[Left ventricular rupture following the procedure of percutaneous transluminal coronary angioplasty: a case report].

A 50-year-old man with 90% concentric stenosis at the proximal portion of the left anterior descending artery was tried to be treated with percutaneous transluminal coronary angioplasty (PTCA). The guide wire did not pass through the stenotic portion, and the staining out of coronary artery was recognized. Four hours after the procedure, he fell to shock status with a sign of electro-mechanical dissociation. An emergent operation confirmed the left ventricular rupture. Repair of the tear and saphenous vein bypass to the left anterior descending artery were performed rapidly, however, he was not weaned from cardiopulmonary bypass. Close communication between cardiologist and surgeon, and immediate surgical back up are essential for saving these patients from severe ischemic events following PTCA failure.

[Free arterial graft in coronary artery bypass surgery].

Free arterial graft of the internal mammary artery (IMA) and the gastroepiploic artery (GEA) has been utilized for coronary artery bypass grafting in 10 patients during 44 months period. There were 6 males and 4 females and age ranged 42 to 73 year old with the mean of 60.8 year old. Eight IMA and 2 GEA were used as a free graft. Sites of distal anastomosis of the free graft were 3 at anterior descending arteries, 3 at diagonal branches and 4 at circumflex arteries. Sites of proximal anastomosis of those grafts were ascending aorta in one, concomitantly utilized saphenous vein graft in 5 and in situ IMA graft in 4 patients. Mean number of grafts was 2.9 (2-5) and mean aortic cross clamp time was 56.2 minutes (16-90 minutes). There was neither operative death, nor perioperative myocardial infarction and intra-aortic balloon pumping was not required. Postoperative angiography was made in 9 patients within 3 postoperative months. Eight (89%) free arterial grafts were patent. Relief of angina was obtained in all patients. We concluded that the complete revascularization with only arterial graft can be achieved more widely by utilizing the free arterial graft with an acceptable patency.

[Results of sequential aorto-coronary bypass grafting using saphenous vein graft].

Seventy patients underwent sequential coronary artery bypass grafting with saphenous vein during 28 months period. Seventy eight sequential grafts comprising 165 distal anastomoses were performed. Early death was 2.9%. Postoperative angiographic evaluation within 6 months showed 86% patency of the 65 sequential grafts and 89% patency of the 136 distal anastomoses. The patency of anastomosis to left anterior descending artery (LAD), diagonal branch (Dx), obtuse marginal branch (OM), posterior lateral branch (PL), and posterior descending branch (PD) was 87% (13/15), 92% (36/39), 91% (40/44), 81% (26/32) and 100% (6/6), respectively. The patency of Dx-LAD, Dx-OM and OM-PL was 86% (12/14), 94% (15/16) and 76% (13/17), respectively. There was no significant difference in the patency rate according to the site of anastomosis and the inner diameter of the coronary artery. The patency of side to side anastomoses was 92% (65/71) which was not significantly different from that of the end to side anastomoses; 86% (56/65). These results showed sequential aorto-coronary vein grafting means to be effective for the complete coronary artery revascularization .

High-resolution electron microscopy study on crystal structures of high-Tc superconductors.

Recent studies of high-resolution electron microscopy on the high-Tc superconductors of Y-Ba-Cu-O and Bi-Ca-Sr-Cu-O are presented. The observed images of crystals thinner than 3 nm, taken under conditions that approached the Scherzer defocus condition, directly show the arrangements of cations and oxygen-vacant positions. The results reveal structural characteristics of the atomic scale; this offers important insights into the origin of the high-Tc superconductivity. The usefulness of high-resolution electron microscopy for studying complicated crystal structures is demonstrated for the high-Tc oxides.

Utilization and excretion of a new sweetener, fructooligosaccharide (Neosugar), in rats.

