Surgical adjuvant chemotherapy: results with one short course with cyclophosphamide after mastectomy for breast cancer.

One single six-day course with cyclophosphamide (total dose 30 mg/kg) was given immediately after mastectomy to 507 breast cancer patients, with 519 randomized controls receiving no adjuvant chemotherapy. The control group now has 234 recurrences and 196 deaths, and the treatment group 175 recurrences and 146 deaths. The differences of 59 recurrences and 50 deaths in favour of the treatment group are significant with p values less than 0.001 and less than 0.01 respectively. The differences in recurrence rates increased gradually, reached 10.71% four years after mastectomy (p less than 0.001), and remained at the same level for another 6 years. The differences in death rates increased until 6 years after mastectomy, and was 10.48% after 10 years. With this pattern, the mechanism is probably not a delay in onset of clinical recurrences, but a definite reduction of recurrence rates due to tumoricidal chemotherapy. Prognostic factors or menstrual state had apparently no influence on the effect of this type of adjuvant chemotherapy. Side effects were of short duration and very moderate. Since there was a good effect in the prognostically most favourable groups of patients, treatment of such cases seems therefore also justified. The same chemotherapy course given 3 weeks after mastectomy seemed without effect.

Radiation therapy in Burkitt's lymphoma of the central nervous system.

Involvement of the central nervous system in patients with with Burkitt's lymphoma was treated by superfractionated irradiation. The primary effect in 20 patients is reported. In 3 of 5 patients with paraplegia, complete regression was achieved. Only one patient of 6 with cranial neuropathy and irradiated to the brain improved. In 8 patients with either cranial neuropathy or other neurologic symptoms of signs, the brain and spinal cord were irradiated. Three of these patients, with recurrences after previous intrathecal chemotherapy, responded with complete regression.

Cerebrospinal irradiation of Burkitt's lymphoma. Failure in preventing central nervous system relapse.

Twenty-two patients with Burkitt's lymphoma in complete remission induced by either cyclophosphamide or a combination of cyclophosphamide, oncovin and methotrexate were randomized to receive or not to receive prophylactic cerebrospinal irradiation. Six of 11 irradiated patients relapsed with tumour of the central nervous system as compared to 4 of 11 controls. Relapse frequency appeared to be related to stage of disease on admission. It is concluded that irradiation does not prevent relapse.

Radiation therapy in Burkitt's lymphoma. Long term results.

In 40 of 61 irradiated patients with Burkitt's lymphoma the long term results were evaluated. Five patients were primarily irradiated while the remaining patients had late recurrences or were chemotherapy failures. Seven patients were alive and free of recurrence after 6 months and 4 after 24 months.

Radiation therapy of nasopharyngeal carcinoma in East Africa.

Results of irradiation, 1 to 3 years after treatment, are presented in 64 cases of nasopharyngeal carcinoma in East Africans. Two types of anatomic point indices are presented. Both correlate well with the results. Prophylactic irradiation of the mediastinum and both axillae was not followed by any evident improvement of the results.

Mass-screening of a female population for detection of early carcinoma of the breast.

In 1969-1970 a breast mass-screening was undertaken in Gävle, with thermography and questionnaire. Mammary radiography and clinical examination of subjects with abnormal screening reports and of a group of normal subjects were performed. In spite of having achieved early detection of malignant tumours in many cases with a preponderance of carcinoma cases in the abnormal group, thermography was found to have considerable drawbacks as a screening method.
