[Spontaneous rupture of the renal pelvis: a case report].

A 63-year-old man admitted to our hospital with severe pain in his right flank on June 4, 1987. After admission, his pain increased progressively and he complained of tenderness from the right flank to the right lower quadrant. Computerized tomography and intravenous pyelography demonstrated spontaneous rupture of the right renal pelvis. A retrograde ureteral catheter was indwelling into the right renal pelvis. Because extravasation had disappeared on the retrograde pyelogram, the Catheter was withdrawn after 12 days. After removal of the ureteral catheter, he did not complain of pain. The 42 patients with spontaneous rupture of the renal pelvis reported in the Japanese literature and our one case were discussed.

[A case of olfactory hallucination with adult enuresis].

A 22-year-old man was referred to us complaining of enuresis. The excretory urogram and cystogram were almost normal. Spina bifida was not present. The results of the urodynamic examination including uroflowmetry, cystometry, urethral pressure profile, electromyogram and sensory threshold of external urethral sphincter were within normal limits. Amitriptyline hydrochloride and flavoxate hydrochloride showed no effects. Because we found a positive spike wave on the electroencephalogram, the patient was referred to a psychopathologist, and was diagnosed as being in the early stage of schizophrenia. He complained of enuresis as his olfactive hallucination of urine. Psychopathological treatment for 1 year and 2 months resulted in disappearance of his complaining of enuresis.

[Manifestation of mictional disturbance in four cases of von Recklinghausen's disease].

Four patients with neurofibromatosis (von Recklinghausen's disease) manifesting mictional disturbance are presented. They were a man aged 41 years (A) and 3 women aged 26 (B), 46 (C) and 32 (D) years, and their chief complaints included urinary retention, dysuria, urinary frequency and urinary retention. None of them had any organic obstructive disorders in the lower urinary tract. Case A who had neurofibroma at the C2 and L1-2 vertebral bodies had inactive bladder and his urinary flow rate was less than 10 ml/sec. In case B, cystometrogram was initially normal, but changed to inactive bladder type one year later and neurofibromas were found in the vertebral bodies of Th7-8 and L2. Case C, who had inactive bladder cystometrically, underwent removal of 1-acoustic neurinoma and neurinomas of cauda equina. Case D had hyperactive bladder and received resection of the neurofibroma of vertebral bodies from C6 to Th4. The finding of cystometrogram and type of urinary miction disorders suggested vertebral neurofibroma at an early stage.

[Clinical effectiveness of short-term administration of norfloxacin in acute urethritis].

Thirty patients with acute urethritis were treated with short-term administration of norfloxacin. The drug was administered randomly in two regimens: Two 400 mg oral doses for a single day or three 200 mg oral doses for three days. For gonorrheal urethritis, the efficacy rates of one and three-day administrations were 75 and 90%, respectively. For non-gonorrheal urethritis, they were 75% each. No adverse effects were observed. The results indicate that short-term administration of norfloxacin is useful in the treatment of acute urethritis, especially for gonorrheal urethritis.

[A case of Werner's syndrome with neurogenic bladder].

The present report is Werner's-syndrome associated with a neurogenic bladder. The patient visited our clinic with the chief complaint of retention of urine and urinary tract infections. The diagnosis of Werner's syndrome was made because the patient had such clinical features as bird-like or "masked" face, short stature, graying of the hair in youth, cataracts in youth, skin changes like scleroderma principally on limbs, early menopause. After trans-urethral-resection of bladder neck and administration of distigmine bromide, her bladder function was improved, she could urinate by herself and she was free from urinary tract infections.

[Cirsoid intrarenal arteriovenous malformation. Report of 2 cases, with special reference to review of 74 cases in Japan].

Two cases of cirsoid intrarenal arteriovenous malformation, in a female aged 67 and in a male aged 18, are reported and 74 cases of the same disease collected from the Japanese literature are reviewed. Several classifications have been proposed for the so-called congenital renal arteriovenous fistula. A truly congenital form of the lesion, however, is considered to be cirsoid type and we collected only this type of arteriovenous malformation. The number of reported cases has been increasing annually. The age distribution ranged from 10 to 84 years old (mean: 38.1 years old); 49 were female and 19 male. Gross hematuria, bladder tamponade and flank pain, which can be called the triad of cirsoid intrarenal arteriovenous malformation accounted for 84.0% of the clinical symptoms, whereas cardiovascular signs and symptoms accounted for only 7.8%. The right kidney was most frequently diseased and the intrarenal lesions showed about the same distribution in the upper pole, in the middle region and in the lower pole. Pathogenetic and pathophysiologic aspects of the renal arteriovenous malformation were discussed. Renal angiography is the most helpful method for obtaining a definite diagnosis, while such findings on the intravenous pyelogram as the so-called cobble stone deformity are thought to be characteristics of the X-ray. For the treatment of cirsoid arteriovenous malformations, conservative therapies including administration of styptics were carried out in 13%, selective transcatheter embolization in 17% and surgical procedures in 70%. The recent number of transcatheter embolization has been increased to establish it as one of the distinct therapeutic methods for this disease. Among the operative methods, partial nephrectomy is now being used more than nephrectomy and 4 cases of ex vivo surgery have been reported recently in Japan.

[Cancer of the bladder occurring in occupational dye users: report of two cases].

Two cases of occupational dye users were found to have bladder tumors. Case 1 was a 32-year-old male, who had worked at the Yuzen process for the last 7 years. Multiple papillary tumors were seen on the whole wall of the bladder endoscopically and total cysto-urethrectomy and ileal conduit were performed. The pathological examination of the bladder tumors revealed non-invasive transitional cell carcinoma grade I--II. His post-operative course has been uneventful without any signs of tumor recurrence for 31 months. Case 2 was a 46-year-old male, who had worked at the throwing stained silk for the last 28 years. He suffered from severe hematuria for six months. A big egg-sized nonpapillary tumor on the posterior wall of the bladder was found and bilateral cutaneous-ureterostomy was performed but severe gross hematuria remained. Total cysto-urethrectomy was performed but the patient died of progressive disease 5 months later. The bladder tumor was a grade III invasive transitional cell carcinoma. The fact that process workers in the dye manufacturing industry have a high risk of bladder cancer has been well known and mass screening examination on them has been systematically performed. Though there is a higher relative risk of bladder cancer in occupational dye users, systematic screening examination has not been done. We emphasize the necessity for establishing systematic mass screening examination of occupational dye users for the early diagnosis of bladder cancer.

[Choreito and Shakuyakukanzoto: their effects on facilitating the voiding of stones in the urinary tract].

We treated 30 patients having 37 urinary calculi in all, with " Choreito " (5 g/day) and " Shakuyakukanzoto " (5 g/day) to study whether these medicines hastened spontaneous calculus excretion. Within 2 months, 91% of the small calculi (5 X 5 mm and smaller), and 33% of the medium-sized (6 X 10 mm and below) were excreted. These results were similar to those obtained with conventional conservative treatment, and suggest that this therapy will be useful.