In order to study the digestibility of the fructooligosaccharide "Neosugar," [U-14C]Neosugar or [U-14C]sucrose was orally administered to germfree, conventional and antibiotic-treated rats and the radioactivities of expired 14CO2, urine and feces were determined 24 h later. More than 50% of the Neosugar was expired as CO2 in conventional rats. This was the same as for sucrose, but the time course was delayed by about 2 h. In germfree rats, no 14CO2 was released for the first 8 h, and 14CO2 released after 8 h probably reflected bacterial colonization of the gut. The radioactivity of the urine was about 3-4% in all groups, but that of the feces from germfree rats was about eight times higher than the level in conventional rats. When [U-14C]Neosugar was anaerobically incubated with the cecal contents of conventional rats, more than 10% of the added Neosugar was metabolized to CO2, about 66% to volatile fatty acids and about 7% to microbes. More than 58% of 1-14C-volatile fatty acids such as acetic acid, propionic acid or butyric acid injected directly into the cecum of conventional rats was excreted as CO2 within 24 h. These results indicate that Neosugar given orally to rats is metabolized mainly to volatile fatty acids and CO2 by intestinal microorganisms, and the volatile fatty acids produced are absorbed and further converted to CO2 in the body. Thus, the data indicate that Neosugar is partially utilized as an energy source.

Congenital cutis laxa.

We have encountered 2 cases (mother and son) with an autosomal dominant form of cutis laxa, which is clinically characterized by inelastic, loose, and pendulous skin without systemic organ involvement. The histopathological findings disclosed that the elastic fibers were decreased in number and had abnormalities in shape. Furthermore, electron microscopic observations showed some variations in structural abnormalities of the elastic fibers between the 2 cases. These findings may suggest that the elastic fibers seen in congenital cutis laxa give various features for elastogenesis and elastolysis in the individual cases, perhaps due to aging.

A case of successful coronary artery bypass grafting in a patient with angina pectoris and hypothyroidism.

There is a clinical dilemma in the treatment of patients with hypothyroidism and coronary artery disease; excess thyroid hormone administration may exacerbate anginal symptoms, and yet inadequate thyroid replacement may induce congestive heart failure. A case of successful coronary artery bypass grafting in a patient with angina pectoris and hypothyroidism is described in this paper. A 55 year-old woman with this complication initially received thyroid replacement therapy under strict monitoring. Forty days after the start of the thyroid replacement therapy, serum levels of thyroid hormone had reached the normal range, and then coronary artery bypass grafting was successfully performed. She recovered without any complications, and is now free from chest pain in spite of thyroid replacement therapy.

Effect of FK973, a new antitumor antibiotic, on the cell cycle of L1210 cells in vitro.

Our previous study showed that FK973 (11-acetyl-8-carbamoyloxymethyl-4-formyl-14-oxa-1,11- diazatetracyclo[7.4.1.0(2,7)0(10,12)]tetradeca-2,4,6-trien-6 ,9-diyl diacetate), a novel substituted dihydrobenzoxazine, which is a derivative of the fermentation product of Streptomyces sandaensis No. 6897, had strong antitumor effects on experimental tumors in vitro and in vivo. In this report, we investigated its effect on the cell cycle of murine leukemia L1210 cells in vitro by means of DNA/5-bromo-2'-deoxyuridine double staining and compared these effects with those of other antitumor drugs. Both FK973 and mitomycin C arrested the cells in the G2 phase. Vinblastine arrested the cells in the M phase and cytosine arabinoside, in the G1 phase. Although FK973 and mitomycin C were shown to act on the cell cycle in a similar way, FK973 was slower in producing its effect. From the results, FK973 arrests the cells in the G2 phase, and it appears that FK973 must be converted into the activated form in the cells for the development of its antitumor effects.

[Study on the masticatory muscle and occlusal function of temporomandibular joint dysfunction syndrome patients in adolescence].

