[Aortic stenosis with left ventricular systolic dysfunction: a severe disease but with good surgical prognosis].

From our series of 72 patients with severe valvular aortic stenosis, we identified 19 showing features of left ventricular systolic disfunction (ventriculographic ejection fraction less than 55% and/or fractional shortening less than 30% at M-mode echocardiography). In these patients, we found a significant inverse correlation between the fractional shortening and the systolic wall stress (r = 0.79, p less than 0.001). Clinically, 18 of the 19 patients were in NYHA class III (n = 11) or IV (n = 5), and two died before they could be operated upon. The remaining 17 had their aortic valve replaced (coronary artery bypass surgery was simultaneously performed in 2 patients). After a mean follow-up of 18 months, all patients are alive and show substantial symptomatic improvement (15 patients in class I and 2 patients in class II). Cardiothoracic index was reduced (61 +/- 5% preoperatively versus 52 +/- 5% postoperatively), and fractional shortening changed from 21 +/- 5% to 30 +/- 5%. The latter remains under normal limits in two thirds of the patients. Our results lend support to the idea that the systolic left ventricular dysfunction in aortic stenosis is due to the increased afterload, rather than to an intrinsic contractility defect. This explains the great functional improvement after the reduction of the systolic wall stress achieved by surgery.

[Retinoic acid effect on the differentiation of blast cells in suspension].

Cells from three patients showed maturation after incubation with retinoic acid (2 had M-3 AML and 1 had CML-B). Three additional patients showed spontaneous maturation (1 with M-2 and 2 with M-4 AML), and in them cell maturation was also achieved after incubation with retinoic acid and cytosine arabinoside (10 nM). These results confirm different maturation capability of leukaemic cells, as well as the possibility to induce cellular maturation with retinoic acid, especially in patients with acute promyelocytic leukaemia (M-3).

Factors influencing maintenance of sperm motility during in vitro processing.

Addition of serum to culture medium during washing of sperm was shown to significantly improve sperm motility upon subsequent 24-hour incubation, distinct from the beneficial effects of adding serum after washing. Commercial sources of human and bovine albumen tested did not preserve motility as well as serum, but addition to serum showed that the lesser effect was probably due to toxic contaminants. One of the most commonly available disposable plastic syringes was found to be markedly toxic to sperm. The materials found to have sperm toxicity also manifested toxicity to mouse embryos. These findings show that a similar degree of caution is advisable in testing of materials contacting sperm in vitro to that which has become standard practice for culture of human embryos.

Effect of beta-lactams and aminoglycosides on human polymorphonuclear leucocytes.

The effects of four beta-lactams (cefotaxime, cefoxitin, ceftazidime and latamoxef) and three aminoglycosides (amikacin, sisomicin and tobramycin) on human polymorphonuclear (PMN) leucocytes were examined. Chemotaxis was studied by agarose migration and in a Boyden chamber. Agarose migration of PMN leucocytes was not affected significantly by ceftazidime and latamoxef (25-200 mg/l). However cefoxitin (50-200 mg/l) decreased agarose migration to 52-65% of the control values, and cefotaxime (25-200 mg/l) reduced migration to 29-67% of control values. The Boyden chamber method showed no significant inhibition of chemotaxis by any of the antibiotics tested. None of these antibiotics affected phagocytosis or killing of Candida albicans by human PMNs.

Toxoplasmosis in pigs in Córdoba, Spain.

A total of 303 pig sera were examined by the Indirect Fluorescence Antibody Test (IFAT), basic Direct Agglutination Test (AD) and Direct Agglutination Test with 2-mercaptoethanol (AD 2-ME). The percentages of positive sera were 32.0 with IFAT, 37.95 with AD and 32.34% with AD 2-ME. No relationship was evident between sex and the incidence of toxoplasmosis in pigs. There were no significant statistical differences between the IFAT and the AD 2-ME, hence both tests can be recommended equally for serological surveys of toxoplasmosis.

In vitro effects of ureidopenicillins on human polymorphonuclear leukocytes.

The in vitro effects of mezlocillin, azlocillin and piperacillin on chemotaxis and adhesivity of human leukocytes were comparatively studied. After incubation with all these antibiotics, chemotactic and adhesivity counts were similar to those of the antibiotic-free cells. Scanning electron microscope examination showed enlargement of surface and length measurements after incubation with azlocillin (P less than 0.005 and P less than 0.001) and mezlocillin (P greater than 0.05 and P less than 0.05), while piperacillin produced no alteration. These findings could provide additional information in the study of leukocyte/antibiotics interactions.

[On-top plasty. Indications and technic in the reconstruction of severely injured hands].

UNLABELLED: The possibilities and advantages of the on-top plasty in the reconstruction of severely damaged hands are described. Based upon ten cases, a classification into four groups is made. In each of these groups different reconstructive procedures are applicable. Group 1: Intact thumb with loss of all fingers. PROCEDURE: Lengthening of the third metacarpal by on-top-plasty of the distal half of the second metacarpal. This simultaneously widens the first intermetacarpal space. Group 2: Partial amputation of the thumb with severe damage to several fingers. PROCEDURE: Thumb reconstruction by transposition of two phalanges of an otherwise useless finger. Group 3: Loss of the thumb and of two and a half fingers. PROCEDURE: Thumb reconstruction by transposition of the remaining basal phalanx. Group 4: Loss of the thumb and of three fingers. PROCEDURE: Thumb reconstruction by on top-plasty of the distal half of the metacarpals and rotational angulatory osteotomy of the fifth metacarpal.

Thrombotic obstruction of the Björk-Shiley aortic valve prosthesis. Report of four cases.

We present four cases of late thrombotic obstruction of the Björk-Shiley aortic valve prosthesis. The incidence of this complication reported in the literature to vary from 0.7% to 5%. In our experience of a total of 623 Björk-Shiley aortic valve replacements, we observed this complication in only 4 patients (0.6%). Two of them had poorly controlled anticoagulation therapy. The clinical presentation was subacute in all four patients. Surgical treatment, thrombectomy and debridement, was performed in all of them. The diagnosis was made upon abrupt and progressive onset of dyspnoea, physical examination data and echocardiographic and radioscopic findings. Angiocardiographic studies were needed in two patients.
