the liver, kidney, adrenal gland than in the blood, whereas the enrichment of iodine-labelled diethylstilbestraldiphosphate in prostate, seminal vesicle, fatty and muscular tissue was always below the enrichment of the blood. But in every case the relation of organ activity to blood activity changed in favour of the organ activity, also with regard to the prostate. 10 minutes after injection the concentration of the activity of the seminal vesicle was somewhat higher than that one of the prostate, but in the further course it was below the concentration of the activity of the prostate. During the first two hours the fatty tissue surrounding the prostate showed a clearly and after the third day a slightly lower activity concentration than the prostate. On the basis of this distribution of the activity for the scintigraphy of the prostate above all a temporary interval of about 10 minutes to 3 hours after the injection of labelled cytonal is involved. The results cause us to perform investigations in men concerning the pharmacokinetics and the possibility of using radioactively labelled cytonal.

[Simultaneous determination of effective renal plasma flow and glomerular filtration rate using a double isotope technic--determination of the filtration fraction].

A method for the simultaneous determination of the effective renal plasma flow and of the glomerular filtration rate with the help of 131J-o-iodohippuric acid and 169Yb-DTPA using a double isotopic technique is described. Apart from this the method serves for the establishment of the filtration fraction. 40 patients with different clinical pictures (glomerulonephritis, pyelonephritis, cystic kidneys, stenosis of the renal artery) were examined. The clearance values established by us were in the areas characteristic for the clinical pictures. Pathological conditions could be established which are not yet characterized by an increase of the fixa of the urine in the serum. By the determination of the filtration fraction differential-diagnostically important insights are afforded. The examination is methodically simply to be performed and is of little stress for the patient. Therefore it seems to be suitable for the routine examination.

[Determination of the renal clearance in combination with isotope nephrography].

The possibilities of a determination of the renal clearance including the separation of sides with the help of simple nuclear-medical examination and measuring techniques are described. In 50 patients the determination of the effective flow of renal plasma was carried out with 131J-labelled iodohippuric acid. Methodic variants were compared. The examinations of the patient last one hour. They can be carried out in the outpatient department within the isotopic nephrography. In addition to this only several takings of blood samples are necessary. A catheterization of the urinary bladder as it is necessary for the classical determinations of the clearance is no more to be done. The results show that with the help of the methods described valuable quantitative informations about the function of the kidneys are to be got.

Studies on plasma free-fatty-acid metabolism and triglyceride synthesis of brown adipose tissue in vivo during cold-induced thermogenesis of the newborn rabbit.

Parameters of plasma free fatty acid metabolism (pool size, half time, disappearance rate, turnover time and absolute turnover rate), the influx of plasma free fatty acids into the glycerides of brown adipose tissue and the pathway of triglyceride synthesis in brown adipose tissue (glycerol-1-phosphate versus monoglyceride pathway) were examined after intravenous injection of [1-14C]palmitate in newborn rabbits. In the thermoneutral environment of 35 degrees C the turnover rate of plasma free fatty acids was 10.20 mumol/min per 100 g body weight and its flux into the glycerides of brown adipose tissue 0.367 mumol/min per 100 g body weight. Cold exposure at an ambient temperature of 20 degrees C caused a decrease to 5.84 mumol/min and 0.207 mumol/min per 100 g body weight, respectively. Both under basal conditions at an ambient temperature of 35 degrees C and under cold-induced thermogenesis at an ambient temperature of 20 degrees C triglyceride synthesis in brown adipose tissue ran through the glycerol 1-phosphate pathway.