The aims of this study was to clarify the role of masticatory and occlusal malfunction in causing the Temporomandibular joint (TMJ) Dysfunction Syndrome in adolescents. We examined the relationships between masticatory and occlusal malfunction and signs of the TMJ dysfunction by using the number of interocclusal points, occlusal interference, the dental arch models and the electromyographs. In particular, the electromyographs was examined by comparison between with at the first examination and after the adjustment of occlusal relations. The subjects were thirty-nine out patients with the TMJ Dysfunction Syndrome. The following results were obtained: 1) The number of interocclusal points in the TMJ Dysfunction Syndrome was fewer than in the normal control group. 2) In the relationships between the occlusal interference on the non-working side and pain as signs of TMJ Dysfunction, 76.9% in all subjects had reasonable relationship which recognized a causal sequence. 3) By analysis using integrated method for electromyographs, it was found that the total electric potentials in TA after the adjustment of occlusal relations tended to decrease in comparison with at the first examination, while, the ration of M was shown to have a tendency of increasing. 4) On comparing the gum chewing patterns (duration, interval, cycle) recorded from the masseter muscle, it was found that the pattern after the adjustment of occlusal relations were more stable than at the first examination. 5) The frequency of occurrence of the silent period after the adjustment of occlusal relations was higher than at the first examination. Also relationships between the change on duration of the silent period and signs of TMJ Dysfunction were found.

[Trends of the patients with malocclusion on dental clinic of pediatric dentistry].

The factors which determine the situation of continuation in malocclusion treatment and characteristic of patient with malocclusion in pediatric dentistry were estimated from the status and the trend of patients undergoing denture guidance at the outpatient clinic for pediatric dentistry. The following results were obtained. 1. Among the patients undergoing denture guidance, a majority were females. Among the different types of malocclusion, the reversed occlusion was the most frequent, being 54.3% among all malocclusions. 2. In particular the age for first visit was from under 3 years of age with reversed occlusion, and at this age, females were more numerous than males. 3. The first examination for denture guidance was related to the time which made effective diagnosis possible, and the patients with reversed occlusion had their first examinations consistently from 3 years of age, and the patients with crowding had their first examinations from 6 years of age to 11 years of age. 4. From the difference between the age of first visit and first examination, it was shown that the patients with reversed occlusion did not have their first examination under 3 years of age and for the patients with crowding who visited first at 3, 4 and 5 years of age, the first examination was performed after 6 years of age. The patients with crowding in the group who came for consultation by reference were later about two years compared with other two groups in the difference of motivation for visiting. 5. It is suggested that the factors which had an influence upon the situation of continuation of malocclusion treatment were; an existence of the use of an appliance, a substantial program of correspondence in regard to dental care for malocclusion before malocclusion treatment and concrete indication of the treatment.

[Clinical experience of bovine internal mammary artery graft (BIOFLOW) for coronary artery bypass surgery].

Bovine internal mammary artery graft (BIOFLOW: BIOVASCULAR INC. Netherlands) has been used for coronary artery bypass grafting (CABG) in 8 patients since May 1988 at Osaka Medical College Hospital. There were 4 males and 4 females. Mean age was 66.9 year-old ranged from 62 to 72. Two were emergency CABG and other six had inadequate saphenous veins. The internal mammary artery and/or the saphenous vein were concomitantly used in all patients. Number of grafts was 2 to 6 with the mean of 2.9 and BIOFLOW (phi 3 mm) was anastomosed to 2 left anterior descending arteries and 6 right coronary arteries, the mean graft flow of BIOFLOW was 72.5 ml/min ranged from 52 to 120 ml/min. One patient died of ventricular arrhythmia on 5 postoperative day. There was no new Q wave infarction. Postoperative angiogram within 2 months showed 4 of 5 BIOFLOW grafts were patent, and no stenosis was found in those patent grafts. All survivors returned to home with a freedom from angina. From our experience, although the number was small and the follow-up period was short, BIOFLOW can be expected as an conduit of choice for CABG when the saphenous vein was not available and the internal mammary artery and the gastroepiploic artery were not enough to accomplish the complete revascularization.

