The relationship between urinary Tamm-Horsfall glycoprotein excretion and urinary activity of glycosidases in normal pregnancy and pre-eclampsia.

Electrophoretic analyses of the urinary proteins of pre-eclamptic patients revealed a decrease in the staining intensity of the protein band representing the Tamm-Horsfall glycoprotein (THP). In the present study the quantitative analysis of the THP excretion rate and the urinary activity of THP oligosaccharide metabolizing glycosidases were investigated. The median THP excretion rate of non-pregnant women (n = 24) was 20 mg/g creatinine (crea.). An increase in the THP excretion rate was seen in pregnancy to a level between 43 mg/g crea. (II. trimester) and 32 mg/g crea. (III. trimester) (n = 29). Hypertension in pregnancy was associated with a decrease in the THP excretion rate to 9 mg/g crea. (n = 85). Post partum, a transient elevated THP excretion rate up to 109 mg/g crea was recorded in the group of hypertensive patients. The urinary activities of the lysosomal beta-mannosidase, alpha-fucosidase, alpha-mannosidase (pH 4.5) and beta-galactosidase increased in normal pregnancy. This effect was most pronounced in the beta-galactosidase activity which increased from 50 U/mg crea. before pregnancy to 280 U/mg crea. at term. Hypertension in pregnancy was associated with a further increase in the activities of the lysosomal glycosidases. In the case of the beta-galactosidase a significant rise from 68 to 310 U/mg crea. was found. The urinary activity of the alpha-mannosidase (pH 5.5) originating from the Golgi apparatus was only elevated in patients with severe pre-eclampsia. Casuistic post partum recordings demonstrated that an elevation of the lysosomal glycosidases activities was followed by a transient increase in the THP excretion rate.

Urinary protein electrophoresis profile in normal and hypertensive pregnancies.

By using of modified urine preparation and a highly sensitive SDS-polyacrylamide gel electrophoresis (SDS-PAGE) we determined the urinary protein profile in 21 healthy males, 25 healthy females, 64 patients with uncomplicated pregnancy and 110 hypertensive pregnant women. The urinary protein patterns were similar in controls and in women with a normal pregnancy. There were no increase in the number of protein bands from the 1st trimester to term, and the electrophoresis pattern did not change in the postpartum period. In both groups, an intensively stained protein band with an apparent molecular weight of 105 kD was detected. The 105 kD band was significantly reduced or completely absent in 91 (83%) out of 110 hypertensive pregnant women. The urinary protein electrophoresis profile correlated significantly with the severity of the disease. The 105 kD band disappeared just before or simultaneously with the onset of clinical symptoms in 18 out of 32 hypertensive pregnant women followed throughout pregnancy. Postpartum the 105 kD in urine reappeared at 2 to 14 days after delivery in 49 of the 53 patients. Using a silver staining and Western blot, the 105 kD band was identified as Tamm-Horsfall protein, which is identical to the immunosuppressive glycoprotein uromodulin. The findings in the SDS-PAGE may reflect a transitory tubular dysfunction in cases of pre-eclampsia, which is usually reversible after delivery. The results of our study support the hypothesis of an immunological basis for this disorder.

[Changes in Tamm-Horsfall protein excretion in the urine of patients with hypertension in pregnancy].

In previous studies we detected an intensively stained protein band with an apparent molecular weight of 105 kD in the urinary electrophoresis pattern of healthy control subjects and pregnant women; this protein band was identified as Tamm-Horsfall protein by using Western blot. The aim of the present study was to determine the changes of this THP band in 64 patients with an uncomplicated course of pregnancy and 135 hypertensive pregnant women by use of a special urine preparation and a modified SDS-PAGE. The number of protein bands did not change in the course of pregnancy and in the post partum period. The THP band was constantly found in all these patients independent of the gestational age. In 83% of the hypertensive pregnant women we observed a marked reduction in intensity or complete disappearance of the THP band. These changes correlated significantly with the severity of the disease. Follow-up analyses in 39 of these patients showed, that in 22 cases disappearance of the THP band occurred simultaneously with and in 11 women before clinical manifestation of the disease. In 81 of the 85 hypertensive pregnant women complete reappearance of the THP band was observed 2 to 14 days after delivery. Our findings may reflect a transitory tubular dysfunction in cases of preeclampsia and support the hypothesis of an immunological pathogenesis of the disease.

[Changes in the urinary protein pattern in SDS polyacrylamide gel electrophoresis in normal and hypertensive pregnancies].

The urinary protein pattern was determined in 46 healthy male and female subjects, 64 patients with an uncomplicated course of pregnancy and 88 hypertensive pregnant women by use of a special urine preparation and a modified SDS-polyacrylamide gel electrophoresis (SDS-PAGE). There were no differences in the protein electrophoresis pattern between the control subjects, independent of sex and age, and the pregnant women. The number of protein bands did not change in the course of pregnancy and in the post partum period. In both groups, an intensively stained protein band with an apparent molecular weight of 105 kD was detected. In 71 of the 88 hypertensive pregnant women we found a marked reduction in intensity or complete disappearance of the 105 kD protein band. Follow-up analysis in 30 of these pregnant women showed, that in 24 cases disappearance of the 105 kD band occurred simultaneously with and in 6 women before clinical manifestation of the disease. There were no differences in the protein electrophoresis pattern between patients with preexisting renal or hypertensive disease and hypertensive women without a complicated history. In 49 of the 52 hypertensive pregnant women complete reappearance of the 105 kD band was observed 2 to 14 days after delivery. By using silver staining and Western blot, the 105 kD band was identified as Tamm-Horsfall protein. Our findings may reflect a transitory tubular dysfunction in cases of preeclampsia and support the hypothesis of an immunological pathogenesis of the disease.

