Leadership transitions and the impact on patient care.

Leadership transitions are increasing dramatically in hospitals, both at the very top and in top roles such as nursing, operations, finance, medical staff, and so on. Increasingly, new leaders will articulate the importance of "customer orientations" as one of their strategic themes because of the importance of being more market-driven in today's environment. This article explores the strengths and potential pitfalls in both articulating a theme about becoming more patient-centered and implementing that theme in a complex organization so that it actually shapes transactions between front-line staff and patients. The article closes with a discussion of the dilemmas of sustaining a complex initiative across time when many of the key leaders in an organization will inevitably be changing during that time.

Managing complexity in health care settings.

Health care settings are becoming ever more complex. With this increased complexity, however, people often become less clear about their responsibilities and accountability. In this article, the authors discuss the process of responsibility charting and illustrate how it can be used to clarify the roles of the many different people whose work may be affected by the implementation of new programs and procedures. Responsibility charting is a structured process for establishing sound patterns of communications and collaboration over time.

Diagnosing organizational decision making through responsibility charting.

Decision making within many organizations is becoming increasingly complex and difficult to manage. This article describes a technique called "Responsibility Charting" which can be used as a basis for understanding and improving decision making, and for enhancing the quality of communication within an organization. The authors present a case study to illustrate the types of analyses possible with this technique.

Effective leadership during organizational transitions.

Leadership transitions offer high leverage opportunities to improve an organization's leadership and overall direction. If managed in ways that link strategic thinking, active executive search, and careful organizational development, leadership transitions can advance the purposefulness of the organization.