[Three cases with temporomandibular joint dysfunction syndrome in adolescents whose mandibular are asymmetrical].

We encountered three patients with TMJ dysfunction syndrome in adolescents whose mandibulars were asymmetrical. In two cases, we tried to treat with EMG Biofeedback therapy in order to improve a form and function. The following results were obtained from the progress and result of the treatment. Asymmetry of mandibular causes the functional disharmony of masticatory muscle and it is probable that its asymmetry causes TMJ symptoms. These symptoms are improved with an occlusal splint, occlusal adjustment and occlusal reconstruction. The form does not change with the EMG Biofeedback therapy which is used in order to activate the masseter muscle. The function changed but was not stable.

There are no reports with regard to fluctuation of signs concerning the TMJ dysfunction syndrome in adolescents. Concerning this problems, we examined 145 children (70 boys and 75 girls) of junior high school age and 429 children (202 boys and 227 girls) of senior high school age for 2-years (total 3-years) longitudinally. The results obtained were as follows: 1. The incidence of the TMJ dysfunction syndrome involved 10.3% in 1984, 15.9% in 1985, 15.9% in 1986 of junior high school age students and 11.7% in 1984, 18.4% in 1985, 31.0% in 1986 of senior high school age students. 2. The incidence of the individual that at least once during the 3-years period showed some or multiple signs of TMJ dysfunction syndrome was 31.0% of junior high school age students and 39.8% of senior high school age students. The incidence of the individual that had shown no signs of the TMJ dysfunction syndrome at all during the 3-year period was 69.0% of junior high school age students and 60.4% of senior high school age students. 3. With regard to individual fluctuation of signs of the TMJ dysfunction syndrome, there was a total of 2.8% of junior high school age students and 5.1% of senior high school age students, signs being recorded for all 3-years. In 28.3% of junior high school age students and 34.5% of senior high school age students, signs fluctuated during the entire 3 years.

[Primary torsion of the omentum--report of a case and review of the Japanese literature].

A case of primary torsion of the omentum is reported. A 37-year-old male was admitted with the chief complaint of RLQ pain of 3 day's duration on June 10, 1986. Tenderness and Blumberg's sign were found at Mc Burney's point with leucocytosis of 11,500/mm3. His body temperature was not elevated. Operation was carried out under the diagnosis of suspicion of acute appendicitis. Laparotomy revealed a dark-red omental mass of 12 X 5 X 1 cm which had been formed by torsion (7 times, clock-wise) and a little clear ascites. The omental mass was resected, and appendectomy was also performed. The postoperative course was uneventful. Eighteen cases including this case in the Japanese literature were reviewed and reported.

[Internal mammary artery grafting to the circumflex coronary artery].

The in-situ left internal mammary artery (LIMA) was anastomosed to the circumflex coronary artery (Cx) in 20 patients. The right internal mammary artery, the saphenous vein and the right gastroepiploic artery were also utilized to bypass the other coronary arteries. Sequential LIMA grafting to the diagonal branch and CX was performed in 2 patients. The sites of LIMA anastomosis were 15 obtuse marginal branches and 5 posterior lateral branches. All LIMA-Cx anastomoses were performed with single 8-0 polypropylene continuous suture technique. Mean number of distal anastomosis was 3.1 ranged from 2 to 4. Mean aortic cross clamp time was 68.5 minutes ranged from 42 to 87 minutes, and mean cardiopulmonary bypass time was 116.6 minutes ranged from 73 to 167 minutes. One patient died of renal failure at 22nd postoperative day, and the other patients were alive with relief of angina. New Q wave was noted in 1 patient. Postoperative angiogram at mean 2.0 months showed 100% patency of LIMA in restudied 12 patients. We concluded that the in-situ LIMA grafting to Cx system can be done with acceptably low mortality and excellent patency rate, and its utilization is particularly desirable in younger patients.