Pharmacokinetics of morphine in striatum and nucleus accumbens: relationship to pharmacological actions.

The pharmacokinetics of morphine was compared with its ability to increase striatal dopamine turnover (estimated by an increase in DOPAC concentration) and to produce the development of a muscular rigidity (estimated as a tonic activity in the electromyogram). After systemic administration of morphine (15 mg/kg IP), the concentration of morphine in blood plasma, striatum and substantia nigra showed a parallel time course with a maximum after 30 min; in the striatum, in addition, normorphine was found in a lower concentration, but with a similar time course. The elevation of striatal DOPAC, in contrast, commenced very rapidly and lasted for about four hours. The rigidity appeared later and disappeared earlier than the striatal DOPAC elevation. After unilateral intrastriatal injection of morphine (15 micrograms), a small amount of the drug penetrated very rapidly to distant sites, such as the contralateral striatum and nucleus accumbens, as well as to the ipsilateral nucleus accumbens. The results suggest that the relationship between pharmacokinetics and pharmacodynamics of morphine, both after systemic and after local injection into the brain, is more complex than could be expected from previous findings.

Adrenaline: occurrence in the bovine retina.

Using high performance liquid chromatography (HPLC) and electrochemical detection, a substance corresponding to adrenaline was identified in crude perchloric acid extracts from bovine retina. The substance did not originate from exogenous contamination such as blood. Retinal tissues also contained phenylethanolamine-N-methyl-transferase (PNMT) activity and the retinal enzyme had a similar substrate specificity as purified bovine PNMT. The pigment epithelium/choroid complex also had PNMT activity and an analysis of extracts by HPLC revealed the presence of small amounts of adrenaline. The results suggest that adrenaline is of functional importance in the bovine retina.

Is noradrenaline a major catecholamine in the bovine retina?

Using high performance liquid chromatography (HPLC) and electrochemical detection, a major substance corresponding to noradrenaline was identified in crude perchloric acid extracts from bovine retina. Prior purification of the extract with aluminium oxide followed by HPLC confirmed this observation. By altering the oxidation potential of the detector, noradrenaline was confirmed by a further criterion. Bovine retina was also shown to metabolize [3H]dopamine to form a substance which was identified as noradrenaline by the same criteria used to establish its presence. The noradrenaline content was found to be 62 +/- 4 ng/g fresh weight of tissue. Its level is therefore of the same order as the dopamine content (86 +/- 3 ng/g fresh weight).

Serotonin-containing neurones in vertebrate retinas.

It has been established by a combination of HPLC and electrochemical detection that frog, lizard, goldfish, rabbit, and bovine retinas contain both dopamine and serotonin. Immunohistological and immunoradiographical methods show that serotonin is localised in amacrine perikarya and processes situated in the inner plexiform layers of frog, lizard, and goldfish retinas. The amount of serotonin in the mammalian retina appears to be too low for detection in neurones. The serotonin in the bovine retina is located mainly in the inner nuclear and plexiform layers, suggesting that the amine is present in the same types of cells as found for frog, lizard, and goldfish retinas. Retinas incubated in [3H]serotonin showed that radioactivity is associated with processes in the inner plexiform layer and amacrine perikarya. These results suggest that the neuronal elements that contain endogenous serotonin also have the capacity to accumulate exogenous amine and are consistent with the opinion that serotonin has a neuronal function in retinas of a variety of vertebrates.

[Changes in the basic immunologic parameters interleukin-2 receptor and CD8-antigen in pre-eclampsia].

The serum level of interleukin-2 receptor (sIl-2R) and CD-8 antigen were determined as basic parameter in terms of the stimulation of the t-cell mediated immune reaction in patients suffering from pre-eclampsia (n = 21), HELLP syndrome (n = 12) and pre-existing hypertension (n = 10). The data were compared with those of healthy nonpregnant female volunteers (n = 10) and normotensive pregnant women in the course of pregnancy. Hypertension in pregnancy was associated with an increase of sIl-2R and sCD-8 levels depending on the severity of the hypertensive disorder. In the group of patients with severe pre-eclampsia the median of sIl-2R serum level increased from 370 U/ml (normotensive patients) to 730 U/ml and that of sCD-8 from 640 U/ml to 1100 U/ml. In patients with pre-existing hypertension there was a smaller increase of sIl-2R (median 520 U/ml) and decrease of sCD-8 (median 520 U/ml). 6 to 8 weeks after delivery in the group of hypertensive patients the sIl-2R levels were normalized and the levels of sCD-8 were elevated (median 1030 U/ml). The results support the hypothesis of T-cell mediated immunological reactions as pathogenetic factors of the genesis of the hypertensive disorders in pregnancy.
