[Granulomatous hepatitis].

The diagnosis of granulomatous hepatitis was made on the basis of granulomas observed in the liver biopsies from 64 patients. It is shown that the macrophagal granulomas are, as a rule, localized inside the lobules, do not possess the characteristic features and represent the morphological manifestations of a non-specific reactive hepatitis. At the same time macrophagal granulomas may be a stage in the formation of the epithelioid-cell granulomas. The latter reflect the long persistence of the antigen in the liver macrophages. In certain cases the epithelioid-cell granulomas acquire specific features and these allow one, when analysing the liver biopsies, to confirm or to suggest the etiology of the disease (sarcoidosis, tuberculosis). An essential help in establishing the etiology of granulomatous hepatitis comes from the repeated clinical examination of patients and repeated liver punctures.

Chronic Disease↗

[Functional activity of natural killer cells in patients with chronic liver diseases].

Functional activity of natural killers (NK) was examined in 67 patients with chronic virus and alcoholic liver diseases, chronic nonspecific reactive and drug hepatitides. The activity of NK was found to be reduced in the groups of patients with chronic virus nonspecific reactive and drug hepatitides. Suppression of NK was most pronounced in patients with virus chronic active hepatitis. No statistically significant alterations in NK were discovered in the groups of patients with liver cirrhoses depending on the etiological factor and activity of the pathological process. The use of immunosuppressive therapy for active liver diseases led to further inhibition of natural cytotoxicity.

[Morphological diagnostic criteria of nonspecific reactive hepatitis].

The material of 100 punch liver biopsies from patients with a secondary nonspecific reactive hepatitis as a complication of chronic disease of the alimentary canal (gall bladder, pancreas, stomach and intestine) was studied. The criteria of morphological diagnostics of this hepatitis were elaborated. It is established that the morphology of the nonspecific reactive hepatitis depends on the pathogenesis and clinical course of the underlying disease. The response of liver macrophagal system is a basis of inflammatory changes.

[Clinico-morphological characteristics of liver diseases associated with nonconjugated hyperbilirubinemia syndrome].

Altogether 98 puncture liver biopsies taken from patients with a syndrome of nonconjugated hyperbilirubinemia are examined in light and electron microscope. It is established that this syndrome frequently develops in inflammatory liver diseases, notably in primary (viral, alcoholic, medicinal) and secondary (nonspecific reactive) hepatitis. Both light and electron-microscopic investigation of liver puncture biopsies allows determining the type of a pathological process in the liver, to specify its etiology and to study the structural basis of bilirubin metabolic disorders in the liver. The mechanism of these metabolic disorders is complex and includes alteration of the bilirubin seizure by a sinusoidal hepatocyte pole, its conjugation and formation of ultimate metabolites.

[Morphologic characteristics of disorders of liver function in myasthenia].

Clinico-morphological study of the liver was carried out in 54 patients with generalized severe myasthenia of 1 1/2-2-year duration. The functional values of the liver were studied before thymectomy and on the 1st post-operation day. Eight liver punctates were examined histologically and electron-microscopically. A picture of nonspecific reactive hepatitis whose clinical manifestations included hyperbilirubinemia, and disorder of the absorbing and excreting functions of the liver was revealed. Ultrastructural signs of cholestasis were found. The observed fibrosis of the organ was accompanied by hypertrophy and hyperplasia of lipocytes but without their manifest transformation into lipofibroblasts . Hypoplasia of hepatocytes was observed in myasthenia which may be due to their thymus-dependent inhibition. The mechanism of disturbance of pigment metabolism in the liver of patients with myasthenia is multi-component and may be associated with disorders in bile evacuation, hematotissue barrier, and probably, structural-functional incompetence of hepatocytes.

Hepatitis↗

[Clinico-morphological characteristics of nonspecific reactive hepatitis].

Nonspecific reactive hepatitis is a manifestation or complication of various diseases, being a variety of secondary hepatitis. Its morphological characteristics are identical in all cases comprising hydropic and fatty degeneration of hepatocytes, their focal necroses in different parts of the lobules, predominantly macrophage and leukocyte infiltration of portal and intralobular stroma. Different morphological types of nonspecific reactive hepatitis are distinguished with regard to location of inflammatory lesions in different parts of the lobules and to their degree of activity. Morphological differential diagnosis of nonspecific reactive hepatitis from chronic hepatitis of viral, drug, and alcohol etiology is described. However, problems of morphogenesis, mechanism of development and differential diagnosis of this kind of chronic liver inflammation have not been studied sufficiently as yet.

[Differential morphological diagnosis of chronic liver diseases based on the results of puncture biopsy].

The possibility of differential diagnosis of chronic liver diseases from investigations of puncture biopsy material is discussed basing on analysis of literature data. The present-day classification of chronic hepatitis, liver cirrhosis, and morphologic criteria their differential diagnosis are described. Analysis of the liver punctate allows one to reveal the nature of the pathologic process, its etiology, and the degree of activity. Of primary importance is the differential diagnosis of the most frequent etiologic forms of chronic liver diseases (hepatitis, cirrhosis), i.e. viral, alcoholic, and medicamentous affections.

[Experience in conducting a test exam in pathological anatomy].

Taking into consideration the experience in examinations at medical schools of various countries and personal experience in conducting examinations by the test method, the authors believe that the test method should be used at examinations in order to assess the knowledge of the students objectively, particularly, when the number of students to be examined is large. This method is sufficiently effective, time saving, permits one to correlate the results of teaching with the tasks set, to detect defects in studying some parts of the course and to correct the process of teaching. It should be used only partially, however, to examine the knowledge of the theoretical part of the course, whereas the practical part should be presented at oral exams allowing personal contacts of the teachers and the students.

[Hepatitis and cirrhosis of the liver (classification, morphology and morphological differential diagnosis].

The current classification of hepatitis and hepatocirrhosis is evaluated on the basis of the literature analysis and the study of the author's own material. Morphological criteria for differential diagnosis of liver diseases of viral and alcohol etiology are proposed. A clinical pathologist examining a liver biopsy may confirm or establish the etiology of the disease, determine the degree of activity and compensation of the process. An analysis of these criteria helps a clinician to choose the method of treatment and to determine the prognosis of the disease.

Chemical and Drug Induced Liver Injury↗

[Morphological characteristics of acute viral hepatitis].

From the literature and their own observations the authors present morphological criteria of acute viral hepatitis which may be used for differential diagnosis from hepatites of other etiology (toxic, drug). Examinations of liver biopsies from patients with viral hepatitis permit to establish the stage of the disease and to predict its possible outcomes.

Acute Disease↗

[The morphologic basis for the mechanism of increased serum acid phosphatase activity in patients with viral hepatitis].

The activity of acid phosphotase in the blood serum and punctates of the liver was studied in 49 patients with acute cyclic form of viral hepatitis of slight and medium-grave course with the use of histological and histochemical methods. The degree of manifestation of necrotic changes in the punctate tissue was determined with the help of the stereometric method. As a result of the studies conducted, it was established that the degree of hyperfermentemia was the highest at the period when the disease was in full swing, and that it did not depend on the form of its course and extent of necrotic changes observed in the liver punctates. The authors consider a morphological evidence of high activity of acid phosphotase in the blood serum aggregations of large hepatocytes, cytoplasm of which was loaded with mature lipofuscin, the topographic identity of the latter with acid phosphotase at present being proved.
