Properties of recombinant hepatitis B vaccine.

A large-scale purification method for hepatitis B surface antigen produced in a recombinant yeast (Saccharomyces cerevisiae) was established. The resulting HBsAg was greater than 99% pure and suitable for vaccine use. The yeast-derived HBsAg was structurally and biochemically similar to plasma-derived HBsAg. The anti-HBs antibody producing potency of the yeast-derived vaccine in mice was significantly higher than that of the plasma-derived vaccine. The yeast-derived vaccine induced protective antibody against hepatitis B virus of either adr or ayw subtype in a chimpanzee efficacy study. These observations demonstrate the usefulness of the yeast-derived vaccine as a second-generation hepatitis B vaccine.

[Hyperthyroidism due to a TSH-secreting pituitary tumor].

A 28-year-old female with a 12-year history of goiter is presented. She had both clinical and laboratory evidence of hyperthyroidism, and her serum TSH was persistently and markedly elevated after treatment with antithyroid drugs. A TRH stimulation test resulted in no further rise in serum TSH after cessation of medication. Menses were regular and serum prolactin levels were normal. Serum LH and FSH responses to LHRH stimulation test were normal. No other evidence of pituitary or peripheral endocrine deficiencies existed. She underwent a subtotal thyroidectomy followed by 131I therapy three years later. A pituitary adenoma with sphenoidal and suprasellar extension was completely removed by transphenoidal approach. On light microscopy, it was mostly composed of chromophobic cells with occasional calcification showing sinusoidal pattern. On electron microscopy, most of the cells contained fine granules, which suggested thyrotroph. The immunoperoxidase technique revealed TSH beta in the cytoplasm of some adenoma cells. Three days postoperatively the patient's serum TSH levels returned to normal. TRH stimulation test produced a normal response in serum TSH. The patient was diagnosed hypothyroid by laboratory findings and is currently on thyroid replacement therapy. The patient became pregnant and delivered twice prior to the operation for pituitary adenoma. The previously reported TSH secreting adenomas associated with hyperthyroidism were reviewed.

Capacitation-related changes in antigen distribution on mouse sperm heads and its relation to fertilization rate in vitro.

The anti-mouse sperm monoclonal antibody OBF13 did not react with fresh epididymal sperm. However, when sperm were incubated in a culture medium capable of inducing capacitation, the entire head of the sperm began to react with this antibody. This change of reactivity was not observed when sperm were incubated in a Ca2+-free medium. The change of the reactivity to the antibody was studied in relation to the fertilizing ability of sperm as measured in an in vitro fertilization system; a significant correlation was observed between the appearance of head-stained sperm and fertilization rate.

Leiomyosarcoma of the maxilla.

A case of leiomyosarcoma in the maxilla is presented together with a review of the literature. The most commonly encountered symptom at the early stage of oral leiomyosarcoma is a slowly enlarging, nonulcerated and painless mass. This tumor shows high incidence of local recurrence or metastasis, and poor prognosis. The histological confirmation of leiomyosarcoma seems difficult in the routine examination using biopsy specimen. Special staining methods and immunohistochemical analysis are useful for diagnosis of this tumor. The tumor slightly responded to chemotherapy consisting of adriamycin and cyclophosphamide.

Evaluation of submandibular gland function by sialo-scintigraphy following sialolithectomy.

Submandibular gland function following transoral sialolithectomy was examined by 99mTc-pertechnetate sialo-scintigraphy in 10 cases. An intraindividual comparison between the function of the treated gland and that of the contralateral normal gland was made using a time-activity curve. Although glandular recovery was not affected by the duration of symptoms or the existence of the symptom at mealtimes, it was inversely proportional to the size of the calculus. Furthermore, the prognosis was more favorable in patients when the anatomically normal orifice of the submandibular duct was preserved.

Adolescent

Current treatment of Graves' disease.

In this review we have described the rationale for the appropriate treatment of patients with Graves' disease. Because the etiology of this disorder remains obscure, its management remains controversial. Since antithyroid drugs and radioiodine became readily available in the early 1950s, they have been widely used for the treatment of thyrotoxicosis, and the number of cases treated surgically has markedly decreased. However, almost four decades of experience have disclosed an unexpectedly high incidence of delayed hypothyroidism after radioiodine treatment and a low remission rate after antithyroid therapy. As a result, surgery is again being advocated as the treatment of choice. The three modalities of treatment have different advantages and disadvantages, and selection of treatment is of importance. In principle, we believe that for most patients a subtotal thyroidectomy should be performed after the patient has been rendered euthyroid by antithyroid drugs. We attempt to leave a thyroid remnant of 6 to 8 gm.
